"Pengkajian resep adalah bagian dari pelayanan farmasi klinik yang dilakukan oleh Apoteker untuk menganalisa adanya masalah terkait obat dan menghindari terjadinya medication error terutama pada tahap peresepan (presribing error.  Penelitian ini bertujuan untuk mengkaji resep pasien dengan diagnosis hiperlipidemia di Apotek Kimia Farma 049 dan mengetahui adanya medication error. Hiperlipidemia merupakan suatu keadaan dimana terjadi peningkatan kadar kolesterol dengan atau tanpa peningkatan kadar trigliserida dalam darah. Penelitian ini menggunakan pendekatan deskriptif dengan metode analisis data resep pasien dengan diagnosis hiperlipidemia yang diterima di apotek selama periode penelitian. Fokus utama dari kajian ini adalah untuk menilai kepatuhan resep dan jenis obat yang diresepkan. Hasil pengkajian resep yang telah dilakukan secara administratif masih terdapat beberapa aspek yang kurang lengkap, seperti; Surat Izin Praktik (SIP) dokter, alamat dokter, nomor telepon dokter, paraf dokter, umur pasien, jenis kelamin pasien, dan berat badan pasien. Berdasarkan pertimbangan klinis secara umum dapat dikatakan tidak ada permasalahan yang terlalu signifikan tetapi hanya perlu monitoring mengenai efek samping pada masing-masing obat, serta konfirmasi kepada dokter mengenai interaksi beberapa obat yang bersifat moderate.

Prescription review is part of clinical pharmacy services carried out by pharmacists to analyze drug-related problems and avoid medication errors, especially at the prescribing stage (prescribing errors).  This study aims to examine prescriptions for patients diagnosed with hyperlipidemia at Kimia Farma 049 Pharmacy and determine whether there are medication errors. Hyperlipidemia is a condition where there is an increase in cholesterol levels with or without an increase in triglyceride levels in the blood. This study used a descriptive approach with a data analysis method for prescriptions from patients diagnosed with hyperlipidemia received at the pharmacy during the research period. The main focus of this study is to assess the fulfillment of prescriptions and the type of medication promised. The results of the review of prescriptions that have been carried out administratively still contain several aspects that are incomplete, such as; Doctor's Practice License (SIP), doctor's address, doctor's telephone number, doctor's initials, patient's age, patient's gender, and patient's weight. Based on clinical considerations in general, it can be said that there are no problems that are too significant, but it is only necessary to monitor the side effects of each drug, as well as confirm with the doctor regarding moderate drug interactions."

"Hipertensi menurut WHO adalah ketika tekanan dalam pembuluh darah terlalu tinggi (140/90 mmHg atau lebih tinggi). Hal ini umum terjadi, namun dapat menjadi serius jika tidak ditangani. Seiring dengan perkembangan ekonomi, hipertensi pada awalnya mempengaruhi mereka yang memiliki status sosial ekonomi tinggi, tetapi pada tahap perkembangan ekonomi selanjutnya, prevalensi hipertensi dan konsekuensinya paling besar pada mereka yang memiliki status sosial ekonomi rendah. Tujuan dari penulisan tugas khusus ini adalah Menganalisis aspek administrasi, aspek farmasetik, dan aspek klinis resep PRB (Program Rujuk Balik) pasien dengan diagnosis hipertensi yang terdapat di apotek Kimia Farma 48 Matraman Jakarta dan Mengidentifikasi obat antihipertensi yang termasuk dalam PRB (Program Rujuk Balik). Pengambilan data dilakukan dengan cara mengambil 2 (dua) sampel resep program rujuk balik (PRB) dengan diagnosis hipertensi. Hasil yang diperoleh adalah masih terdapat ketidaksesuaian terutama dalam aspek administratif dengan peraturan Kementerian Kesehatan Nomor 35 Tahun 2014 dan Obat antihipertensi yang diresepkan sesuai dengan diagnosis pasien tersebut dan menggunakan terapi 2 kombinasi serta tercantum di dalam fornas.

Hypertension according to WHO is when the pressure in the blood vessels is too high (140/90 mmHg or higher). It is common, but can be serious if left untreated. Along with economic development, hypertension initially affects those with high socioeconomic status, but in later stages of economic development, the prevalence of hypertension and its consequences is greatest in those with low socioeconomic status. The purpose of writing this special assignment is to analyze the administrative, pharmacetic, and clinical aspects of PRB (Program Rujuk Balik) prescriptions for patients with a diagnosis of hypertension found at Kimia Farma 48 Matraman Jakarta pharmacy and to identify antihypertensive drugs included in the PRB (Program Rujuk Balik). Data collection was carried out by taking 2 (two) samples of program rujuk balik (PRB) prescriptions with a diagnosis of hypertension. The results obtained were that there were still discrepancies, especially in administrative aspects with the Ministry of Health Regulation Number 35 of 2014 and the antihypertensive drugs prescribed were in accordance with the patient's diagnosis and used 2 combination therapies and were listed in fornas."

