"Pengkajian resep adalah hal yang perlu dilakukan untuk melihat apakah pengobatan yang dilakukan bersifat rasional. Hal ini perlu diperhatikan khususnya pada pasien jantung koroner dan hipertensi karena pasien umumnya mengkonsumsi jumlah obat yang cukup banyak. Pengkajian resep dilakukan di Apotek Kimia Farma 348 Dramaga pada periode bulan Januari – Maret 2023. Telaah dilakukan dengan memperhatikan tiga aspek, yaitu telaah administratif, farmasetik, dan klinis pada 20 resep. Berdasarkan hasil telaah yang dilakukan, didapatkan hasil, yaitu pada aspek telaah administratif, terdapat beberapa syarat yang belum terpenuhi, seperti kurangnya informasi mengenai jenis kelamin, berat badan, dan usia pasien yang dapat diatasi dengan melakukan konfirmasi kembali pada rumah sakit dan dokter atau pasien yang bersangkutan; pada aspek telaah farmasetik memenuhi seluruh syarat; dan pada aspek telaah klinis terdapat beberapa syarat yang belum terpenuhi, seperti adanya polifarmasi dan kemungkinan interaksi obat. Permasalahan ini dapat diatasi dengan memberikan edukasi pada pasien untuk memberikan jeda konsumsi obat serta memberikan informasi mengenai pentingnya ketepatan waktu konsumsi obat untuk mendapatkan efek terapi yang maksimal.

Prescription review needs to be done to see whether patient’s treatment is rational. This needs to be considered especially in patients with coronary heart disease and hypertension because patients generally consume a large number of drugs. A prescription review was carried out at Kimia Farma 348 Dramaga Pharmacy in the period January - March 2023. The review was carried out by paying attention to three aspects, namely administrative, pharmaceutical and clinical reviews on 20 recipes. Based on the results of the review carried out, the results were obtained, namely the aspect of administrative review, there were several conditions that had not been fulfilled, such as a lack of information regarding the patient's gender, weight, and age which could be overcome by confirming to the hospital, doctor, or patient; pharmaceutical review aspect meets all requirements; and in clinical aspect there are several requirements that have not been fulfilled, such as the existence of polypharmacy and the possibility of drug interactions. This problem can be overcome by educating patients to pause in taking drugs and providing information about the importance of taking medication on time to get the maximum therapeutic effect."

"Penyakit jantung koroner merupakan penyakit yang disebabkan oleh adanya penumpukan atau pembentukan plak pada dinding arteri yang berfungsi untuk mensuplai darah ke jantung sehingga dapat muncul nyeri hingga serangan jantung. Terapi yang digunakan untuk jantung koroner bertujuan untuk mengurangi atau mencegah nyeri dada, mengurangi tekanan darah, memperlebar arteri, hingga mengelola kondisi medis yang berkontribusi pada penyakit jantung koroner. Apoteker memiliki kewajiban untuk memastikan terapi yang digunakan khususnya untuk penyakit jantung koroner dan tidak terbatas pada penyakit lainnya sesuai dan tidak terjadi medication error. Berdasarkan hal tersebutlah pada penelitian ini dilakukan salah satu cara mencegah medication error, yaitu dengan mengkaji resep pasien dengan penyakit jantung koroner. Penelitian ini dilakukan dengan memilih resep pasien jantung koroner kemudian dilakukan pengkajian resep meliputi administratif, farmasetika, hingga aspek klinis. Berdasarkan pengkajian resep yang telah dilakukan, terdapat beberapa aspek yang belum sesuai yaitu terkait dengan alamat pasien, berat badan pasien, polifarmasi, dan interaksi obat. Namun, ketidaksesuaian dari aspek-aspek tersebut masih dapat diterima. Informasi pasien terkait dengan berat badan dapat dikonfirmasi kepada pihak rumah sakit jika dibutuhkan. Aspek terkait dengan polifarmasi dapat diterima karena pengobatan yang digunakan bertujuan untuk mengatasi penyakit penyerta dan atau untuk meningkatkan efek terapi. Sedangkan untuk interaksi obat yang terjadi dapat dihindari dengan penggunaan obat yang dijeda.

