"Asuhan keperawatan pada pasien kanker yang dilakukan pada praktik klinik lanjut keperawatan medikal bedah adalah pada pasien kanker payudara yang mendapatkan kemoterapi dengan penerapan peaceful end of life theory di Rumah Sakit Kanker Dharmais Jakarta Selain itu asuhan keperawatan juga dilakukan dalam bentuk resume keperawatan Dari pengelolaan asuhan tersebut ditemukan bahwa peaceful end of life teori dapat di terapkan pada pasien kanker yang memasuki tahap paliatif Sedangkan diagnosa keperawatan yang paling sering muncul adalah nyeri kronis dan kecemasan Pada praktik klinik ini terdapat juga penerapan evidence based practice EBN tentang penggunaan oral cryotherapy untuk mengurangi oral mukositis pada pasien yang mendapatkan kemoterapi 5 flourourasil 5 fu menemukan hasil bahwa oral cryotherapy mampu mengurangi kejadian oral mukositis Selanjutnya untuk memenuhi peran perawat sebagai innovator maka dilakukan evaluasi dan ujicoba pengembangan format pengkajian keperawatan Kata kunci kanker kemoterapi oral mukositis.

"Cryotherapy Oral telah terbukti secara ilmiah sebagai terapi non farmakologi untuk menurunkan angka kejadian mukositis oral. Dengan menurunnya angka kejadian mukositis oral, maka dapat dipastikan kenyamanan dan kualitas hidup pasien bertambah, lama dan biaya rawat menurun, sehingga kualitas hidup maksimal. Ketika pasien merasa nyaman, maka pasien akan kooperatif dalam menjalani tindakan kemoterapi. Tujuan penelitian ini adalah untuk melihat apakah dengan pemberian ice cubes Normal Saline 0,9 % berpengaruh terhadap penurunan mukositis oral pada pasien KNF yang menjalani kemoterapi Fluorouracil (5FU). Penelitian ini menggunakan metode quasi eksperimen. Sebanyak 94 pasien KNF yang dipilih menggunakan teknik consecutive sampling. Hasil penelitian menunjukkan kelompok intervensi yang diberikan cryotherapy oral Normal Saline 0,9% mengalami grade mukositis lebih rendah (p=0,001). Faktor confounding yang meliputi usia, jenis kelamin, oral hygiene dan riwayat merokok tidak berhubungan dengan kejadian mukositis oral dengan nilai (p >0,05). Penelitian ini diharapkan dapat acuan dan meningkatkan pengetahuan perawat serta mendorong untuk meningkatkan tindakan keperawatan mandiri yang profesional.

Oral cryotherapy has been scientifically proven as a non-pharmacological therapy to reduce the incidence of oral mucositis. With the decline in the incidence of oral mucositis, patient’s comfort improved, the length of stay and cost of care decreases, and maximum quality of life is achieved. The aim of this study is to see whether the administration of ice cubes Normal Saline 0,9% affects the decrease in oral mucositis in Nasopharingeal Cancer (NPC) patients undergoing Fluorouracil (5FU) chemotherapy. This study used quasi experiment method. A total of 94 NPC patients were selected using a consecutive sampling technique. The result showed that the intervention group given oral cryotherapy Normal Saline 0,9% had a  lower mucositis grade (p=0,001). Confounding factors which included age, sex, oral hygiene and smoking history were not associated with the incidence of oral mucositis with a value (p>0,05). This research is expected to increase nurses’ knowledge and also encourage nurse to improve independent nursing professional care plan."

"Salah satu program spesialistik keperawatan medikal bedah adalah keperawatan onkologi. Keperawatan onkologi merupakan suatu area spesialisasi keperawatan yang mempelajari penyakit akibat tumor atau keganasan. Program spesialis keperawatan onkologi bertujuan untuk meningkatkan pemberian asuhan keperawatan pada kasus paliatif dan kasus kanker yang bersifat komplek dan komprehensif yang hanya dapat diberikan oleh perawat spesialis onkologi. Tujuan Karya Ilmiah Akhir ini melaporkan pencapaian target kompetensi spesialis keperawatan onkologi yang telah dilakukan. Target kompetensi pertama yaitu analisis pendekatan Teori Keperawatan Virginia Henderson pada pasien kanker penis. Selanjutnya target kompetensi kedua yaitu penerapan instrumen Oral Assessment Guide OAG sebagai format pengkajian mukositis akibat kemoterapi, dan target kompetensi ketiga yaitu program inovasi berupa penerapan intervensi oral cryoterapi sebagai tindakan mandiri perawat dalam mencegah dan mengatasi mukositis. Kesimpulan: Model teori keperawatan Virginia Henderson efektif di terapkan sebagai acuan dalam memberikan asuhan keperawatan pada pasien dengan masalah onkologi atau paliatif. Selain itu, instrumen Oral Assessment Guide OAG terbukti efektif dan efisien sebagai deteksi awal mukositis dan untuk intervensi mandiri keperawatan, intervensi oral cryoterapi juga terbukti efektif dalam mencegah dan mengurangi mukositis pada pasien kanker yang menjalani kemoterapi sehingga mudah untuk diaplikasikan oleh perawat ruangan.

The aim of this Final Scientific Works reports on achieving the target of specialist competence of oncology nursing that has been done. The first target of competence is the analysis of the Virginia Henderson Nursing Theory approach in patients with penile cancer. Furthermore, the second target of competence is the application of Oral Assessment Guide OAG instrument as a mukositis assessment format due to chemotherapy, and the third competency target is innovation program in the form of oral cryotherapy intervention as the independent action of nurse in preventing and overcoming mukositis.

Conclusion: Virginia Henderson 39;s model of nursing theory is effectively applied as a reference in providing nursing care in patients with oncology or palliative problems. In addition, the Oral Assessment Guide OAG instrument proved to be effective and efficient as early detection of mucositis and for self-care nursing intervention, oral intervention cryotherapy has also proven effective in preventing and reducing mucositis in cancer patients undergoing chemotherapy making it easy to apply by nurses."
