"Apotek merupakan sarana pelayanan kefarmasian tempat dilakukan praktik kefarmasian dimana apoteker sebagai penangung jawab praktik yang telah diatur dalam undang-undang. Standar pelayanan kefarmasian menjadi tolak ukur yang dipergunakan sebagai pedoman bagi tenaga kefarmasian dalam menyelenggarakan pelayanan kefarmasian. Resep tertulis dari dokter dapat menyumbangkan pemasukan yang tinggi terhadap apotek. Semakin banyak resep obat dari dokter yang masuk akan memberikan keuntungan dan membuat keuangan menjadi lebih sehat untuk operasional apotek. Analisis keuntungan apotek dapat dilihat melalui metode pareto atau ABC. Metode ini memfokuskan pengelola apotek dalam menganalisis keuntungan operasional apotek yang didapat dari resep dokter yang masuk sehingga dapat mengetahui resep dokter yang memberi pemasukan yang besar bagi omzet apotek. Penggunaan metode ini akan mengelompokkan menjadi kelompok A, B, dan C berdasarkan nilai pemakaian dan nilai investasi. Pelaksanaan analisis pareto terhadap resep yang diterima Kimia Farma 382 Depok menggunakan pendekatan kuantitatif dengan menggunakan metode analisis pareto. Hasil pendapatan dari resep dokter klinik audi dental untuk Apotek Kimia Farma 382 Depok pada periode februari 2023 sebesar Rp21.393.535,00.

A pharmacy is a pharmaceutical service facility where pharmaceutical practice is carried out where the pharmacist is responsible for the practice as regulated by law. Pharmaceutical service standards are benchmarks used as guidelines for pharmaceutical personnel in providing pharmaceutical services. The more drug prescriptions that come in from doctors will provide profits and make finances healthier for pharmacy operations. Pharmacy profit analysis can be seen using the Pareto or ABC method. This method focuses pharmacy managers on analyzing the operational profits of pharmacies obtained from incoming doctor's prescriptions so that they can find out which doctor's prescriptions provide large income for pharmacy turnover. Using this method will group them into groups A, B, and C based on usage value and investment value. "Perencanaan merupakan tahapan yang penting karena faktor perencanaan obat yang baik dapat mencegah terjadinya kekurangan dan kelebihan stok obat, serta pemborosan anggaran. Perencanaan merupakan kegiatan penentuan penyusunan daftar kebutuhan obat (jenis dan jumlah) sesuai dengan kebutuhan dan anggaran, sebelum dilakukannya proses pengadaan. Terdapat tiga metode yang dapat digunakan untuk perhitungan kebutuhan pada perencanaan, diantaranya Metode Konsumsi, Metode Morbiditas dan Metode Proxy Consumption. Pada tahap perencanaan juga terdapat analisis atau evaluasi rencana kebutuhan sediaan farmasi yang bertujuan untuk menjamin ketersediaan obat dan efesiensi anggaran. Analisis perencanaan pengadaaan obat tersebut dapat dilakukan dengan berbagai metode, yaitu Analisis ABC, Analisis VEN, dan Analisis Kombinasi. Procurement is the activity of determining the preparation of a list of drug requirements (type and quantity) in accordance with needs and budget, before the procurement process is carried out. There are three methods that can be used to calculate needs in procurement, including the Consumption Method, Morbidity Method and Proxy Consumption Method. At the planning stage there is also an analysis or evaluation of the planned need for pharmaceutical supplies which aims to ensure drug availability and budget efficiency. Analysis of drug procurement planning can be carried out using various methods, namely ABC Analysis, VEN Analysis and Combination Analysis. Among these three methods, the ABC Pareto analysis method is a method for evaluating pharmaceutical supply planning in pharmacies that is often used. ABC Pareto analysis is used to determine the priority of items used in pharmacies by looking at the cumulative percentage of the amount of use (use value), the cumulative percentage of the investment amount (investment value), as well as the critical index value (through the total score of use value and investment value). ABC Pareto analysis can help pharmacists to plan medicines or goods with the most profitable economic value, so that they can gain more and faster profits from selling these medicines."

