"ABSTRAKBanyaknya permasalahan yang ditimbulkan akibat gangguan muskuloskeletal maka diperlukan peran Ners spesialis keperawatan muskuloskeletal untuk memberikan asuhankeperawatan profesional dan advance berdasarkan kebutuhan pasien. Tujuan penulisanKarya Ilmiah Akhir ini adalah menganalisis kegiatan praktik Ners Spesialis Keperawatan Medikal Bedah berdasarkan standar kompetensi spesialis keperawatan medikal bedah kekhususan sistem muskuloskeletal. Ners Spesialis dalam melakukan asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy baik pada kasus kelolaan utama maupun resume. Penerapan Evidence Based Nursing (EBN) melalui intervensi latihan isometrik pada pasien pasca operasi fraktur ektremitas bawah, efektif menurunkan nyeri dan meningkatkan kekuatan otot. Proyek inovasi berupa pengembangan Enhanced Recovery after Surgery (ERAS) for orthopaedic untuk mempercepat proses pemulihan pasien operasi hip. Pemberian asuhan keperawatan pada pasien dengan gangguan muskuloskeletal menggunakan pendekatan teori Model Adaptasi Roy dapat membantu pasien untuk beradaptasi terhadap penyakitnya. Intervensi latihan isometrik jika dikombinasikan dengan terapi non farmakologis yang lain seperti nafas dalam berpotensi bisa memberikan efek yang lebih baik terhadap penurunan nyeri.

ABSTRACTThe number of problems caused by musculoskeletal disorders requires the role of nursespecialist musculoskeletal nursing to provide professional and advanced nursing care based on patient needs. The purpose of writing this final scientific paper is to analyzethe practical activities of nurse specialist medical surgical nursing based on the competency standards of medical surgical nursing specialists, specifically the musculoskeletal system. Nurse specialists in conducting nursing care in patients withmusculoskeletal disorders use the Roy Adaptation Model theory approach both in thecase of main and resume managed. The application of Evidence Based Nursing (EBN)through isometric exercise intervention in patients with lower extremity fracture surgery, effectively reduces pain and increases muscle strength. The innovation project was in the form of developing Enhanced Recovery after Surgery (ERAS) for orthopedics to accelerate the recovery process for hip surgery patients. Giving nursing care to patients with musculoskeletal disorders using the Roy Adaptation Model theory approach can help patients adapt to the disease. An isometric exercise intervention if combined with other non-pharmacological therapies such as deep breathing has the potential to have a better effect on reducing pain."

"Praktek Spesialis Keperawatan merupakan sebuah proses penting dari pendidikan profesi dalam rangka mengaplikasikan peran perawat spesialis yang berdampak pada upaya peningkatan kualitas layanan keperawatan. Praktek ini dilakukan dan dikembangkan berdasarkan ilmu pengetahuan dan teknologi melalui berbagai penelitian menggunakan model Pendekatan Kebutuhan Dasar Virginia Henderson dan Teori Sosial Kognitif Bandura di RS Fatmawati dan RS Persahabatan Jakarta. Tiga kompetensi yang harus dicapai dalam praktek residensi ini adalah memberikan asuhan keperawatan menggunakan pendekatan teori keperawatan, menerapkan implementasi keperawatan yang berbasis bukti ilmiah (evidencebased nursing practice), dan melakukan proyek inovasi keperawatan yang berguna bagi lahan yang menjadi tempat praktek residensi. Sebagai pemberi asuhan keperawatan, mahasiswa residensi telah memberikan asuhan keperawatan menggunakan pendekatan teori Virginia Henderson pada satu kasus kelolaan utama dan 30 kasus resume pasien HIV/AIDS dengan berbagai infeksi oportunistik. Sebagai peneliti, mahasiswa telah mengaplikasi penerapan terapi spiritual terhadap penurunan tingkat stres pada orang dengan HIV/AIDS. Pengaruh terapi tersebut selaras dengan evidencebase nursing terkait menunjukkan hasil bahwa terdapat pengaruh yang signifikan antara pemberian terapi spiritual terhadap penurunan Tingkat stress pada orang dengan HIV/AIDS ODHA). Sebagai inovator, mahasiswa telah melakukan inovasi edukasi berbasis web untuk meningkatkan pengetahuan pasien Sistemic Lupus Eritematosus (SLE) di RS Persahabatan Jakarta. Hasil inovasi menunjukkan bahwa penggunaan media edukasi melalui webApp yang diakses menggunakan barcode menunjukkan peningkatan pengetahuan yang cukup signifikan.