"Peraturan Menteri Kesehatan RI Nomor 73 tahun 2016 menetapkan standar pelayanan kefarmasian di Apotek, yang bertujuan untuk meningkatkan mutu kehidupan pasien. Pelayanan kefarmasian telah berkembang dari orientasi produk menjadi berorientasi pada pasien. Pengkajian resep adalah aspek operasional penting dalam pelayanan kefarmasian, yang bertujuan untuk mengidentifikasi dan menyelesaikan permasalahan terkait obat sebelum disiapkan. Dalam pengkajian resep, terdapat tiga aspek yang diperhatikan: administratif, farmasetik, dan klinis. Tujuan penelitian ini adalah melakukan pengkajian pada resep pasien dengan diagnosis osteoarthritis di Apotek Kimia Farma 049 untuk mengidentifikasi kemungkinan kesalahan pengobatan dan memberikan saran. Ditemukan bahwa resep pasien osteoarthritis memiliki beberapa permasalahan administratif, seperti ketidaklengkapannya, tetapi dari segi farmasetik dan klinis sudah sesuai. Kesimpulan dari pengkajian adalah bahwa meskipun ada permasalahan administratif pada resep, obat yang diresepkan sudah sesuai dengan tatalaksana osteoarthritis. Saran yang diberikan adalah untuk memperhatikan aspek administratif yang kurang lengkap pada resep dan melakukan konfirmasi kepada pasien atau dokter jika diperlukan.

Minister of Health Regulation No. 73 of 2016 sets the standard for pharmaceutical services in pharmacies, aimed at improving the quality of patient life. Pharmaceutical services have evolved from product-oriented to patient-oriented. Prescription assessment is a crucial operational aspect in pharmaceutical services, aiming to identify and resolve medication-related issues before preparation. In prescription assessment, three aspects are considered: administrative, pharmaceutical, and clinical. The aim of this study is to assess prescriptions for patients diagnosed with osteoarthritis at Kimia Farma Pharmacy 049 to identify potential medication errors and provide recommendations. It was found that prescriptions for osteoarthritis patients had some administrative issues, such as incompleteness, but were adequate in terms of pharmaceutical and clinical aspects. The conclusion of the assessment is that despite administrative issues in prescriptions, the prescribed medications are suitable for osteoarthritis management. Recommendations include paying attention to incomplete administrative aspects in prescriptions and confirming with patients or doctors if necessary."

"Laporan Praktik Kerja Profesi Apoteker ini membahas terkait kesesuaian resep pasien BPJS Kronis Poli Jantung dan Pembuluh di Apotek Kimia Farma 0389 jika ditinjau dari segi administratif, farmasetik, dan klinis yang dianalisis melalui hasil DRPs (Drug Related Problems) yang terjadi. Penelitian ini dilakukan secara observasional dengan metode deskriptif. Dari hasil penelitian ini disarankan agar kegiatan pemberian informasi obat dan konseling oleh apoteker lebih ditingkatkan terutama pada pasien polifarmasi yang memiliki risiko interaksi antara obat yang digunakan dan perlu adanya peningkatan pengetahuan bagi pihak rumah sakit atau penulis resep melalui sosialisasi peraturan obat yang dipersyaratkan oleh BPJS sehingga pasien bisa mengklaim obat sesuai dengan apa yang doketr resepkan untuk menghindari indikasi yang tidak diobatai karena ketidaktersediaan obat dan fakktor tidak sesuai persyaratan BPJS.

This Pharmacist Professional Work Practices Report discusses the suitability of prescriptions for BPJS Chronic Heart and Vessel Poly patients at Kimia Farma 0389 Pharmacy when viewed from an administrative, pharmaceutical, and clinical perspective which is analyzed through the results of the DRPs (Drug Related Problems) that occur. This research was conducted observationally using descriptive methods. From the results of this research, it is recommended that the activities of providing drug information and counseling by pharmacists be improved, especially for poly pharmacy patients who have a risk of interactions between the drugs used and there needs to be increased knowledge for hospitals or prescribers through socialization of drug regulations required by BPJS so that patients can claim medicine according to what the doctor prescribed to avoid indications that are not treated due to unavailability of medicine and factors that do not meet BPJS requirements."