Penyelenggaraan pelayanan kefarmasian di Apotek harus dapat menjamin ketersediaan obat yang aman, bermutu, dan juga berkhasiat. engkajian resep pasien merupakan hal yang penting dalam menjaga kualitas pelayanan kefarmasian dan mencegah terjadinya medication error. Data didapatkan dengan mengklasifikasikan resep yang diterima di Apotek Kimia Farma 389 berdasarkan tanggal diterima resep dan asal rumah sakit pasien. Data yang diambil adalah sebanyak lima resep pasien penyakit kronis RS Bhakti Yudha Depok selama bulan Oktober 2023 di Apotek Kimia Farma 389 yang diambil secara acak.Pada resep pasien ditemukan beberapa masalah terkait obat pada aspek administratif seperti paraf/tanda tangan dokter, berat badan pasien, dan alamat pasien. Masalah terkait obat padapertimbangan klinis seperti dosis obat yang kurang dan interaksi antara obat yang perlu diperhatikan. Masalah tersebut dapat diatasi dengan mengatur jadwal konsumsi obat pasien dan mengkonsultasikan kembali kepada dokter terkait pemberian obat yang kurang dosis.Perlu dilakukan pengkajian resep secara rutin untuk menjamin rasionalitas dankesesuaian dosis obat yang akan diterima pasien dan meminimalisir terjadinya kesalahan medikasi (medication error) dan masalah terkait obat.

A pharmacy is a place that provides, stores and distributes medicines and other health products. Pharmacies also act asinformation centers, health services, and as chronic disease management partners. The provision of pharmaceuticalservices in pharmacies must be able to guarantee the availability of safe, quality and efficacious medicines. Reviewing patient prescriptions is important in maintaining the quality of pharmaceutical services and preventing medication errors. Data was obtained by classifying prescriptions received at Kimia Farma 389 Pharmacy based on the date theprescription was received and the patient's hospital origin. The data taken were five prescriptions from patients withchronic diseases at Bhakti Yudha Hospital, Depok during October 2023 at Kimia Farma 389 Pharmacy which weretaken randomly. In the patient prescriptions, several drug-related problems were found in administrative aspects such as the doctor's initials/signature, the patient's weight, and patient address. Drug-related problems in clinical considerations such as insufficient drug dosage and interactions between drugs that need to be considered. This problem can be overcome by arranging the patient's medication consumption schedule and consulting the doctor again regarding administering insufficient doses of medication. It is necessary to review prescriptions regularly to ensure the rationality and suitability of the dosage of medication that the patient will receive and minimize the occurrence of medication errors and problems. drug related."

"Apotek Kimia Farma Unit 389 merupakan salah satu apotek yang melayani resep pasien BPJS di Depok. Laporan ini disusun untuk mengkaji dan menganalisis resep di Apotek Kimia Farma Unit 389 Depok sehingga dapat memberikan informasi mengenai kelengkapan resep dan kejadian 'drug related problems' (DRPs) pada resep. Sampel yang digunakan berjumlah lima resep pasien BPJS kronis poli penyakit dalam di Apotek Kimia Farma Unit 389 periode April 2023. Hasil pengkajian dan analisis resep menunjukkan bahwa semua resep tidak lengkap ditinjau dari aspek administratif, aspek farmasetik, dan aspek klinis berdasarkan Peraturan Menteri Kesehatan Nomor 73 Tahun 2016 tentang Standar Pelayanan Kefarmasian di Apotek. Kejadian DRPs terjadi pada semua resep. DRPs yang berkaitan dengan masalah efektivitas terapi terjadi pada resep 1 dan 3, sedangkan yang berkaitan dengan masalah keamanan terapi terjadi pada resep 1, 2, 4, dan 5.

Kimia Farma Pharmacy Unit 389 is one of the pharmacies that serve prescriptions for BPJS patients in Depok. This report was prepared to assess and analyze prescriptions at the Kimia Farma Pharmacy Unit 389 Depok so that it can provide information about the completeness of prescriptions and the incidence of drug related problems (DRPs) in prescriptions. The sample used consisted of five prescriptions for chronic BPJS patients of internal medicine poly at Kimia Farma Pharmacy Unit 389 for the period of April 2023. The results of the assessment and analysis of prescriptions showed that all prescriptions were incomplete in terms of administrative aspects, pharmaceutical aspects, and clinical aspects based on the Regulation of the Minister of Health Number 73 of 2016 concerning Pharmaceutical Service Standards in Pharmacies. DRPs occurred in all prescriptions. DRPs related to therapeutic effectiveness issues occurred in prescriptions 1 and 3, while those related to therapeutic safety issues occurred in prescriptions 1, 2, 4, and 5."

