Latar belakang : Penegakan miastenia gravis (MG) didasarkan pada gejala klinis, pemeriksaan fisik dan pemeriksaan penunjang, sampai saat ini belum ada pemeriksaan yang menjadi standar utama dalam penegakan MG. Pemeriksaaan dalam penegakan MG yang hasilnya cepat, tidak invasif dan mudah dilakukan serta biayanya murah yaitu ice pack test (IPT) dan Repetitive Nerve Stimulation (RNS).

Metode: Disain penelitian potong lintang menggunakan data primer dan rekam medis pasien yang dicuriga MG dengan ptosis di Poliklinik Saraf, Instalasi Gawat Darurat (IGD), dan Ruang Rawat Inap di RSUPN Cipto Mangunkusumo (RSCM) sejak Juli 2019 – November 2019.

Hasil: Dari 38 subjek penelitian dengan ptosis, didapatkan 35 subjek terkonfirmasi MG (SF-EMG/Achr antibodi/respon terapi), 19 di antaranya (54,29%) MG jenis okular dan 16 (45,71%) MG jenis general. Hasil ice pack test positif pada 29 subjek (76,3%). Hasil uji diagnostik pemeriksaan ice pack test diperoleh sensitivitas 80%, spesifisitas 66,67%, nilai AUC 73,3%; Hasil uji diagnostik pemeriksaan RNS diperoleh sensitivitas 60%, spesifisitas 100%, nilai AUC 80%; Sedangkan uji diagnostik kombinasi pemeriksaan diperoleh sensitivitas 94,28%, spesifisitas 66,67%, nilai AUC 80,5%.

Kesimpulan: Kombinasi pemeriksaan Ice Pack Test dan Repetitive Nerve Stimulation (RNS) memiliki nilai diagnostik yang baik, sehingga dapat digunakan sebagai pemeriksaan penunjang untuk menegakkan diagnosis MG di RSUPN Cipto Mangunkusumo.

Background: Diagnosis of myasthenia gravis (MG) is based on clinical symptoms, physical examination and supporting examination, so far there has been no examination that has become the main standard in the enforcement of MGSupporting  examination of MG that are fast, non-invasive and easy to do are ice pack test (IPT) and Repetitive Nerve Stimulation (RNS).

Methods: This study was conducted with cross-sectional design using primary data and medical records of suspicious MG patients with ptosis in Cipto Mangunkusumo General Hospital between july-november 2019.

Results: Of the 38  subjects with ptosis, 35 subjects were confirmed MG with SF-EMG/AChR antibodies/respon therapy, 19 (54,29 %) ocular type and 16 (45,71%) general type. The ice pack test was positive in 29 subjects (76,3 %). The diagnostic test results of the ice pack test has sensitivity 80%, Specificity 66,67%, AUC(area under the curve)  value 73,3%; the RNS has sensitivity 60%, Specificity 100%, AUC value 80%; while the combination test has sensitivity 94,28%, Specificity 66,67% and AUC(area under the curve) value 80,5%

Conclusions: The combination of IPT and RNS has good diagnostic value, so that it can be used as a supporting examination to diagnosis of MG in Cipto mangunkusumo general hospital.

"Cold chain product atau produk rantai dingin merupakan obat-obatan yang harus disimpan pada suhu dingin untuk menjaga kualitas obat. Dalam pendistribusian produk rantai dingin PBF Enseval menggunakan box Styrofoam dan, polyurethane box, yang dilengkapi dengan satu data logger dan ice pack yang diletakan di sekeliling bagian dalam dari masing masing wadah obat yang digunakan. Ice pack di atas dibuat dengan menggunakan alat Combi Cooler Freezer PF-20, sehingga Freezer yang digunakan harus dapat menjamin kualitas ice pack yang dihasilkanya. Agar menjamin kinerja freezer berjalan sesuai spesifikasi yang diinginkan, maka perlu dilakukan kualifikasi kinerja secara berkala. kualifikasi kinerja merupakan prosess yang dilakukan untuk mendapatkan dan mendokumentasikan sistem dan atau alat bekerja sesuai dengan metode dan spesifikasi yang diinginkan dan telah disetujui. Kualifikasi kinerja dilakukan dengan cara meletakan thermometer data logger yang telah dibungkus plastik ke dalam freezer di beberapa lokasi selama 3 x 24 jam. Berdasarkan hasil kualifikasi kinerja diketahui bahwa pendistribusian dingin pada combi cooler Freezer merk Gea model PF-20 F.15 tidak merata dan terjadi Penyimpangan suhu di beberapa lokasi pada freezer. Tindak lanjut yang dapat dilakukan yaitu pembuatan ice pack dilakukan pada posisi yang memenuhi kriteria kualifikasi kinerja pada freezer.

Cold chain products are medicines that must be stored at cold temperatures to maintain drug quality. PBF Enseval distributes cold chain items using Styrofoam and polyurethane boxes that features a data logger and ice packs inside the medicine containers. The Combi Cooler Freezer PF-20 was used to create the ice pack previously mentioned, so the freezer must be able to ensure the quality of the ice pack it produces. To ensure the performance of the freezer runs according to the desired specifications, it is necessary to carry out performance qualifications on a regular basis. Performance qualification is a process carried out to obtain and document systems and or work tools according to the desired and approved methods and specifications. Performance qualification is carried out by placing a plastic-wrapped data logger thermometer into the freezer at several locations for 3 x 24 hours. Based on the outcomes of performance qualifications, it can be concluded that the Gea brand Freezer combi cooler Freezer model PF-20 F.15's cold distribution is uneven and that temperature deviations exist in several places inside the freezer. The follow-up that can be done is to make ice packs in a position that meets the performance qualification criteria in the freezer."