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.

Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Program Jaminan Kesehatan Nasional (JKN) merupakan bentuk kepedulian pemerintah terhadap akses kesehatan pada masyarakat sehingga masyarakat dapat memperoleh pengobatan yang berkualitas, terjamin mutunya, dan terjangkau. Sebagai wujud keselarasan dengan tujuan adanya JKN, maka apoteker perlu memaksimalkan pengobatan yang diberikan kepada pasien. This prescription review will reduce the incidence of irrational use of drugs. This prescription review was conducted on patients participating in the BPJS Health Referral Program (PRB) and on general patients."

Data didapatkan dengan mengklasifikasikan resep yang diterima di Apotek Kimia Farma 389 berdasarkan tanggal diterima resep dan asal rumah sakit pasien. Data yang diambil adalah sebanyak lima resep pasien penyakit kronis RS Bhakti Yudha Depok selama bulan Oktober 2023 di Apotek Kimia Farma 389 yang diambil secara acak.Pada resep pasien ditemukan beberapa masalah terkait obat pada aspek administratif seperti paraf/tanda tangan dokter, berat badan pasien, dan alamat pasien. Masalah terkait obat padapertimbangan klinis seperti dosis obat yang kurang dan interaksi antara obat yang perlu diperhatikan. Masalah tersebut dapat diatasi dengan mengatur jadwal konsumsi obat pasien dan mengkonsultasikan kembali kepada dokter terkait pemberian obat yang kurang dosis.Perlu dilakukan pengkajian resep secara rutin untuk menjamin rasionalitas dankesesuaian dosis obat yang akan diterima pasien dan meminimalisir terjadinya kesalahan medikasi (medication error) dan masalah terkait obat.

A pharmacy is a place that provides, stores and distributes medicines and other health products. Pharmacies also act asinformation centers, health services, and as chronic disease management partners. The provision of pharmaceuticalservices in pharmacies must be able to guarantee the availability of safe, quality and efficacious medicines. Reviewing patient prescriptions is important in maintaining the quality of pharmaceutical services and preventing medication errors. Data was obtained by classifying prescriptions received at Kimia Farma 389 Pharmacy based on the date theprescription was received and the patient's hospital origin. The data taken were five prescriptions from patients withchronic diseases at Bhakti Yudha Hospital, Depok during October 2023 at Kimia Farma 389 Pharmacy which weretaken randomly. In the patient prescriptions, several drug-related problems were found in administrative aspects such as the doctor's initials/signature, the patient's weight, and patient address. Drug-related problems in clinical considerations such as insufficient drug dosage and interactions between drugs that need to be considered. This problem can be overcome by arranging the patient's medication consumption schedule and consulting the doctor again regarding administering insufficient doses of medication. It is necessary to review prescriptions regularly to ensure the rationality and suitability of the dosage of medication that the patient will receive and minimize the occurrence of medication errors and problems. drug related."

"Masalah terkait peresepan obat merupakan kejadian medication error yang dapat merugikan pasien. Salah satu upaya yang bisa dilakukan untuk mencegah masalah ini ialah melalui kegiatan pengkajian resep. Oleh sebab itu dalam karya tulis ini dilakukan pengkajian terhadap kelengkapan administratif, farmasetik serta keseuaian klinis pada resep dari 5 pasien poli saraf yang diterima oleh apotek Kimia Farma unit 0389 dalam rangka melihat dan mengevaluasi permasalahan yang sering terjadi dalam peresepan obat. Hasil pengkajian terhadap kelima resep obat tersebut menunjukkan bahwa dari aspek administrasinya, drug related problem atau medication error yang sering terjadi ialah mengenai tidak tercantumnya informasi terkait alamat dokter, nomor telepon dokter, alamat pasien serta berat badan pasien. Berdasarkan aspek farmasetiknya, informasi mengenai bentuk sediaan obat serta cara dan lama pemberian obat tidak tercantum pada 3 dari 5 resep yang dikaji. Pengkajian terhadap kesesuain klinis menunjukkan bahwa drug related problem yang terjadi pada resep yang dikaji ialah DRP kategori P2 mengenai keamanan pengobatan dengan domain sekunder berupa kejadian obat merugikan yang mungkin terjadi. DRP tersebut diantaranya adalah adanya polifarmasi obat, interaksi obat serta efek samping obat yang merugikan.