"Apoteker bertanggung jawab dalam menjalankan pelayanan kefarmasian untuk meningkatkan kualitas hidup pasien. Pelayanan kefarmasian di apotek meliputi 2 kegiatan yaitu pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai (BMHP) serta pelayanan farmasi klinik dalam bentuk pelayanan obat atas resep dokter. Dalam alur pelayanan resep, apoteker menganalisis adanya permasalahan terkait obat (drug related problem) yang dapat mengganggu keberhasilan proses terapi (Kemenkes RI, 2016). Tugas khusus ini bertujuan untuk menganalisis kelengkapan resep obat penyakit kronis dari RS Graha Permata Ibu di Apotek Kimia Farma 389 periode Oktober 2023 dengan meninjau aspek administratif, farmasetik, dan pertimbangan klinis khususnya terkait efek samping dan interaksi obat sesuai PerMenkes No. 73 Tahun 2016. Pada analisis ini, metode pemilihan sampel resep yang digunakan adalah metode Purposive Sampling. Sebelum dilakukan analisis, resep obat yang masuk di Apotek Kimia Farma 389 dipisahkan berdasarkan beberapa kriteria. Kemudian, dilakukan analisis kelengkapan resep obat terkait aspek administratif, farmasetik, dan pertimbangan klinis untuk mengetahui masalah terkait pengunaan obat pada pasien penyakit kronis tersebut. Hasil pengkajian resep obat penyakit kronis pada pasien RS GPI yang masuk di Apotek Kimia Farma 389 menunjukkan beberapa masalah terkait kelengkapan resep, terutama pada aspek administratif dan pengkajian klinis.

Pharmacists are responsible for carrying out pharmaceutical services to improve the quality of life of patients. Pharmaceutical services in pharmacies include 2 activities, which are the management of pharmaceutical preparations, medical devices, and consumable medical materials and clinical pharmacy services in the form of prescription drug services. In the flow of prescription services, pharmacists analyze drug-related problems that can interfere with the success of the therapeutic process (Ministry of Health of the Republic of Indonesia, 2016). Prescription review has an important role in ensuring the legality of a prescription and minimizing therapeutic failures caused by prescription writing errors or inappropriate drug information (Harahap, 2020). Identifying potential interactions between drugs listed during prescription review can improve pharmacists' understanding in providing drug information to patients with chronic diseases (Kurniapuri & Supadmi, 2015). This special assignment aims to analyze the completeness of chronic disease drug prescriptions from Graha Permata Ibu Hospital at Kimia Farma Pharmacy 389 for the period October 2023 by reviewing administrative aspects, pharmaceuticals, and clinical considerations, especially related to side effects and drug interactions according to PerMenkes No. 73 of 2016. In this analysis, the prescription sample selection method used was the Purposive Sampling method. Before the analysis was carried out, incoming drug prescriptions at Apotek Kimia Farma 389 were separated based on several criteria. Then, an analysis of the completeness of drug prescriptions related to administrative aspects, pharmaceuticals, and clinical considerations was carried out to find out problems related to the use of drugs in these chronic disease patients. The results of the assessment of chronic disease drug prescriptions in GPI Hospital patients admitted to Kimia Farma 389 Pharmacy showed several problems related to prescription completeness, especially in administrative aspects and clinical assessment."

"Penulisan laporan tugas khusus praktik kerja profesi Apoteker (PKPA) di Apotek Kimia Farma 048 Matraman bertujuan untuk mengidentifikasi penyakit kronis dan menganilisis resep untuk pasien rujuk balik (PRB) berdasarkan kajian yang berlaku di Apotek. Penulisan laporan ini menggunakan data resep pasien rujuk balik sebanyak 20 resep yang memiliki riwayat penyakit kronis kemudian rujuk balik ke Apotek Kimia Farma 048 Matraman. Hasil yang didapatkan adalah rata – rata pasien yang dirujuk balik ke Apotek memiliki obat untuk penyakit kronis seperti Hipertensi yaitu Amlodipin Besilat, Bisoprolol. Selain itu, penyakit kronis lainnya terdapat Diabetes Melitus dengan contoh obat yang didapatkan seperti Metformin, Glimepirid, dan Insulin baik insulin kerja panjang maupun kerja pendek. Dari obat – obat tersebut dianalisis bagaimana mekanisme kerjanya dan indikasi serta dosis yang direkomendasikan di resep telah sesuai atau belum sesuai. Tidak hanya analisis resep, namun pengkajian kesesuaian resep juga dilakukan berdasarkan persyaratan administratif, farmasetik, dan klinis. Dari analisis 20 resep tersebut, diketahui bahwa resep telah sesuai dengan persyaratan administratif, farmasetik, dan klinis sehingga disimpulkan bahwa telah memenuhi aturan yang berlaku.