Coronary heart disease is a disease caused by the buildup or formation of plaque on the walls of the arteries which function is to supply blood to the heart. The plaque can cause chest pain up to a heart attack if left untreated. Therapy used for coronary heart disease aims to reduce or prevent chest pain, reduce blood pressure, widen the arteries, and manage medical conditions that contribute to coronary heart disease. Pharmacists have an obligation to ensure that the therapy used, specifically for coronary heart disease and not limited to other diseases, is appropriate and medication errors do not occur. Based on this, this research conducted one way to prevent medication errors, namely by reviewing the prescription of patients with coronary heart disease. This research was conducted by selecting prescriptions for coronary heart patients and then reviewing prescriptions covering administrative, pharmaceutical, and clinical aspects. Based on the prescription review that has been done, there are several aspects that are not suitable, namely related to the patient's address, patient's weight, polypharmacy, and drug interactions. However, the discrepancy of these aspects is still acceptable. Patient information related to body weight can be confirmed to the hospital if needed. Aspects related to polypharmacy are acceptable because the treatment used aims to overcome comorbidities and or to increase the effect of therapy. Meanwhile, drug interactions that occur can be avoided by separating the consuming time of each drug"

"Sebagai seorang apoteker, salah satu cara untuk memastikan pasien telah mendapatkan pengobatan yang rasional adalah melalui pengkajian resep, yang meliputi ketepatan penilaian kondisi pasien, tepat diagnosis, tepat indikasi, tepat jenis obat, tepat dosis, tepat cara dan lama pemberian, tepat informasi, dengan memperhatikan keterjangkauan harga, kepatuhan pasien, dan waspada efek samping. Pengkajian resep dilakukan berdasarkan aspek administratif, farmasetik dan klinis. Pengkajian resep dilakukan pada pasien yang menebus obat di Apotek Kimia Farma Dramaga 348 yang memiliki diagnosis diabetes tipe 2, hipertensi, dan diabetes tipe 2 yang disertai dengan hipertensi. Resep-resep yang akan dikaji umumnya berisi obat-obatan rutin pasien yang diberikan selama 1 bulan. Dari hasil kaji resep, Aspek administratif belum memenuhi persyaratan karena kurangnya informasi mengenai berat badan dan alamat pasien. Namun, kekurangan tersebut dapat diatasi dengan mengkonfirmasi kembali ke pasien dan dokter yang bersangkutan. Aspek farmasetik sudah memenuhi persyaratan, sementara aspek klinis belum memenuhi persyaratan karena ada beberapa obat yang saling berinteraksi dan merupakan polifarmasi. Namun, hal tersebut dapat diintervensi dengan mengedukasi pasien untuk menjeda konsumsi pemberian obat dan memotivasi pasien pentingnya untuk meminum obat secara rutin untuk mendapatkan hasil terapi yang optimal.

As a pharmacist, one way to ensure that patients have received rational treatment is through reviewing prescriptions, which includes accurate assessment of the patient's condition, correct diagnosis, correct indication, correct type of drug, correct dosage, correct method and duration of administration, correct information, with pay attention to affordability, patient compliance, and side effects. Prescription reviews are carried out based on administrative, pharmaceutical and clinical aspects. Prescription reviews were carried out on patients who redeemed medicines at Kimia Farma Dramaga 348 Pharmacy who had a diagnosis of type 2 diabetes, hypertension, and type 2 diabetes along with hypertension. The prescriptions that will be studied generally contain the patient's routine medicines that are given for 1 month. From the results of the prescription review, the administrative aspects did not meet the requirements due to lack of information regarding the patient's weight and address. However, this deficiency can be overcome by confirming it again with the patient and the doctor concerned. The pharmaceutical aspect meets the requirements, while the clinical aspect does not meet the requirements because there are several drugs that interact with each other and polypharmacy. However, this can be intervened by educating patients to separate the consume between interacted medications and motivating patients to the importance of taking medication regularly to get optimal therapeutic results."