"BPOM menetapkan Cara Distribusi Obat yang Baik (CDOB) sebagai pedoman untuk Pedagang Besar Farmasi (PBF) yang mengatur pemeliharaan suhu pada saat penerimaan, penyimpanan, dan pengiriman produk rantai dingin untuk dapat menjamin kualitas dan mutunya. Salah satu alat yang dapat digunakan untuk menjaga suhu selama pengiriman adalah ice pack. Ice pack merupakan alat yang reusable, tahan terhadap bocor, ekonomis, dan efektif untuk mempertahankan suhu selama pengiriman. Validasi pengiriman produk rantai dingin perlu dilakukan untuk mengetahui konfigurasi terbaik dan menjamin kualitas produk selama transportasi. Konfigurasi ice pack yang paling baik untuk mempertahankan suhu 2- 8°C produk rantai dingin selama waktu tertentu diperoleh dengan menggunakan alat bantu temperature data logger. Dua konfigurasi yang divalidasi adalah konfigurasi dengan 4 ice pack dan 6 ice pack. Berdasarkan data yang diperoleh, penggunaan 4 ice pack merupakan konfigurasi yang terbaik karena mampu menjaga suhu dan tidak keluar dari rentang 2-8°C selama 8 jam.

Indonesian FDA (BPOM) published Good Distribution Practice (GDP) intended to assist pharmaceutical distributors relating to temperature during receipt, storage, and delivery of cold chain products to maintain the quality and integrity of products. One of the cold chain equipment that is widely used to maintain temperature during shipment is Ice packs. Ice packs are reusable, leak proof, budget-friendly, and effective as a cooling device. Cold chain transport validation needs to be carried out with the device to confirm the best configuration and maintain the quality of products during transportation. The best ice pack configuration to maintain a temperature of 2-8°C for cold chain products for a certain time is obtained using a temperature data logger tool. Apothecary internship program at PT Anugerah Pharmindo Lestari on May 2018 aimed to figure out the roles and responsibilities of the apothecary in pharmaceutical distributors and medical device distributors and to identify the application of Good Distribution Practice (GDP) and Good Distribution Practice for Medical Devices (GDPMD) in Pharmaceutical distributors and medical device distributors. The internship was conducted for two weeks. The special assignment given entitled Store Time Analysis of Preconditioned Cold Bag For 20 Minutes. The purpose of this special assignment was to obtain how long the duration that needed for a big Cold bag to give and maintain temperature 2-8 oC after the cold bag was preconditioned for 20 minutes."

"Penyakit kardiovaskular merupakan sekumpulan penyakit yang mempengaruhi sistem peredaran darah termasuk jantung dan pembuluh darah, disebabkan oleh banyak faktor yang mencakup penyakit bawaan dan didapat. Penyakit kardiovaskular yang disebabkan oleh aterosklerosis meliputi penyakit arteri koroner, penyakit arteri serebral, dan penyakit arteri perifer dengan dua komplikasi utama infark miokard dan stroke iskemik. Perawat spesialis memiliki peran penting sebagai pemberi asuhan keperawatan langsung, peneliti, dan inovator. Peran sebagai pemberi asuhan keperawatan langsung dijalankan dengan mengelola satu kasus utama tentang asuhan keperawatan pada pasien pascaoperasi CABG dengan Low Ejection Fraction, PMI, dan topangan IABP serta 30 kasus resume keperawatan. Peran sebagai peneliti dijalankan melalui penerapan EBN tentang penggunaan gel ice pack untuk mengurangi nyeri pada pasien pascaoperasi jantung yang menjalani chest tube removal. Peran sebagai inovator dijalankan dalam penyusunan proyek inovasi tentang penerapan FICA Spiritual Assessment Tool untuk memenuhi kebutuhan spiritual pasien dengan gangguan sistem kardiovaskular. Hasil analisis praktik residensi menunjukkan bahwa pendekatan teori model konservasi Levine dapat diterapkan dalam pemberian asuhan keperawatan pada pasien dengan gangguan sistem kardiovaskular, pengaplikasian gel ice pack dapat mengurangi nyeri saat dilakukan chest tube removal pada pasien pascaoperasi jantung, dan instrumen FICA Spiritual Assessment Tool mudah untuk diterapkan sebagai upaya pemenuhan kebutuhan spiritual pasien dengan gangguan sistem kardiovaskular.

Cardiovascular diseases caused by atherosclerosis include coronary artery disease, cerebral artery disease, and peripheral artery disease, with two major complications: myocardial infarction and ischemic stroke. Specialist nurses play an important role as direct nursing care providers, researchers, and innovators. The role of direct nursing care providers is carried out by managing a major case of nursing care for post-CABG patients with Low Ejection Fraction, PMI, and IABP support, as well as 30 nursing resume cases. The role of researchers is carried out through the application of EBN on the use of ice pack gel to reduce pain in post-cardiac surgery patients undergoing chest tube removal. The role of innovators is carried out in the preparation of innovation projects on the application of the FICA Spiritual Assessment Tool to meet the spiritual needs of patients with cardiovascular disorders. The results of the residency practice analysis show that Levine's conservation model theory approach can be applied in providing nursing care to patients with cardiovascular disorders, the application of ice pack gel can reduce pain during chest tube removal in post-cardiac surgery patients, and the FICA Spiritual Assessment Tool is easy to apply as an effort to meet the spiritual needs of patients with cardiovascular disorders."
