Penyakit jantung koroner merupakan penyakit yang disebabkan oleh adanya penumpukan atau pembentukan plak pada dinding arteri yang berfungsi untuk mensuplai darah ke jantung sehingga dapat muncul nyeri hingga serangan jantung. Terapi yang digunakan untuk jantung koroner bertujuan untuk mengurangi atau mencegah nyeri dada, mengurangi tekanan darah, memperlebar arteri, hingga mengelola kondisi medis yang berkontribusi pada penyakit jantung koroner. Apoteker memiliki kewajiban untuk memastikan terapi yang digunakan khususnya untuk penyakit jantung koroner dan tidak terbatas pada penyakit lainnya sesuai dan tidak terjadi medication error. Berdasarkan hal tersebutlah pada penelitian ini dilakukan salah satu cara mencegah medication error, yaitu dengan mengkaji resep pasien dengan penyakit jantung koroner. Penelitian ini dilakukan dengan memilih resep pasien jantung koroner kemudian dilakukan pengkajian resep meliputi administratif, farmasetika, hingga aspek klinis. Berdasarkan pengkajian resep yang telah dilakukan, terdapat beberapa aspek yang belum sesuai yaitu terkait dengan alamat pasien, berat badan pasien, polifarmasi, dan interaksi obat. Namun, ketidaksesuaian dari aspek-aspek tersebut masih dapat diterima. Informasi pasien terkait dengan berat badan dapat dikonfirmasi kepada pihak rumah sakit jika dibutuhkan. Aspek terkait dengan polifarmasi dapat diterima karena pengobatan yang digunakan bertujuan untuk mengatasi penyakit penyerta dan atau untuk meningkatkan efek terapi. Sedangkan untuk interaksi obat yang terjadi dapat dihindari dengan penggunaan obat yang dijeda.

Coronary heart disease is a disease caused by the buildup or formation of plaque on the walls of the arteries which function is to supply blood to the heart. The plaque can cause chest pain up to a heart attack if left untreated. Therapy used for coronary heart disease aims to reduce or prevent chest pain, reduce blood pressure, widen the arteries, and manage medical conditions that contribute to coronary heart disease. Pharmacists have an obligation to ensure that the therapy used, specifically for coronary heart disease and not limited to other diseases, is appropriate and medication errors do not occur. Based on this, this research conducted one way to prevent medication errors, namely by reviewing the prescription of patients with coronary heart disease. This research was conducted by selecting prescriptions for coronary heart patients and then reviewing prescriptions covering administrative, pharmaceutical, and clinical aspects. Based on the prescription review that has been done, there are several aspects that are not suitable, namely related to the patient's address, patient's weight, polypharmacy, and drug interactions. However, the discrepancy of these aspects is still acceptable. Patient information related to body weight can be confirmed to the hospital if needed. Aspects related to polypharmacy are acceptable because the treatment used aims to overcome comorbidities and or to increase the effect of therapy. Meanwhile, drug interactions that occur can be avoided by separating the consuming time of each drug