"Hipertensi adalah salah satu penyakit yang menjadi masalah kesehatan terbesar di dunia, termasuk di Indonesia dengan prevalensi sebesar 25,8% pada tahun 2013. Apotek Kimia Farma Raya Bogor bekerja sama dengan Rumah Sakit Tugu Ibu dalam pemberian obat pada pasien BPJS rawat jalan. Dalam kajian ini, lima resep antihipertensi dipilih untuk dievaluasi dari aspek administrasi, farmasetis, dan klinis. Hasil pengkajian menunjukkan bahwa resep-resep tersebut memenuhi semua aspek yang diharuskan kecuali informasi berat badan yang tidak ada. Telaah klinis menunjukkan bahwa semua resep tepat indikasi, dosis, dan pemberiannya.

Hypertension is one of the major global health problems, including in Indonesia with a prevalence of 25.8% in 2013. Apotek Kimia Farma Raya Bogor collaborates with Tugu Ibu Hospital in providing medication for BPJS outpatients. In this study, five antihypertensive prescriptions were selected for evaluation from administrative, pharmaceutical, and clinical aspects. The evaluation results indicate that the prescriptions meet all required aspects except for the missing weight information. Clinical review shows that all prescriptions are appropriate in indication, dosage, and administration."

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.

Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Penyakit Paru Obstruksi Kronik (PPOK) merupakan penyakit yang ditandai dengan kelainan pada alveolar yang disebabkan oleh paparan signifikan terhadap partikel atau gas berbahaya sehingga mengakibatkan penyumbatan pada paru-paru dan keterbatasan aliran udara. Dalam upaya pencegahan dan pengobatan PPOK, apoteker berperan peting dalam pelayanan farmasi klinik yang berkaitan dengan sediaan farmasi untuk meningkatkan kualitas hidup pasien. Pengkajian resep merupakan salah satu pelayanan farmasi klinik yang dapat dilakukan oleh apoteker untuk menganalisa masalah terkait obat serta mencegah terjadinya kesalahan pemberian obat. Penelitian ini bertujuan untuk mengkaji aspek administrasi, kesesuaian farmasetik, dan pertimbangan klinis, serta kerasionalan 5 resep pasien paru di Apotek Kimia Farma Raya Bogor periode April 2023. Penelitian ini dilakukan dengan pengumpulan dan pengkajian resep pasien paru pada bulan april 2023. Hasil penelitian menunjukkan kelima resep pasien paru di Apotek Kimia Farma Raya Bogor telah memenuhi aspek administratif, aspek kesesuaian farmasetik, aspek pertimbangan klinis, dan resep rasional.

Chronic Obstructive Pulmonary Disease (COPD) is a disease characterized by alveolar abnormalities caused by significant exposure to harmful particles or gases, resulting in blockage of the lungs and limited air flow. In efforts to prevent and treat COPD, pharmacists play an important role in clinical pharmacy services related to pharmaceutical preparations to improve the quality of life of patients. Prescription review is one of the clinical pharmacy services that can be carried out by pharmacists to analyze drug-related problems and prevent medication administration errors. This research aims to examine administrative aspects, pharmaceutical suitability and clinical considerations, as well as the rationale for 5 prescriptions for pulmonary patients at Kimia Farma Raya Bogor for the period April 2023. This research was conducted by collecting and reviewing pulmonary patient prescriptions in April 2023. The research results showed that the five prescriptions for pulmonary patients at Kimia Farma Raya Bogor Pharmacy had fulfilled administrative aspects, pharmaceutical suitability aspects, clinical consideration aspects, and rational prescriptions."