Nursing Specialist Practice is an important process of nurse’s professional education to apply the role of specialist nurses which has an impact on efforts to improve the quality of nursing services. This practice is carried out and developed based on irginia Henderson's Model of Basic Needs Approach and Bandura's Cognitive Social Theory science and technology through various research at Fatmawati Hospital and Persahabatan Hospital. The three competencies that must be achieved in this residency practice are providing nursing care using a theoretical nursing approach, implementing evidencebased nursing practice, and conducting nursing innovation projects that are useful for the land where the residency practice is located. As a nursing care provider, the resident students have provided nursing care using the Virginia Henderson theory approach on a primary management case and 30 resume cases of HIV/AIDS patients with various opportunistic infections. As researchers, students have applied the application of Spiritual Therapy to Reduce Stress Levels in People with HIV/AIDS. The effect of the therapy is in line with related evidence-based nursing, showing that there was a signifact effect on decreasing level of stress in PLWH. Meanwhile, as an innovator, student has carried out web-based educational innovations to increase knowledge of Systemic Lupus Erythematosus (SLE) patients at Persahabatan Hospital Jakarta. The results of the innovation show that the use of educational media neglected webApp accessed using barcodes showed significant results in increasing knowledge of SLE patients."

"ABSTRAKPrevalensi hospitalisasi pasien diabetes cukup tinggi dengan berbagai macam alasan untuk dirawat inap, namun upayadalam pengontrolan glukosa darah sering mengalami kegagalan walaupun sudah dilakukan dengan pendekatan medis.Kegagalan dalam pengontrolan glukosa darah dapat mengakibatkan komplikasi baik akut maupun kronis. Pencegahankomplikasi memerlukan peran dari berbagai multidisiplin ilmu salah satunya adalah perawat spesialis medikal bedahdalam memberikan asuhan keperawatan dengan menggunakan pendekatan berbagai teori keperawatan. Salah satunyaadalah Model adaptasi Roy yang dapat dijadikan acuan dalam memberikan asuhan keperawatan yang holistik dankomprehensif dengan meminimalkan stimulus yang mempengaruhi adaptasi agar tercapai perilaku yang adaptif. "Kondisi menderita penyakit fisik menimbulkan permasalahan psikososial ansietas dan gangguan citra tubuh. Permasalahan psikososial ini menjadi faktor comorbid yang menambah berat kondisi fisik klien. Pelaksanaan tindakan ners spesialis didasarkan pada perilaku dan adaptasi sebagai mahluk yang holistik yang meliputi: Cognitive Behavioral Therapy CBT, Thought Stopping TS, Progressive Muscle Relaxation PMR dengan mempertimbangkan toleransi fisik klien. Tindakan ners spesialis Family PsychoEducation FPE pada keluarga dan Supportive Theraphy ST pada kelompok pelaku rawat care giver diberikan untuk memperkuat dukungan dari luar klien demi mempertahankan kelangsungan proses perawatan klien. Hasil: Pelaksanaan tindakan ners spesialis menunjukkan penurunan tanda dan gejala pada klien di semua aspek respons terhadap stresor. Penurunan yang paling besar terjadi pada kombinasi terapi CBT PMR FPE yang dilakukan pada klien dengan diagnosis ansietas pada aspek afektif sebesar 79.2 dan klien dengan diagnosis gangguan citra tubuh pada aspek perilaku sebesar 86.3 . Kesimpulan: Tindakan ners spesialis Cognitive Behavioral Therapy CBT, Thought Stopping TS, Progressive Muscle Relaxation PMR dengan mempertimbangkan toleransi fisik klien dan Family PsychoEducation FPE pada keluarga serta Supportive Theraphy ST pada kelompok pelaku rawat care giver dapat membantu penurunan respons klien terhadap stressor.

