"ABSTRACT
Although iron supplementation exists for pregnant women, the prevalence of anemia during pregnancy remains high. The lack of compliance of the target group is one of the reasons which may reduce the efficacy of the supplementation program.
Recent studies in preschool children and non pregnant women has been reported that the administration of intermittent iron supplement was equally effective in improving the iron status as daily supplement.
This research was to investigate whether a weekly dose of 120 mg iron supplementation would improve the iron status in the same way as a daily dose of 60 mg iron supplementation in pregnant women.
The effect of daily vs weekly iron supplementation was studied in pregnant women in non randomized experimental community trial. The subjects were pregnant women who attended the selected Health Centers for the first time in their current pregnancy. Of the 176 pregnant women enrolled, a complete data set were available for 139 (79 %). Duration of supplementation was 8 - 20 weeks.
Three health centers each, matched with socioeconomic condition were allocated for control group and treatment group. Daily group served as control received 60 mg Fe + 0.25 mg folic acid daily (68 pregnant women), while weekly group received 120 mg Fe + 0.50 mg folic acid weekly (71 pregnant women).
Hemoglobin concentration in both group increased significantly after supplementation (p < 0.001). Improvement of hemoglobin was influenced by initial hemoglobin level (p < 0.001), and hookworm infestation (p < 0.05).
Serum ferritin level decreased in daily and weekly group (p > 0.05). Serum ferritin change was not influenced by initial hemoglobin level (p ] 0.05), however was influenced by hookworm infestation (p < 0.001).
The duration of supplementation had no effect on hemoglobin changes (p > 0.05), but it influenced serum ferritin changes (p < 0.05).
It was concluded that supplementation with 120 mg elemental iron on weekly basis had similar effects as daily dose of 60 mg on hematological status, but was not enough to improve iron stores in pregnant women.

"Penelitian-penelitian terdahulu telah mengajukan beberapa faktor yang dapat menyebabkan mengapa suplementasi besi pada wanita hamil masih belum memuaskan hasilnya. Faktor-faktor tersebut adalah distribusi yang tidak memadai, hasil jangkauan dan ketaatan berobat yang rendah, dan juga penyerapan usus yang rendah. Penelitian ini bertujuan untuk mengukur status besi wanita hamil setelah konsumsi biskuit yang mengandung bubuk ikan dan preparat besi-folat. Penelitian ini dilakukan di Purworejo, Jawa Tengah, dari bulan Pebruari sampai Oktober 2002. Tujuh-puluh wanita hamil dalam trimester kedua-ketiga dengan umur kehamilan antara 2-3 bulan ikut dalam penelitian yang menggunakan randomized controlled trial (RCT). Sepuluh wanita mengundurkan diri dan sisanya dibagi dalam dua kelompok yang terdiri dari 28 wanita yang diberikan biskuit yang diperkaya dengan protein ? zat besi (kelompok PIEC) dan 32 wanita diberikan biskuit yang diperkaya dengan zat besi (kelompok IEC). Pemberian biskuit dilakukan selama 12 minggu. Dari hasil penelitian didapatkan peningkatan kadar hemoglobin (Hb) dan reseptor transferin serum (sTfR) pada kedua kelompok. Selain itu terdapat penurunan kadar serum feritin (SF) di kedua kelompok. Namun demikian, pada akhir penelitian, peningkatan kadar Hb dan sTfR kedua kelompok ini berbeda bermakna, sedangkan penurunan SF tidak berbeda bermakna. Protein hewani berupa bubuk ikan cendrung meningkatkan absorpsi zat besi non-heme sehingga dapat meningkatkan kadar Hb dan sTfR pada wanita hamil. (Med J Indones 2003; 12: 243-6)