"Pelayanan kefarsmasian adalah suatu pelayanan langsung dan bertanggung jawab seorang apoteker kepada pasien yang berkaitan dengan sediaan farmasi dengan maksud mencapai hasil yang pasti untuk meningkatkan mutu kehidupan pasien. Pengkajian resep merupakan suatu proses pemeriksaan resep. Skrining resep dilakukan bertujuan untuk menganalisis adanya masalah terkait obat dan apabila terdapat kesalahan atau kejanggalan dapat dikonsultasikan dengan dokter penulis resep sehingga pasien dapat terhindar dari risiko medication error. Hipertensi adalah suatu keadaan dimana tekanan darah meningkat melebihi batas normal. Faktor resiko yang menyebabkan hipertensi yaitu umur, jenis kelamin, keturunan, obesitas, merokok, kurangi aktivitas fisik, konsumsi garam berlebihan, dyslipidemia. Terapi pada pasien hipertensi bertujuan untuk mencapai dan mempertahankan target tekanan darah. Adanya peningkatan tekanan darah yang terjadi pada pasien hipertensi dapat menurunkan aliran darah ke ginjal. Aliran darah ginjal yang rendah akan menstimulasi reabsorpsi asam urat. Tekanan darah yang semakin tinggi memiliki kemungkinan untuk memperbesar risiko penyakit mikrovaskuler yang dapat memicu iskemia jaringan. Penggunaan obat yang tepat untuk penderita hipertensi diperlukan agar pengobatan pasien menjadi efektif. Penggunaan obat yang tidak efektif dapat mengakibatkan kegagalan terapi.

Prescription screening is carried out with the aim of analyzing drug-related problems and if there are errors or irregularities, it can be consulted with the prescribing doctor so that patients can avoid the risk of medication errors. Hypertension is a condition where blood pressure increases beyond normal limits. Risk factors that cause hypertension are age, gender, heredity, obesity, smoking, reduced physical activity, excessive salt consumption, dyslipidemia. Therapy for hypertensive patients aims to achieve and maintain target blood pressure. Increased blood pressure that occurs in hypertensive patients can reduce blood flow to the kidneys. Low renal blood flow will stimulate uric acid reabsorption. Increasing blood pressure has the potential to increase the risk of microvascular disease that can trigger tissue ischemia. The use of appropriate drugs for hypertensive patients is needed so that patient treatment is effective. Ineffective use of drugs can result in therapy failure."

"Pengkajian resep obat merupakan salah satu kegiatan pelayanan kefarmasian di apotek. Laporan praktik kerja profesi apoteker ini bertujuan untuk melakukan proses pengkajian resep dari salah satu resep pasien penyakit kronis hipertensi di Apotek. Aspek yang dikaji meliputi kelengkapan aspek administratif, farmasetik, dan klinik dari resep. Pengkajian dan analisis resep pasien penyakit kronis hipertensi dilakukan dengan pengambilan data dari salah satu resep penyakit kronis periode Mei 2023 di Apotek Kimia Farma 202 Kejayaan Depok. Resep ini ditujukkan untuk pasien atas nama Ny. Z yang menerima resep enam item obat dari dr. X, Sp.PD, yaitu Amlodipin 10 mg, Atorvastatin 20 mg, Bisoprolol 5 mg, Candesartan 16 mg, Hidroklorotiazid, dan Spironolakton 25 mg. Hasil pengkajian menunjukkan bahwa aspek administratif dan farmasetik resep tidak lengkap karena ada beberapa aspek yang tidak diisi dan tercantum pada resep. Hasil pengkajian resep berdasarkan aspek klinis menunjukkan telah memenuhi ketepatan indikasi, dosis, aturan, cara dan lama penggunaan obat, namun terdapat potensi interaksi antar obat. Resep tidak memenuhi seluruh aspek administratif, farmasetik, dan klinis secara lengkap. Masih terdapat beberapa aspek yang belum lengkap diantaranya informasi administratif kekuatan sediaan pada aspek farmasetik.