Physical ilness could contribute to psychosocial problems such as anxiety and disturbed body image. This psychosocial problems became a comorbid factor which could make the clients health problems more serious. The implementation Cognitive Behavioral Therapy CBT, Thought Stopping TS, Progressive Muscle Relaxation PMR holistically applied by taking into consideration the physical tolerance of clients. The provision of Family PsychoEducation FPE for patient's family and Supportive Theraphy ST for patient's care giver by mental health psychiatric nurse specialist could empower client continuity of care sustainability. Result The combination CBT PMR FPE nurse specialist implementation have decreased the sign and symptoms up to 79.2 of efective aspect for clients with anxiety, and 86.3 of behavioral aspect for client who has disturbed body image. Conclusion It was conclude that the implementation of Cognitive Behavioral Therapy CBT, Thought Stopping TS, Progressive Muscle Relaxation PMR by taking into consideration the client's physical tolerance and Family PsychoEducation FPE for client's family and Supportive Theraphy ST for client's care giver can decrease patient's responses to stressors."

"Praktik residensi keperawatan medikal bedah adalah program pendidikan yang bertujuan untuk menghasilkan perawat ners spesialis dengan kekhususan tertentu. Kegiatan praktik residensi terdiri dari pemberian asuhan keperawatan pada pasien dengan gangguan sistem perkemihan menggunakan pendekatan model adaptasi Roy. Asuhan keperawatan dilakukan kepada pasien dengan kasus gagal ginjal terminal, keganasan dan obstruksi. Penggunaan pendekatan teori Roy bertujuan untuk membantu meningkatkan adaptasi individu terhadap lingkungannya. Penerapan evidence based nursing yang berupa ice therapy bertujuan untuk mengurangi nyeri penusukan AV fistula pada pasien yang menjalani hemodialisis. The residency practice of medical surgical nursing is an educational program that aims to produce nurse specialist with particular specialty. It consist of giving nursing care to patients with urinary system disorder using Roy's model adaptation. Nursing care during residency practice was given to patient with renal failure, malignancy and urinary tract obstruction. The using Roy adaptation model is aimed to improve patient adaptation with the environment. Implementation of evidence based nursing (EBN) using ice therapy is aimed to reduce pain cannulation AV fistula in patients undergoing hemodialysis. Ice therapy is simple, more effective, and without side effect so that it can be applied as non-pharmacological pain management intervention. The implementation of innovation project through early ambulation for kidney transplantation patient to increase knowledge and ability of patients to carry out early ambulation as to accelerate the length of stay hospital."

Perawat spesialis diharapkan dapat melaksanakan perannya dalam memberikan pelayanan keperawatan pada kasus dengan gangguan sistem pernapasan. Peran tersebut sejalan dengan kompetensi yang harus dicapai selama praktik residensi berupa pemberian asuhan keperawatan, penerapan praktik keperawatan berbasis bukti dan proyek inovasi. Pemberian asuhan keperawatan diterapkan kepada satu pasien kelolaan utama dengan kasus PPOK dan 30 kasus resume menggunakan pendekatan model adaptasi Roy. Penerapan tindakan keperawatan berbasis bukti ilmiah yang dilakukan yaitu pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing pada pasien dyspnea dengan penyakit paru. Proyek inovasi yang diterapkan yaitu Pengembangan Self Manajemen Dengan Video Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien Dengan Penyakit Paru Obstruksi Kronis (PPOK). Hasil analisis praktik menunjukkan bahwa model adaptasi Roy dapat digunakan sebagai pendekatan dalam asuhan keperawatan pasien dengan gangguan sistem pernapasan untuk mengembangkan mekanisme adaptif pasien. Pemberian posisi condong kedepan dan Latihan nafas pursed lips breathing dapat menurunkan sesak napas pada pasien dengan penyakit paru. Selain itu, Latihan Breathing Exercise : ACBT Dalam Meningkatkan Airway Clearance Pada Pasien mendapat evaluasi yang positif dan dapat diterapkan di tatanan klinis sebagai inovasi pengkajian dan manajemen keperawatan mandiri pada pasien PPOK selama di rumah.

Medical surgical nursing residency practice is part of professional education that aims to produce specialist nurses. Specialist nurses are expected to carry out their role in providing nursing services in cases with respiratory system disorders. The role is in line with the competencies to be achieved during residency practice in the form of providing nursing care, implementing evidence-based nursing practice and innovation projects. Nursing care was applied to one primarily managed patient with a case of COPD and 30 resume cases using Roy's adaptation model approach. The application of scientific evidence-based nursing actions carried out is the provision of a forward leaning position and pursed lips breathing exercises in dyspnea patients with lung disease. The innovation project applied is the Development of Self Management with Video Breathing Exercise: ACBT in Increasing Airway Clearance in Patients with Chronic Obstructive Pulmonary Disease (COPD). The results of the practice analysis show that Roy's adaptation model can be used as an approach in nursing care of patients with respiratory system disorders to develop patient adaptive mechanisms. Forward leaning and pursed lips breathing exercises can reduce shortness of breath in patients with lung disease. In addition, Breathing Exercise: ACBT in Increasing Airway Clearance in Patients received a positive evaluation and can be applied in clinical settings as an innovation in self-assessment and nursing management in COPD patients while at home."

