"Praktik Kerja Profesi Apoteker (PKPA) merupakan salah satu upaya untuk memberikan bekal keterampilan pada mahasiswa apoteker agar memiliki keterampilan kefarmasian sehingga  nantinya calon apoteker diharapkan mampu melakukan praktik kefarmasian secara profesional, legal, dan etik di Puskesmas. Pelaksanaan praktik kerja profesi ini berlangsung selama dua minggu dengan tugas khusus, yakni Evaluasi Rasionalitas Penggunaan Antipsikotik Pasien Skizofrenia di Puskesmas Kecamatan Kramat Jati Periode Oktober 2022. Tujuan dilakukannya tugas khusus ini untuk mengevaluasi rasionalitas peresepan antipsikotik pada pasien skizofrenia di Puskesmas Kecamatan Kramat Jati tahun 2022. Metode yang digunakan pada tugas khusus ini adalah menggunakan desain observasional deskriptif dengan teknik pengambilan sampel secara retrospektif pada pasien skizofrenia periode bulan Oktober 2022. Subjek pada penelitian, yakni data resep pasien skizofrenia Puskesmas Kecamatan Kramat Jati bulan Oktober 2022. Instrumen penelitian yang digunakan, yakni data excel yang diperoleh melalui website ePuskesmas. Berdasarkan hasil penelitian maka dapat disimpulkan bahwa penggunaan obat antipsikotik pada pasien skizofrenia di Puskesmas Kecamatan Kramat Jati Periode Agustus 2022 dinilai belum memenuhi ketepatan penggunaan antipsikotik. Hasil  evaluasi menunjukkan ketepatan indikasi sebanyak 25 pasien [100%], tepat pemilihan obat sebanyak 18 pasien [72%] , tepat pasien sebanyak sebanyak 25 pasien [100%], tepat dosis sebanyak 24 pasien [96 %], serta tepat frekuensi sebanyak 24 pasien [96%].

Pharmacy intenrship is one of the efforts to provide skills provision for pharmacist students so that they have pharmaceutical skills so that later pharmacist candidates are expected to be able to practice pharmacy in a professional, legal and ethical manner at the district health center. The duration internship is for two weeks with a special assignment, namely  Evaluation of Rational Antipsychotics Schizophrenic Patients at the Kramat Jati District Health Center. The purpose of this special assignment was to evaluate the rationality of prescribing antipsychotics in schizophrenic patients at the Kramat Jati District Health Center in 2022. The method used in this special assignment was to use a descriptive observational design with a retrospective sampling technique in schizophrenia patients The research subjects were prescription data of schizophrenic patients at the Kramat Jati District Health Center in October 2022. The research instrument used was data excel obtained through the ePuskesmas website. Based on the results of the study, it can be concluded that the use of antipsychotic drugs in schizophrenic patients at the Kramat Jati District Health Center for the August 2022 period is considered not to meet the accuracy of antipsychotic use. The results of the evaluation showed the accuracy of the indications for 25 patients [100%], the correct drug selection for 18 patients [72%] , the correct dosage for 25 patients [100%], the correct dosage for 24 patients (96 %), and the correct frequency for 24 patients. patients [96%]."