Problems related to drug prescribing are medication errors that can harm patients. One effort that can be made to prevent this problem is through reviewing recipes. Therefore, in this paper, an assessment was carried out on the completeness of the administration, pharmacy and clinical suitability of prescriptions from 5 polyneurology patients received at Kimia Farma 0389 Sin order to see and evaluate problems that often occur in drug prescribing. The results of research on the five drug prescriptions show that from the aspect of drug administration, the most common drug-related problems or medication errors that occur are the absence of information regarding the doctor's address, doctor's telephone number, patient address, and patient's weight. Based on the pharmaceutical aspect, information regarding the dosage form of the drug as well as the method and duration of drug administration was not included in 3 of the 5 prescriptions studied. The clinical suitability assessment showed that the problem related to the drug that occurred was DRP category P2 regarding drug safety with a secondary domain in the form of the possibility of drug side effects. This DRP includes drug polypharmacy, drug interactions, and drug side effects."

"Hipertensi adalah salah satu penyakit yang menjadi masalah kesehatan terbesar di dunia, termasuk di Indonesia dengan prevalensi sebesar 25,8% pada tahun 2013. Apotek Kimia Farma Raya Bogor bekerja sama dengan Rumah Sakit Tugu Ibu dalam pemberian obat pada pasien BPJS rawat jalan. Dalam kajian ini, lima resep antihipertensi dipilih untuk dievaluasi dari aspek administrasi, farmasetis, dan klinis. Hasil pengkajian menunjukkan bahwa resep-resep tersebut memenuhi semua aspek yang diharuskan kecuali informasi berat badan yang tidak ada. Telaah klinis menunjukkan bahwa semua resep tepat indikasi, dosis, dan pemberiannya.

Hypertension is one of the major global health problems, including in Indonesia with a prevalence of 25.8% in 2013. Apotek Kimia Farma Raya Bogor collaborates with Tugu Ibu Hospital in providing medication for BPJS outpatients. In this study, five antihypertensive prescriptions were selected for evaluation from administrative, pharmaceutical, and clinical aspects. The evaluation results indicate that the prescriptions meet all required aspects except for the missing weight information. Clinical review shows that all prescriptions are appropriate in indication, dosage, and administration."

"Penggunaan obat yang rasional adalah faktor penting dalam keberhasilan pelayanan kefarmasian. Pengkajian resep obat asma di Apotek Kimia Farma 202 Kejayaan Depok dilakukan untuk menilai kesesuaian resep dengan standar yang berlaku. Penelitian ini bertujuan untuk mengkaji resep obat asma dari segi administrasi, farmasetik, dan klinis serta memahami permasalahan yang muncul dari pengkajian resep tersebut. Pengkajian dilakukan terhadap resep asma yang diterima di Apotek Kimia Farma 202 selama bulan Juli 2023. Data dikumpulkan melalui observasi langsung dan wawancara dengan apoteker. Analisis dilakukan berdasarkan aspek administrasi, farmasetik, dan klinis sesuai Peraturan Menteri Kesehatan No. 73 Tahun 2016. Hasil analisis menunjukkan bahwa sebagian besar resep sudah memenuhi standar administrasi, namun terdapat beberapa kekurangan dalam aspek farmasetik dan klinis, seperti ketidaktepatan dosis dan bentuk sediaan obat yang tidak tercantum dengan lengkap. Peningkatan kualitas pelayanan resep di Apotek Kimia Farma 202 dapat dicapai melalui peningkatan pengawasan dan edukasi kepada tenaga kesehatan mengenai penulisan resep yang lengkap dan jelas.

Rational drug use is a critical factor in the success of pharmaceutical services. The evaluation of asthma medication prescriptions at Apotek Kimia Farma 202 Kejayaan Depok was conducted to assess the conformity of prescriptions with applicable standards. This study aims to review asthma drug prescriptions in terms of administration, pharmaceutics, and clinical aspects, as well as to understand the issues arising from this evaluation. The review was carried out on asthma prescriptions received at Apotek Kimia Farma 202 during July 2023. Data were collected through direct observation and interviews with pharmacists. Analysis was conducted based on administrative, pharmaceutical, and clinical aspects in accordance with Minister of Health Regulation No. 73 of 2016. The results of the analysis indicated that most prescriptions met administrative standards; however, there were some deficiencies in pharmaceutical and clinical aspects, such as inaccurate dosages and incomplete drug formulations. Improving the quality of prescription services at Apotek Kimia Farma 202 can be achieved through enhanced supervision and education for healthcare professionals regarding the writing of complete and clear prescriptions."