"Latarbelakang: Praktik Spesialis Keperawatan Medikal Bedah Kekhususan Endokrin di RSUPN dr. Cipto Mangunkusumo Jakarta merupakan kegiatan pembelajaran dalam mengaplikasikan peran dan fungsi Ners Spesialis. Belum ada Karya Ilmiah Akhir Spesialis (KIAS) yang menelaah pasien diabetes melitus dengan komplikasi selulitis dan Chronic Kidney Disease (CKD) dengan penerapan teori adaptasi Roy. Tujuan: KIAS ini bertujuan untuk menganalisis asuhan keperawatan pada pasien diabetes melitus yang mengalami selulitis dan CKD maupun resume 30 kasus. Metode: KIAS ini terdiri dari analisis kasus utama, resume 30 kasus, penerapan evidence based nursing dan proyek inovasi. Teori model keperawatan yang dipakai adalah teori adaptasi Roy. Hasil: Ners spesialis berperan tidak hanya dalam memberikan asuhan keperawatan langsung tetapi menerapkan intervensi berbasis bukti ilmiah pada area endokrin yaitu memberikan aplikasi pelembab mengandung urea, gliserin dan petrolatum untuk mengatasi xerosis dan fisura tumit kaki pasien diabetes melitus tipe 2. Selain itu, Ners spesialis juga melakukan inovasi melalui analisis situasi dan kajian literatur untuk mencegah kejadian hipoglikemia berat pada pasien diabetes melitus tipe 2 melalui penerapan Hy-Newss bundle. Kesimpulan: Teori model adaptasi Roy sesuai untuk diaplikasikan pada pasien dengan gangguan endokrin. Pemberian pelembab mengandung urea, gliserin dan petrolatum efektif untuk menurunkan skor derajat xerosis pada pasien diabetes melitus tipe 2. Hy- Newss dapat diterapkan sebagai alat skrining risiko terjadi hipoglikemia berat pada pasien diabetes melitus tipe 2.

Background: Medical surgical nursing specialist practice in endocrine specific at RSUPN dr. Cipto Mangunkusumo Jakarta is a learning activity in applying the role and function of nurse specialist. There is no KIAS that examines diabetes mellitus patients with complications of cellulitis and Chronic Kidney Disease (CKD). Aim: This KIAS aims to analyze nursing care in patients with diabetes mellitus who have cellulitis and CKD or resume 30 cases. Method: This KIAS consists of main case analysis, 30 case resumes, application of evidence based nursing and innovation projects. The nursing model theory used is Roy’s adaptation theory. Results: Nurse specialist play a role not only in providing direct care but applying scientific evidence-based practice in the endocrine area, which is providing moisturizing applications containing urea, glycerin and petrolatum to treat xerosis and heel fissures in patient with type 2 diabetes mellitus. In addition, nurse specialist innovate through situation analysis and literature review to prevent the incidence of severe hypoglycemia in patients with type 2 diabetes mellitus through the application of Hy-Newss bundle. Conclusion: Roy’s adaptation model theory is suitable for application in patients with endocrine disorders. Moisturizers containing urea, glycerin and petrolatum are effective in reducing the degree of xerosis score in patient with type 2 diabetes mellitus. Hy-Newss bundle can be applied as a screening tool for the risk of severe hypoglycemia in patients with type 2 diabetes mellitus."

Model adaptasi Roy digunakan dalam asuhan keperawatan dengan pasien dengan gangguan system pernapasan. Pendekatan model adaptasi Roy bertujuan untuk mempertahankan integritas adaptasi manusia. Model adaptasi Roy berperan dalam membantu mengembangkan mekanisme koping yang adaptif terhadap stimulusdari lingkungan. Pada target kompetensi penerapan asuhan berbasis bukti dengan penerapan Aroma Hand Massage untuk mengurangi nyeri, tingkat kecemasan dan depresi pada pasien dengan kanker paru terminal.Hasil penerapan menunjukkan bahwa terapi aroma hand massage menurunkan skor nyeri dan depresi pada pasien dengan kanker paru terminal. Proyek inovasi adalah penerapan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien dengan kanker paru. Hasil penerapan aplikasi tersebut  menunjukkan bahwa kemanfaatan, mudah dalam penggunaan dan kepuasan responden dalam mengaplikasikannya.