A special assignment report pharmacist professional internship is writing at Apotek Kimia Farma with the purpose to identified chronic disease and to analyzed prescriptions for patients with referral back (PRB) based on applicable studies at pharmacy. The writing of this report used prescriptions data for 20 referral patients who have a history of chronic disease and then referral back to Apotek Kimia Farma 048 Matraman. The results was obtained that the average of the patients who has referral back to drugstore have drugs for chronic disease such as hypertension, like Amlodipine Besylate and Bisoprolol. In addition to, another chronic disease there’s Diabetic with the example of drugs are Metformin, Glimepiride and Insulin both long acting and short-acting insulin. From these drug, it was analyzed how the mechanism of action and the indications and the dosage was recommended in the prescriptions was appropiate or not. Not only prescriptions was analyzed, but also the assessment of the prescriptions suitability was carried out based on administrative, pharmaceutical, and clinical requirements. From the analyzed of the 20 prescriptions, it was found that the prescriptions were in accordance with administrative, pharmaceutical, and clinical requirements so that it was concluded that they had met the applicable regulations."

"Penggunaan obat yang rasional adalah faktor penting dalam keberhasilan pelayanan kefarmasian. Pengkajian resep obat asma di Apotek Kimia Farma 202 Kejayaan Depok dilakukan untuk menilai kesesuaian resep dengan standar yang berlaku. Penelitian ini bertujuan untuk mengkaji resep obat asma dari segi administrasi, farmasetik, dan klinis serta memahami permasalahan yang muncul dari pengkajian resep tersebut. Pengkajian dilakukan terhadap resep asma yang diterima di Apotek Kimia Farma 202 selama bulan Juli 2023. Data dikumpulkan melalui observasi langsung dan wawancara dengan apoteker. Analisis dilakukan berdasarkan aspek administrasi, farmasetik, dan klinis sesuai Peraturan Menteri Kesehatan No. 73 Tahun 2016. Hasil analisis menunjukkan bahwa sebagian besar resep sudah memenuhi standar administrasi, namun terdapat beberapa kekurangan dalam aspek farmasetik dan klinis, seperti ketidaktepatan dosis dan bentuk sediaan obat yang tidak tercantum dengan lengkap. Peningkatan kualitas pelayanan resep di Apotek Kimia Farma 202 dapat dicapai melalui peningkatan pengawasan dan edukasi kepada tenaga kesehatan mengenai penulisan resep yang lengkap dan jelas.

Rational drug use is a critical factor in the success of pharmaceutical services. The evaluation of asthma medication prescriptions at Apotek Kimia Farma 202 Kejayaan Depok was conducted to assess the conformity of prescriptions with applicable standards. This study aims to review asthma drug prescriptions in terms of administration, pharmaceutics, and clinical aspects, as well as to understand the issues arising from this evaluation. The review was carried out on asthma prescriptions received at Apotek Kimia Farma 202 during July 2023. Data were collected through direct observation and interviews with pharmacists. Analysis was conducted based on administrative, pharmaceutical, and clinical aspects in accordance with Minister of Health Regulation No. 73 of 2016. The results of the analysis indicated that most prescriptions met administrative standards; however, there were some deficiencies in pharmaceutical and clinical aspects, such as inaccurate dosages and incomplete drug formulations. Improving the quality of prescription services at Apotek Kimia Farma 202 can be achieved through enhanced supervision and education for healthcare professionals regarding the writing of complete and clear prescriptions."

