"Era Informasi dewasa ini ditandai oleh peningkatan kebutuhan informasi di segala aspek kehidupan, termasuk di bidang kesehatan. Mutu rekam medis digunakan sebagai indikator dalam melihat kualitas pelayanan kesehatan. Pelayanan kesehatan yang baik secara umum berarti memiliki rekam medis yang baik pula. Pengelolaan rekam medis harus disesuaikan dengan ketentuan pokok baik yang dikeluarkan oleh ANRI maupun Depkes RI.Penelitian ini dilakukan di Rumah Sakit Persahabatan dan Rumah Sakit Umum Fakultas Kesehatan Universitas Kristen Indonesia, dengan tujuan untuk mengetahui gambaran pengelolaan rekam medisnya, mengidentifikasi perbedaan pengelolaan rekam medis di kedua rumah sakit, mengetahui kendala yang dihadapi dalam pengelolaan rekam medis pada kedua rumah sakit tersebut. Pengelolaan rekam medis yang dilihat dalam penelitian ini adalah disain formulir, pemberkasan & penggunaan, serta penyusutan.Data yang diperlukan dalam penelitian ini dikumpulkan dengan teknik wawancara terhadap Kepala Unit Kerja Rekam Medic dan pengamatan langsung ke Unit Kerja Rekam Medis. Untuk mempermudah pengumpulan data digunakan kisi-kisi wawancara, yang diuji validitasnya dengan pengujian validasi isi, yaitu membandingkan antara isi instrumen dengan isi materi ajaran yang telah dipelajari. Analisis data dengan teknik deskriptif.Hasil penelitian menunjukkan bahwa :(1). Pengelolaan rekam medis di Rumah Sakit Persahabatan lebih baik daripada di Rumah Sakit Umum Fakultas Kedokteran Universitas Kristen Indonesia, terlihat dalam : a) Pemberkasan & penggunaan : Memiliki fasilitas ruang simpan yang lebih baik, Ada prosedur peminjaman, Ada ketentuan pokok dalam penyimpanan. b) Penyusutan rekam medisnya : Telah dilakukan pemisahan antara rekam medis aktif dan inaktif, Memiliki Jadwal Retensi Rekam Medis, Ada prosedur pemusnahan. (2). Faktor yang mempengaruhi pengelolaan rekam medis yang lebih baik tersebut di atas : a) Sumber Daya Manusia : Latar belakang pendidikan direktur yang lebih baik, pemahaman tentang pentingnya rekam medis lebih baik pula; Persentase staf rekam medis yang berpendidikan memadai lebih besar, kinerja staf rekam medis juga lebih baik; Adanya program pengembangan dan pendidikan staf rekam medis, dapat meningkatkan pengetahuan dan ketrampilan staf rekam medis. b) Kesadaran terhadap pedoman yang berlaku dalam pengelolaan rekam medis lebih baik, yang ditunjukkan dengan melanjutkan pembuatan juklak dan protap yang disesuaikan dengan kondisi rumah sakit. c) Adanya evaluasi dan pengendalian mutu, ada usaha perbaikan dalam pengelolaan rekam medis, (3). Kendala yang dihadapi dalam pengelolaan rekam medis di Rumah Sakit Persahabatan adalah masih seringnya ditemukan ketidaklengkapan rekam medis oleh dokter yang menangani, di Rumah Sakit Umum Fakultas Kedokteran Universitas Kristen Indonesia adalah : Kurangnya pemahaman terhadap rekam medis dari pihak pimpinan rumah sakit, dokter, dan staf medis lainnya; kurangnya staf medis dari segi mutu dan jumlah; Tidak sesuainya letak unit rekam medis dengan sistem penyimpanan yang sentralisasi.

Today information era is characterized by increase of demand for information concerning all life aspects, including health. Quality of medical record is used as indicators for health service quality. In general, good health service means that medical record used are also good. Medical record management should be based on regulations issued by National Archive Republic Indonesia and Department of Health Republic Indonesia.

This research was conducted at Persahabatan Hospital and Medical Faculty Universitas Kristen Indonesia General Hospital, in order to understand medical record management at these two hospitals, to identify differences on medical records management in these two hospitals, to understand problems faced by the hospitals in applying their medical record management. Medical record management used in this research including form design, medical record filling and usage, and medical record disposal.

Data needed for this research were collected through interview with Director of Medical Record Unit and direct observation to Medical Record Unit. To support data collection, interview guidelines are used, which have been validated through content validation test, by comparing the instrument contents with materials learned at university. Data analysis uses descriptive technique.

Research result reveals that: (1). Medical record management at Persahabatan Hospital is better than in Medical Faculty Universitas Kristen Indonesia General Hospital, as indicated by: a) Its filling wig usage: Availability of better filling rooms, availability of medical record borrowing procedure, Availability of basic regulations on medical record storage. b) Medical record disposal: Active and inactive medical records are separated, Medical Record Retention Schedule is availability, Medical record disposal procedure has been established. (2). Factors affecting such better medical record management are: a) Human Resources factor: Better educational background of director, better understanding on importance of medical record; Percentage of medical record staff with higher education is bigger, better work performance of the medical record department staff Availability of development and education program for medical record department staff may increase their knowledge and skills. b) Awareness on applied regulations concerning medical record management is better, as shown by development of technical guidelines and standard operating procedures based on the hospital current conditions. c) Availability of evaluation and quality control programs, efforts to improve medical record management. (3). Problems faced in medical record management at Persahabatan Hospital is incomplete medical record often found by relevant practitioners. While problems faced in Medical Faculty Universitas Kristen Indonesia General Hospital are: Lack of understanding on medical record among the hospital executive, practitioners, and other medical staff members; Insufficient of medical staff both in term of quality and quantity; Inappropriate location of medical record unit with centralized storage system."