Previous studies have revealed that several factors influenced the relatively low success of iron supplementation for pregnant women. The factors included poor distribution, low coverage and compliance, as well as low absorption. The aim of this study is to measure the iron status of pregnant women after consuming crackers containing fish powder and iron-folate. This study was carried out in the Purworejo district (Central Java) from February through October 2002. Using a randomized controlled trial (RCT) design, 70 pregnant women in their second-third month of pregnancy were recruited, and divided into two groups. Ten women dropped out during the study. The first group consisted of 28 women were given protein ? iron enriched crackers (PIEC group), while the second group of 32 women were given iron?enriched crackers (IEC group) for a total of 12 weeks. The results showed that the hemoglobin (Hb) levels and serum transferrin receptors (sTfR) of both groups were increased. Serum ferritins (SF) of both groups were decreased. At the end of the study, the increase in Hb and sTfR levels between the two groups were significantly different, while the decrease in SF was not significantly different. Animal protein from fish powder tended to improve absorption of non-heme iron among pregnant women, resulting in improved Hb and sTfR levels. (Med J Indones 2003; 12: 243-6)"

"Indonesia merupakan salah satu negara dengan prevalensi anemia ibu hamil yang tergolong tinggi dibandingkan dengan negara-negara berkembang lain. Padahal anemia pada ibu hamil menyumbang 40% penyebab kematian ibu yang tinggi di Indonesia, selain itu juga berdampak pada pertumbuhan janin yang terhambat, meningkatkan risiko BBLR dan bayi lahir prematur, serta mengakari stunting dan anemia dini. Salah satu cara yang direkomendasikan oleh WHO untuk mencegah anemia pada ibu hamil adalah melakukan suplementasi Tablet Tambah Darah (TTD) oral setiap hari sebagai bagian dari perawatan antenatal pada ibu hamil. Kemenkes (2015) menganjurkan untuk ibu hamil mengonsumsi TTD minimal 90 tablet selama masa kehamilan sebagai bentuk pencegahan anemia. Berdasarkan survei SDKI 2017, provinsi dengan tingkat konsumsi TTD terendah adalah Provinsi Sulawesi Tenggara (14%) disusul dengan Provinsi Sumatera Utara (15%). Pada hasil riset SDKI 2012 sebelumnya Provinsi Sumatera Utara juga berada di posisi 4 terendah konsumsi TTD ibu hamil sesuai anjuran, dengan persentase 8,7%. Dengan demikian tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang berhubungan dengan kepatuhan ibu hamil dalam mengonsumsi TTD. Penelitian ini menggunakan desain cross sectional dan data yang diambil adalah data sekunder SDKI 2017 di Provinsi Sumatera Utara dengan sampel sebanyak 464 orang. Variabel dalam penelitian adalah: umur, pekerjaan, pendidikan, wilayah tempat tinggal, paritas, jarak antar kelahiran, frekuensi ANC, jenis tenaga dan tempat pemeriksaan kehamilan, dan ketersediaan TTD di rumah. Hasil analisis univariat adalah ibu hamil di Sumatera Utara menunjukkan bahwa lebih banyak persentase ibu yang masuk dalam kelompok umur berisiko, memiliki pendidikan rendah dan menengah, dengan status bekerja, tinggal di perkotaan, memiliki riwayat paritas multipara, memiliki jarak antar kelahiran ≥24 bulan, frekuensi ANC ix Universitas Indonesia sesuai, nakes sebagai tenaga ANC, faskes sebagai tempat ANC dan memiliki ketersediaan TTD di rumah. Hasil dari penelitian bivariat menunjukkan tiga variabel terbukti secara statistik memiliki hubungan yang bermakna dengan kepatuhan ibu mengonsumsi TTD yakni pendidikan (p=0,002, OR=2,153), paritas (p=0,020, OR=3,544) dan frekuensi ANC (p=0,001, OR=2,419). Sedangkan variabel yang tidak menunjukkan hubungan yang bermakna antara lain umur (p=0,444), pekerjaan (p=0,236), wilayah tempat tinggal (p=0,523), jarak antar kelahiran (p=0,968), tenaga ANC (p=0,517), tempat ANC (p=1,000) dan ketersediaan TTD di rumah (tidak dapat dianalisis). Saran bagi instansi terkait dapat melakukan kolaborasi lintas sektor untuk mengatasi rendahnya tingkat pendidikan wanita di Sumatera Utara, menggencarkan promosi dan informasi TTD di sosial media, menggencarkan penyuluhan dan edukasi mengenai KB, membuat program layanan tambahan mengenai TTD untuk ibu hamil dengan pendidikan rendah. Selain itu bagi peneliti lain dapat melakukan penelitian yang lebih baik dari segi pengujian data, metode dan sumber data.