Prescription assessment is a crucial component of pharmaceutical services in pharmacies. This professional pharmacist internship report aims to evaluate the prescription review process for a patient with chronic hypertension at a pharmacy. The assessment covers the completeness of administrative, pharmaceutical, and clinical aspects of the prescription. The review and analysis of the chronic hypertension patient's prescription were conducted using data from a chronic disease prescription in May 2023 at Kimia Farma 202 Pharmacy in Kejayaan, Depok. The prescription was issued to a patient named Mrs. Z, who received a six-item prescription from dr. X, Sp.PD, including Amlodipine 10 mg, Atorvastatin 20 mg, Bisoprolol 5 mg, Candesartan 16 mg, Hydrochlorothiazide, and Spironolactone 25 mg. The assessment results revealed that the administrative and pharmaceutical aspects of the prescription were incomplete due to several missing or unspecified elements. The clinical assessment indicated that the prescription met the criteria for appropriate indications, dosage, administration method, and duration of use, but potential drug interactions were identified. Overall, the prescription did not fully meet all administrative, pharmaceutical, and clinical aspects. Some aspects, such as the strength of the preparation in the pharmaceutical section, were incomplete."

"Pengkajian dan pelayanan resep penting dilakukan untuk menganalisis adanya masalah terkait obat, sebagai upaya pencegahan kesalahan pemberian obat (medication error). Pengkajian resep dilakukan pada beberapa aspek yaitu aspek administratif, farmasetik dan klinis dengan beberapa komponen pada setiap aspeknya. Masalah terkait obat salah satunya dikarenakan adanya polifarmasi dalam peresepan obat. Beberapa jenis penyakit kronis dapat menyebabkan terjadinya komplikasi penyakit, sehingga memungkinkan terjadinya polifarmasi. Tujuan dilakukan penelitian untuk mengetahui kelengkapan dan kesesuaian aspek administratif, farmasetik dan klinis dari resep pasien BPJS dengan penyakit kronis di Apotek Kimia Farma 389 Depok periode resep bulan Juli 2023. Resep yang dikaji sebanyak 5 resep dengan kriteria berupa resep pasien BPJS dengan penyakit kronis, terdiri dari minimal 5 obat, dan resep periode Juli 2023. Resep yang memenuhi kriteria kemudian dilakukan dikaji pada aspek administratif, farmasetik dan klinis. Hasil yang diperoleh yaitu kelima resep yang dikaji belum lengkap secara administrasi dan diperlukan konfirmasi lebih lanjut kepada dokter penulis resep. Aspek farmasetik resep 1, 4 dan 5 telah lengkap memuat keterangan nama, bentuk dan kekuatan sediaan. Sedangkan aspek farmasetik resep 2 dan 3 belum lengkap karena tidak terdapat keterangan bentuk dan kekuatan sediaan. Aspek klinis dari kelima resep menunjukan adanya polifarmasi dan interaksi obat yang memerlukan pemantauan kondisi pasien lebih lanjut.

Assessment and prescription services are crucial for analyzing medication-related issues, as part of efforts to prevent medication errors. Prescription assessment covers several aspects: administrative, pharmaceutical, and clinical, each with specific components. One of the medication-related issues is due to polypharmacy in prescribing. Various chronic diseases can lead to disease complications, thereby potentially causing polypharmacy. The research aims to determine the completeness and appropriateness of the administrative, pharmaceutical, and clinical aspects of prescriptions for BPJS patients with chronic diseases at Apotek Kimia Farma 389 Depok, during the prescription period of July 2023. Five prescriptions were reviewed, each meeting the criteria of being for BPJS patients with chronic diseases, consisting of at least 5 medications, and being from the July 2023 period. These prescriptions were assessed for administrative, pharmaceutical, and clinical aspects. The results revealed that all five prescriptions reviewed were incomplete administratively, necessitating further confirmation from the prescribing doctor. Pharmaceutically, prescriptions 1, 4, and 5 were complete with information regarding drug name, form, and strength, while prescriptions 2 and 3 lacked information on form and strength of the drugs. Clinically, all five prescriptions showed polypharmacy and drug interactions requiring further patient condition monitoring."