Some of the competencies must be achieved in residency practice of medical surgical are case management using nursing model theory, the application of  evidence-based nursing practice, and innovation projects. The implementation of evidence based nursing used Aroma Hand Massage was effective and useful significantly to reduce score pain and depression on patient with terminal lung cancer. Innovation project was the use of mobile phone application for assessment and management nursing intervention  on patient with lung cancer it was  useful, easy and fun to be applied."

"Peran perawat spesialis diperlukan untuk peningkatan kualitas pelayanan asuhan keperawatan yang kompleks dan akurat sebagai pemberi asuhan keperawatan tingkat lanjut kasus neurosain, pendidik, advokat, serta agen pembaharu melalui penerapan evidence based nursing (EBN) dan proyek inovasi. Asuhan keperawatan tingkat lanjut menggunakan teori Adaptasi Roy yaitu pengelolaan pasien meningitis tuberkulosis sebagai kasus utama dan 30 resume gangguan sistem neurologi. Teori Roy banyak bertujuan meningkatkan perilaku adaptif dan mengubah perilaku inefektif. Diagnosis keperawatan yang paling banyak ditemukan pada pasien gangguan sistem neurologi adalah ketidakefektifan perfusi jaringan serebral dan hambatan mobilitas fisik. Penerapan EBN dilakukan pada pasien stroke yang mengalami disfagia. Pasien diberikan latihan menelan shaker exercise hasilnya menunjukkan peningkatan kemampuan menelan dan tidak terjadi aspirasi. Proyek inovasi kelompok menerapkan Pengembangan Media Edukasi Perawatan Pasien Brain Tumor Craniotomy. Penerapan proyek inovasi meningkatkan pengetahuan pasien, keterampilan pasien latihan napas dalam dan mobilisasi setelah operasi, dan menambah kepercayaan diri perawat saat memberikan edukasi. Pengalaman praktik residensi diharapkan menambah kompetensi dan peran perawat spesialis di lahan klinik.

The role of nurse specialists is needed to improve the quality of complex and accurate nursing care services as providers of advanced nursing care in cases of neuroscience, educators, advocates, and agents of reform through the application of evidence based nursing (EBN) and innovation projects. Advanced nursing care uses Roy's Adaptation theory, which is the management of meningitis tuberculosis patients as the main case and 30 resumes of neurological system disorders. Roys theory aims to improve adaptive behavior and change ineffective behavior. The most common nursing diagnoses found in patients with neurological system disorders are ineffective perfusion of cerebral tissue and barriers to physical mobility.

EBN application is performed on stroke patients who have dysphagia. The patient is given training to swallow the exercise shaker, which results in increased swallowing ability and no aspiration. The group innovation project applies Development of Educational Media for Nursing Brain Tumor Craniotomy Patients. The application of innovation projects increases patient knowledge, the skills of patients in deep breathing exercises and mobilization after surgery, and increases nurse confidence when providing education. The residency practice experience is expected to increase the competency and role of specialist nurses on the clinic grounds."

Peran selanjutnya sebagai innovator, proyek inovasi yang dilakukan berupa penggunaan mobile phone application untuk pengkajian dan manajemen keperawatan mandiri pada pasien kanker paru. Hasil penerapan menunjukkan bahwa mobile phone application bermanfaat bagi pasien dalam manajemen gejala secara mandiri.

Nursing Specialist Practice is an important part of professional education in order to apply the role of specialist nurses to improve service quality. Nursing specialist practice is carried out and developed based on science and technology. Competency that must be achieved during the residency practice of surgical medical nursing is to provide nursing care using the theory of nursing models, application of scientific evidence-based nursing practice, and do innovation projects. The competency target in the application of scientific evidence-based nursing actions is the application of acupressure therapy to reduce nausea and vomiting in lung cancer patients undergoing chemotherapy.

The results of the application showed that acupressure effectively reduced nausea and vomiting on patients with lung cancer undergoing chemotherapy. The next role as an innovator, innovation projects carried out in the form of the use of mobile phone applications for the assessment and management of independent nursing on patients with lung cancer. The results of the application show that mobile phone applications are beneficial for patients in symptom management independently."
