"Residensi Keperawatan Medikal Bedah merupakan aplikasi praktek pada program spesialis yang menggali ilmu sesuai dengan peminatan, dalam hal ini adalah sistem pernapasan. Kegiatan yang dilakukan selama residensi adalah melaksanakan asuhan keperawatan dengan pendekatan model teori self care deficit Dorothea Orem. Masalah keperawatan yang sering muncul pada pasien dengan gangguan sistem respirasi adalah batuk, sesak, nyeri, demam, gangguan nutrisi dan kelelahan. Kegiatan berikutnya adalah pemberian intervensi berbasis bukti (evidence based nursing) dengan memberikan latihan napas pursed lip breathing pada pasien Penyakit Paru Obstruksi Kronik (PPOK).
Hasilnya adalah pursed lip breathing dapat menurunkan frekwensi pernapasan, meningkatkan arus puncak respirasi dan saturasi oksigen. Kegiatan berikutnya adalah dan melaksanakan program inovasi tentang water seal drainage 1 botol. Hasilnya adalah sangat direkomendasikan untuk digunakan pada pasien yang memerlukan pemasangan water seal drainage dilihat dari keamanan dan estetika penglihatan.
Disimpulkan bahwa model Teori self care deficit Dorothea Orem dapat diterapkan dalam pelaksanaan asuhan keperawatan pada pasien gangguan sistem respirasi, pursed lip breathing pada pasien penyakit paru obstruksi kronik, dan penggunaan water seal drainage 1 botol inovasi sangat direkomendasikan dapat memberi keamanan bagi pasien dan baik secara estetika penglihatan.

Medical surgical nursing residency is the practice in the application of specialist practice where student explore a new science a technologi related to, in the respiratory system. The activities that always do during the residency is carring out nursing care with theoretical approach model self care deficit Dorothea Orem.
The nursing problem that always appears to patient with disorder of respiratory system are coughing, tightness, pain, fever, nutrisional disorders and fatique. The next step is giving evidence based nursing by giving breathing excercise is pursed lip breathing to Chronic Obstruction Pulmonary Disease (COPD).
The result is pursed lip breathing that make be able to lower frequency breathing , increasing the current peak of respiration and oxygen saturation. The next activity is creating a new innovation program about water seal drainage one bottle. The result is highly recommended for use by patients who need the installation of a water seal drainage in the context of safety and visual aesthetics.
Concluded that the theory of model self care deficit dorothea orem can be applied in the implementation of nursing care for with disorders of respiration system, pursed lip breathing for patients chronic obstruction pulmonary disease and the user of water seal drainage one bottle of innovation is very recommended and it can give for patients and it?s better also from visual aesthetics.

"Chronic Obstructive Pulmonary Disease (COPD) is a major cause of morbidity and mortality worldwide. By 2020, COPD is expected to be the fifth leading cause of disability and the third leading cause of death. COPD is associated with significant co-morbid illnesses including cardiovascular disease, pulmonary vascular disease, osteoporosis, depression and thoracic malignancies. In addition, COPD patients often have systemic manifestations of their disease, including peripheral muscle dysfunction, decreased activity levels and nutritional abnormalities, all of which contribute to poor functional status, decreased quality of life and increased health care utilization. Lack of recognition and understanding of the complex interactions of these common co-morbid conditions and systemic effects often contributes to sub-optimal care for patients with COPD."

"ABSTRAK
Keperawatan respirasi merupakan salah satu area khusus dimana perawat memiliki peran dalam memberikan asuhan keperawatan berdasarkan kebutuhan pasien. Perawat spesialis respirasi memiliki peran lebih yang berbeda, dengan menerapkan konsep dan teori keperawatan dalam penanganan kasus pada pasien gangguan pernapasan. Peran ners spesialis sebagai klinisi ahli, pendidik, manajer kasus, konsultan, dan peneliti untuk merencanakan atau memperbaiki kualitas keperawatan bagi klien dan keluarganya. Teori keperawatan yang digunakan dalam merawat pasien (satu kasus kelolaan dan 30 kasus resume) dengan menggunakan model adaptasi Roy. Peran sebagai peneliti diterapkan dengan melakukan latihan breathing retraining pada pasien asma. Sementara peran untuk memperbaiki kualitas pelayanan salah satunya adalah menerapkan inovasi bagi pasien tuberkulosis dalam mencegah terjadinya putus obat.

"Sleep loss and obesity : intersecting epidemics represents a major contribution to the field of sleep medicine. It is a comprehensive review of the neurobiology of sleep, circadian timing and obesity, the deleterious effects of sleep loss and obesity on health, and the worrisome associated social and medical costs in a range of patient populations and overall to society.
The number of individuals who are obese has reached alarming levels. As a result, the incidence of Type 2 diabetes, cardiovascular disorders, heart disease, and kidney failure have also increased. The surgeon general estimates that the total annual cost of obesity in the US is about $117 billion. This cost is expected to escalate significantly because the number of overweight and obese children is increasing rapidly. Indeed, the new generation is expected to have a shorter life-span then their parents. In addition, sleep loss is emerging as an important contributing factor to obesity. People who sleep less or are sleep deprived tend to eat more, especially carbohydrates, and have a higher body mass index. Increased weight restricts the upper airway, causing obstructive sleep apnea and further sleep loss. In the end there is a vicious cycle of weight gain and sleep loss.
In the past few years there has been a tremendous growth in our understanding of brain mechanisms controlling energy metabolism. Interestingly the neurons regulating waking also regulate feeding. There is also a mechanism that regulates the timing of feeding and sleep. In shift-workers this system is likely to be disturbed, and this has an adverse impact on both feeding and sleep.
Sleep loss and obesity : intersecting epidemics is the first title to clearly examine how obesity and sleep loss are interacting epidemics.