"Home pharmacy care adalah salah satu aspek pelayanan farmasi yang ada di apotek dengan pelaksanaannya dilakukan di rumah khususnya pada pasien lanjut usia dan memiliki riwayat penyakit kronis (diabetes, penyakit kardiovaskular, TBC, asma). Tujuan dilakukannya home pharmacy care adalah meningkatkan pemahaman mengenai pengobatan sehingga penggunaan obat sesuai, serta memastikan kepatuhan penggunaan obat pasien sehingga keberhasilan terapi akan meningkat. Akibat dari peningkatan kasus virus corona yang terjadi di Indonesia, pelaksanaan home pharmacy care di Apotek Kimia Farma 352 tidak dilakukan dengan berkunjung ke rumah pasien untuk meminimalkan resiko penularan. "Pelayanan kefarmasian di Puskesmas merupakan salah satu hal yang dapat menunjang pelayanan kesehatan yang bermutu karena sifatnya yang langsung bertanggung jawab kepada pasien. Salah satu pelayanan kefarmasian yang penting di puskesmas yaitu pelayanan resep. Peresepan yang baik dapat meningkatkan penggunaan obat secara rasional sehingga pasien menerima obat sesuai dengan indikasi klinis, dalam dosis yang tepat, untuk jangka waktu yang cukup, serta dengan biaya yang rendah. Untuk itu, diperlukan suatu daftar (formularium) dari obat yang harus tersedia dalam fasilitas kesehatan tingkat pertama. Formularium Nasional berfungsi sebagai acuan atau pedoman bagi penyedia layanan kesehatan yang bertujuan untuk menyediakan obat-obatan yang aman, berkhasiat, bermutu, dan terjangkau dalam jenis dan jumlah tertentu. Namun, pengelolaan obat yang tidak efisien dapat memberikan dampak negatif, baik secara medis maupun ekonomi. Oleh karena itu, perlu adanya seleksi obat yang tepat melalui sistem formularium puskesmas untuk meningkatkan mutu terapi obat dan menurunkan kejadian efek samping obat. Analisis formularium nasional dilakukan dengan cara mendata obat-obat untuk dimasukkan ke formularium puskesmas lalu membandingkannya dengan Rencana Kebutuhan Obat (RKO) tahun 2022 dan tahun 2023. Daftar obat yang disusun sebagai formularium puskesmas didasarkan pada formularium nasional tahun 2021. Obat-obatan yang telah diseleksi dari formularium nasional lalu dibandingkan dengan RKO tahun 2022 dan tahun 2023. Dari hasil perbandingan, terdapat obat-obatan yang termasuk ke dalam RKO 2022 namun tidak termasuk dalam RKO 2023. Selain pelayanan resep, apoteker sebagai tenaga kesehatan di puskesmas memiliki tugas untuk meningkatkan kesejahteraan masyarakat, salah satunya dengan memberikan edukasi kesehatan kepada pengunjung puskesmas. Pemberian edukasi menggunakan alat bantu berupa leaflet dapat memudahkan peserta edukasi untuk memahami materi yang disampaikan. Pembuatan leaflet dilakukan dengan metode studi literatur, serta pelaksanaan edukasi kesehatan dilakukan dengan penyuluhan singkat serta penyebaran leaflet pada peserta penyuluhan. Pelaksanaan penyuluhan singkat menggunakan leaflet sebagai alat bantu sangat memudahkan materi sampai kepada peserta penyuluhan dan dipahami dengan baik.

Pharmaceutical services at Community Health Centers are one of the things that can support quality health services because they are directly responsible to patients. One of the important pharmaceutical services at community health centers is prescription services. Good drug prescribing can increase the rational use of drugs so that patients receive drugs according to clinical indications, in the right dose, for a sufficient period of time, and at a low cost. For this reason, a list (formulary) of drugs that must be available in first-level health facilities is needed. The National Formulary functions as a reference or guideline for health service providers whose aim is to provide safe, efficacious, quality and affordable medicines in certain types and quantities. However, inefficient drug management can have negative impacts, both medically and economically. Therefore, there is a need for appropriate drug selection through the health center formulary system to improve the quality of drug therapy and reduce the incidence of drug side effects. National formulary analysis is carried out by listing the drugs to be included in the health center formulary and then comparing it with the 2022 and 2023 Drug Needs Plans (RKO). The list of drugs compiled as a health center formulary is based on the 2021 national formulary. from the national formulary and then compared with the 2022 and 2023 RKO. From the comparison results, there are medicines that are included in the 2022 RKO but are not included in the 2023 RKO. Apart from prescription services, pharmacists as health workers at community health centers have the task of improving community welfare, one of which is by providing health education to health center visitors. Providing education using tools in the form of leaflets can make it easier for education participants to understand the material presented. Making leaflets was carried out using the literature study method, and the implementation of health education was carried out by providing short counseling and distributing leaflets to counseling participants. Carrying out short counseling using leaflets as a tool really makes it easier for the material to reach the counseling participants and be understood well.