"Hipertensi adalah salah satu penyakit yang menjadi masalah kesehatan terbesar di dunia, termasuk di Indonesia dengan prevalensi sebesar 25,8% pada tahun 2013. Apotek Kimia Farma Raya Bogor bekerja sama dengan Rumah Sakit Tugu Ibu dalam pemberian obat pada pasien BPJS rawat jalan. Dalam kajian ini, lima resep antihipertensi dipilih untuk dievaluasi dari aspek administrasi, farmasetis, dan klinis. Hasil pengkajian menunjukkan bahwa resep-resep tersebut memenuhi semua aspek yang diharuskan kecuali informasi berat badan yang tidak ada. Hypertension is one of the major global health problems, including in Indonesia with a prevalence of 25.8% in 2013. Apotek Kimia Farma Raya Bogor collaborates with Tugu Ibu Hospital in providing medication for BPJS outpatients. In this study, five antihypertensive prescriptions were selected for evaluation from administrative, pharmaceutical, and clinical aspects. The evaluation results indicate that the prescriptions meet all required aspects except for the missing weight information. Clinical review shows that all prescriptions are appropriate in indication, dosage, and administration."

Screening prescriptions, especially for polypharmacy, is essential to prevent drug-related problems and medication errors.Research Problem and Aim: The research problem focuses on the importance of screening prescriptions, particularly for polypharmacy, to prevent medication errors and improve patient outcomes. The aim of the study is to investigate the impact of prescription screening on reducing medication errors and enhancing patient safety in a pharmacy setting.Methodology: The research design involves conducting prescription screening, including administrative, pharmaceutical, and clinical screening, to ensure the legality of prescriptions and minimize treatment errors. The study participants consist of pharmacists working at Apotek Roxy Beji, and the context is within the period of July during a Professional Work Practice of a Pharmacist (PKPA) program. The findings indicate that prescription screening, especially for polypharmacy, is essential in preventing drug-related problems and medication errors. The screening process helps in avoiding misinterpretations, communication failures, and medication errors between healthcare professionals, ultimately improving patient safety and quality of life. The study highlights the significance of prescription screening in reducing medication errors and enhancing patient safety. "Kegiatan pengkajian resep meliputi kajian administrasi, kesesuaian farmasetik, dan pertimbangan klinis. Jika ditemukan adanya ketidaksesuaian dari hasil pengkajian apoteker harus menghubungi dokter penulis resep. Pada setiap tahap alur pelayanan Resep dilakukan upaya pencegahan terjadinya kesalahan pemberian obat (medication error). Penyusunan tugas khusus Praktik Kerja Profesi Apoteker di Apotek Roxy Sawangan bertujuan untuk mengidentifikasi kelengkapan resep obat terapi gangguan pencernaan berdasarkan aspek administratif, aspek farmasetik dan aspek klinis yang terdapat pada Apotek Roxy Sawangan periode April 2023.Tugas khusus ini disusun dengan metode deskriptif dan melakukan kegiatan pengkajian resep pada dua resep pasien di Apotek Roxy Sawangan. Obat yang terdapat pada kedua resep tersebut memiliki indikasi yang beririsan yaitu sebagai terapi gangguan pencernaan. Berdasarkan pengkajian resep yang dilakukan terhadap kedua resep dapat disimpulkan pada resep 1 aspek administratif dan farmasetik sudah lengkap tetapi pada aspek klinis menunjukkan ketidaktepatan dosis pemberian sirup sukralfat. Selanjutnya pada resep 2, aspek administratif kurang lengkap karena tidak ada alamat , paraf dokter dan berat badan pasien pada aspek administratif dan farmasetik sudah lengkap dan sesuai.

The preparation of a special assignment for Pharmacist Professional Work Practice at Roxy Sawangan Pharmacy aims to identify the completeness of the prescription of indigestion therapy drugs based on administrative aspects, pharmaceutical aspects and clinical aspects found at Roxy Sawangan Pharmacy for the period April 2023.This special assignment was prepared using a descriptive method and carried out prescription review activities on two patient prescriptions at Roxy Sawangan Pharmacy. The drugs contained in the two prescriptions have indications that overlap, namely as therapy for indigestion. Based on the prescription review carried out on the two prescriptions, it can be concluded that in recipe 1 the administrative and pharmaceutical aspects are complete but the clinical aspects show the inaccuracy of the dose of sucralfate syrup. Furthermore, in prescription 2, the administrative aspect is incomplete because there is no address, the doctor's initials and the patient's weight in the administrative and pharmaceutical aspects are complete and appropriate."
