"Kasus gangguan jiwa berat di dunia terutama pasien skizofrenia terjadi peningkatan khususnya di negaranegara berkembang. Sebagian besar klien yang dirawat di Ruang Antareja Rumah Sakit Marzoeki adalah Klien harga diri rendah . Tujuan Karya Ilmiah Akhir ini adalah menggambarkan hasil manajemen kasus spesialis terhadap klien harga diri rendah. Tindakan keperawatan dilakukan pada 20 orang pasien meliputi terapi individu harga diri rendah, terapi kognitif, terapi perilaku kognitif, terapi psikoedukasi keluarga pada pasien dengan harga diri rendah. Hasil menunjukkan bahwa kombinasi terapi individu, terapi kognitif, terapi perilaku kognitif, dan terapi psikoedukasi keluarga efektif dalam menurunkan tanda dan gejala pasien harga diri rendah dan peningkatan kemampuan pasien dan keluarga dengan pendekatan teori stress adaptation model oleh Stuart dan teori of Human Caring oleh Watson. Rekomendasi hasil karya ilmiah ini adalah terapi kognitif, terapi perilaku kognitif dan terapi psikoedukasi keluarga dapat dilakukan pada pasien harga diri rendah di seluruh tatanan pelayanan kesehatan jiwa.

Cases of severe mental disorder in the world, especially patients with skizofrenia increased, especially in developing countries. Most of the clients were treated in Antareja Ward Dr. H. Marzoeki Mahdi Bogor is low self esteem. The purpose of this work is to describe final scientific result of the patients case management specialist of low self esteem. Nursing Intervention performed on 20 patients include low self esteem individual therapy, cognitive therapy, cognitive behavioral therapy, family psychoeducation therapy in patienst with low self esteem.

The result showed that the combination of individual therapy, cognitive therapy, cognitive behavioral therapy, family psychoeducation therapy effective in reducing the signs and symptoms of low self esteem patients and increased patient and family’s ability to approach theorical models of stress adaptation by Stuart and theory of Human Caring by Watson. Recommendations of this scientific work is cognitive therapy, cognitive behavioral therapy and family psychoeducation therapy can be performed in patients with low self esteem throughout the structure of mental health services."

"Gejala negatif yang ditemukan pada pasien skizofrenia umumnya mengarah kepada respons emosional yang ganjil, seperti ekspresi wajah dan nada bicara monoton, sulit merasa senang, enggan bersosialisasi, kehilangan minat pada berbagai aktivitas. Perubahan tersebut mengarah kepada harga diri rendah kronik. Pada pasien dengan harga diri rendah kronik, seringkali ditemukan pikiran otomatis yaitu penilaian negatif kepada diri sendiri atau orang lain. Jika pikiran tersebut tidak segera diatasi, maka akan sangat mempengaruhi kemampuan pasien dalam mengendalikan pikiran yang berdampak pada perasaan dan perilaku pasien dalam aktivitas sehari-hari. Harga diri rendah kronik merupakan 1 dari 5 diagnosa keperawatan yang paling sering ditemukan di RSJ. Ilustrasi kasus. Studi kasus ini dilakukan kepada 4 orang pasien perempuan yang dirawat di wisma Setyowati RSJ Prof. Dr. Soerojo Magelang dengan diagnosa keperawatan harga diri rendah kronik. Semua pasien diberikan terapi keperawatan generalis dan spesialis. 2 orang pasien mendapatkan terapi perilaku kognitif dan psikoedukasi keluarga, sedangkan 2 orang lainnya mendapatkan terapi kognitif dan psikoedukasi keluarga. Terapi spesialis diberikan dalam 6-9 kali pertemuan baik secara langsung dengan pasien dan keluarga maupun secara daring dengan keluarga. Tanda dan gejala harga diri rendah kronik diukur di setiap awal pertemuan dan di tiap akhir pertemuan dilakukan evaluasi harga diri menggunakan Rosenberg self-esteem scale (RSES). Kesimpulan. Hasil yang diperoleh dari asuhan keperawatan spesialis pada keempat pasien ditemukan penurunan tanda dan gejala harga diri rendah kronik dan peningkatan tingkat harga diri dari kategori rendah menjadi tinggi menggunakan RSES.

Negative symptoms found in schizophrenic patients generally lead to odd emotional responses, such as monotonous facial expressions and tone of voice, difficulty feeling of happiness, reluctant to socialize, loss of interest in various activities. These changes lead to chronic low self-esteem. In patients with chronic low self-esteem, automatic thoughts are often found, namely negative judgments about themselves or others. If these thoughts are not immediately addressed, it will greatly affect the patient's ability to control thoughts which have an impact on the patient's feelings and behavior in daily activities. Chronic low self-esteem is 1 of 5 nursing diagnoses that are most often found in the mental hospitals.

Case illustration. This case study was conducted on 4 female patients who were treated at Wisma Setyowati RSJ Prof. Dr. Soerojo Magelang with chronic low self-esteem nursing diagnosis. All patients were given generalist and specialist nursing therapies. 2 patients received cognitive behavioral therapy and family psychoeducation, while 2 others received cognitive therapy and family psychoeducation. Specialist therapy is given in 6-9 meetings, both in person with patients and families and online with families. Signs and symptoms of chronic low self-esteem were measured at the beginning of each meeting and at the end of each meeting, self-esteem was evaluated using the Rosenberg self-esteem scale (RSES).

Conclusion. The results obtained from specialist nursing care for the four patients found a decrease in signs and symptoms of chronic low self-esteem and an increase in the level of self-esteem from low to high category using RSES."