"Penggunaan obat rasional (POR) apabila memenuhi kriteria tepat indikasi, tepat diagnosis, tepat pemilihan obat, tepat dosis, tepat cara pemberian obat, tepat interval waktu pemberian obat, tepat lama pemberian obat, dan waspada efek samping. Penerapan POR akan menjamin bahwa pasien mendapatkan pengobatan yang bermutu. Selain itu, analisis POR juga dapat menekan angka resistensi antibiotik dan penggunaan injeksi yang tidak diperlukan. Untuk itu, dilakukan penelitian untuk mengevaluasi pelaksanaan POR di puskesmas Kelurahan Kramat Jati dengan perbandingan target indikator kinerja POR tahun 2019 yang ditetapkan oleh Kementerian Kesehatan RI. Pengumpulan data dilakukan menggunakan metode observasional retrospektif, yaitu dengan mengumpulkan data POR bulan Juli-September tahun 2022. Hasil evaluasi menunjukkan capaian kinerja POR di Puskesmas Kelurahan Kramat Jati pada bulan Juli sebesar 96,43%, bulan Agustus sebesar 93,40%, dan pada bulan September 90,54%. Penelitian ini menyimpulkan bahwa penggunaan obat di puskesmas Kelurahan Kramat Jati pada Juli-September 2022 dikatakan rasional.

Rationality drugs use (RDU) meets the criteria of correct indication, correct diagnosis, correct selection of drug, correct dose, correct method of drug administration, correct time interval for drug administration, correct duration of drug administration, and alertness to side effects. Implementing RDU will ensure that patients receive quality treatment. Apart from that, RDU analysis can also reduce the rate of antibiotic resistance. For this reason, research was conducted to evaluate the implementation of RDU at the Kramat Jati Subdistrict Health Center by comparing the 2019 RDU performance indicator targets set by the Indonesian Ministry of Health. Data collection was carried out using a retrospective observational method, namely by collecting RDU data for July-September 2022. The evaluation results showed that the POR performance achievement at the Kramat Jati Subdistrict Health Center in July was 96.43%, in August it was 93.40%, and in in September 90.54%. This research concludes that the use of drugs at the Kramat Jati Subdistrict Health Center in July-September 2022 is said to be rational."

Tujuan dari tugas khusus ini adalah untuk mengetahui jumlah persentase penggunaan dan kesesuaian obat pasien hipertensi pada pasien hipertensi dengan penyakit penyerta diabetes mellitus tipe 2. Hasil yang didapatkan dari tugas khusus ini adalah masih besarnya ketidaksesuaian penggunaan obat antihipertensi pada pasien hipertensi dengan penyakit penyerta diabetes mellitus tipe 2.

"Resep adalah permintaan tertulis dari dokter, dokter gigi atau dokter hewan kepada apoteker baik dalam bentuk maupun elektronik untuk menyediakan dan menyerahkan sediaan farmasi dan/atau alat kesehatan bagi pasien. Resep dikatakan rasional apabila terdapat informasi terkait obat yang akan diberikan kepada pasien. Penyusunan tugas khusus ini bertujuan untuk mengatasi permasalahan yang mungkin terjadi pada telaah peresepan. Penyusunan tugas khusus ini menggunakan metode studi literatur, dengan menggunakan resep yang diperoleh tahun 2022 di Apotek Atrika. Penulis menganalisis permasalah kelengkapan resep meliputi kelengkapan aspek administratif, aspek klinis, dan aspek farmasetik. Resep dikaji dengan menggunakan referensi Farmakope Indonesia edisi III, Info Spesialite Obat, PIONAS, dan MIMS. Sehingga dapat disimpulkan bahwa masih terdapat kekurangan pada kajian administratif seperti jenis kelamin pasien, berat badan pasien, alamat pasien, nomor telepon pasien, dan tanda tangan atau paraf dokter.

A prescription is said to be rational if there is information relating to the drug to be given to the patient. The preparation of this special assignment aims to overcome problems that may occur in the recipe review. The preparation of this special assignment uses the literature study method, using recipes obtained in 2022 at the Atrika Pharmacy. The author analyzes the problem of prescription completeness including the completeness of administrative aspects, clinical aspects, and pharmaceutical aspects. Recipes were reviewed using references to the Indonesian Pharmacopoeia, Edition III, Drug Specialist Info, PIONAS, and MIMS. So it can be concluded that there are still deficiencies in administrative studies such as patient gender, patient weight, patient address, patient telephone number, and doctor's signature or initials."

"Salah satu faktor penunjang kelancaran pelayanan kesehatan di puskesmas adalah tersedianya obat yang cukup untuk masyarakat yang melakukan pengobatan di puskesmas. Ketersediaan obat yang optimal dapat direncanakan dari proses perencanaan termasuk proses pencatatan dan pelaporan. Proses perencanaan kebutuhan obat tiap tahunnya dilakukan dengan sistem berjenjang (bottom-up) dari puskesmas yang berada di kelurahan hingga ke kecamatan. Tiap puskesmas akan membuat dan melaporkan Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO). Formulir ini berfungsi untuk memonitoring pemakaian dan permintaan obat setiap bulan dan dapat diketahui profil penggunaan obat yang paling banyak hingga paling sedikit digunakan di puskesmas sehingga dapat disesuaikan dengan pola penyakit yang tersebar di masyarakat wilayah kerja puskesmas tersebut. Penelitian ini bertujuan untuk mengetahui kesesuaian kartu stok obat dengan LPLPO dan mengetahui profil 10 (sepuluh) obat terbanyak di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 sehingga dapat membantu dalam proses perencanaan dan permintaan untuk periode selanjutnya. Pengambilan data menggunakan metode retrospektif dengan melihat LPLPO Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022. Hasil dari penelitian ini berupa 10 (sepuluh) profil penggunaan obat terbanyak dari 228 obat di Puskesmas Kecamatan Kramat Jati Periode Oktober-Desember 2022 dengan urutan sebagai berikut: Metformin 500 mg; Parasetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipin 5 mg; Deksametason 0,5 mg; Vitamin B Kompleks; Glimepirid 2 mg; dan Vitamin B6 10 mg.

One of the factors supporting the smooth operation of healthcare services at health centers is the availability of sufficient medications for the community seeking treatment. Optimal medication availability can be planned through processes including recording and reporting. The annual medication planning process is conducted in a hierarchical (bottom-up) system from health centers in villages to those in districts. Each health center will prepare and report the Medication Usage Report and Request Form (LPLPO). This form functions to monitor monthly medication usage and requests, allowing the identification of the most to least used medications at the health center, which can then be adjusted according to the disease patterns prevalent in the community served by the health center. This study aims to determine the conformity of medication stock cards with LPLPO and to identify the profile of the ten most used medications at the Kramat Jati District Health Center for the period of October-December 2022, thereby assisting in the planning and requesting process for the next period. Data collection was conducted retrospectively by reviewing the LPLPO of the Kramat Jati District Health Center for the period of October-December 2022. The results of this study include the profile of the ten most used medications out of 228 at the Kramat Jati District Health Center for the period of October-December 2022, in the following order: Metformin 500 mg; Paracetamol 500 mg; Calcium lactate 500 mg; Guaifenesin 100 mg; Chlorpheniramine maleate (CTM) 4 mg; Amlodipine 5 mg; Dexamethasone 0.5 mg; Vitamin B Complex; Glimepiride 2 mg; and Vitamin B6 10 mg."