Compared to other developing countries, Indonesia is one of the high prevalence countries with anemia among pregnant women. Whereas anemia in pregnant women causes 40% of high maternal mortality in Indonesia, it also impact on retarding fetal growth, increasing the risk of low birth weight and premature birth, as well as rooting stunting and early anemia. WHO recommends to prevent anemia in pregnant women by iron supplementation every day as part of antenatal care. Kemenkes RI (2015) recommends pregnant women to consume a minimum 90 tablets of iron supplement during pregnancy as a form of anemia prevention. Based on the IDHS 2017 survey, the province with the lowest level of iron consumption was Southeast Sulawesi (14%) followed by North Sumatra (15%). In the previous 2012 IDHS research results, North Sumatra was also in the 4th lowest position in the consumption of iron for pregnant women with 8.7%. Thus, the purpose of this study was to determine what factors influence the adherence to Iron Supplement consumption. This can be seen from several factors such as age, occupation, education, area of residence, parity, distance between births, frequency of ANC, type ANC workers, ANC place, and availability of iron tablets at home. This study uses a cross sectional design and using secondary data from the IDHS 2017 in North Sumatra with a sample of 464 people. The results of univariate analysis were pregnant women in North Sumatra showed that there was a higher percentage of mothers who belonged to the risk age group, had low and secondary education, working, lived in urban areas, multiparity parity, had a birth interval of 24 months, the frequency of ANC appropriate, health workers as ANC personnel, health facilities as ANC places and have the availability of iron tablets at home. The results of bivariat analysis showed that three variables were statistically proven to have a significant relationship with adherence of taking iron tablets among pregnant women, such as education (p=0.002, OR=2.153), parity (p=0.020, OR=3.544) and frequency of ANC (p=0.001, OR =2.419). xi Universitas Indonesia While the variables that did not show a significant relationship were age (p=0.444), occupation (p=0.236), area of residence (p=0.523), distance between births (p=0.968), ANC personnel (0.517), ANC location (p=1,000) and availability of iron at home (can not be analyzed). From this study there is some suggestions for the government can collaborate across sectors to overcome the low level of women's education in North Sumatra, intensify promotion and information on iron tablets on social media, intensify counseling and education about family planning, create additional service programs regarding iron tablets for pregnant women with low education. In addition, other researchers can do better research in terms of testing data, methods and data sources."

Masa-masa kehamilan merupakan masa-masa penting bagi ibu dan bayi yang dikandungnya. Pada masa kehamilan, kebutuhan nutrisi ibu meningkat, termasuk kebutuhan zat besi. Apabila tidak terpenuhi, defisiensi zat besi meningkatkan risiko kelahiran preterm dan bayi dengan berat badan lahir rendah (BBLR). Penelitian ini bertujuan untuk mengetahui korelasi antara asupan zat besi dengan indikator zat besi pada ibu hamil trimester pertama. Metode penelitian yang digunakan adalah cross-sectional dengan mengukur data sekunder dalam satu waktu. Asupan zat besi dihitung dengan metode food frequency questionnaire. Indikator zat besi yang diperiksa meliputi kadar hemoglobin, kadar ferritin, jumlah eritrosit dan nilai MCV. Data penelitian dianalisis dengan uji normalitas data Kolmogorov-Smirnov dan uji korelasi Spearman dengan SPSS versi 20.00. Hasil penelitian ini menunjukkan bahwa 35,5% responden obese dan 19,4% memiliki berat badan lebih. Responden dengan asupan zat besi rendah sebesar 82,3% . Hasil penelitian ini untuk indikator zat besi menunjukkan bahwa 11,3% responden menderita anemia (Hb< 11 mg/dL), 27,4% responden memiliki jumlah eritrosit rendah (<4,2 juta/dL), 14,5% memiliki nilai MCV mikrositik (<80 fl) dan 6,5% responden memiliki kadar ferritin yang rendah (<15 μg/L). Uji korelasi antara asupan zat besi dengan kadar hemoglobin, kadar ferritin, jumlah eritrosit dan nilai MCV pada ibu hamil trimester pertama tidak menunjukkan korelasi yang signifikan (p>0,05).

ABSTRACT
Pregnancy is one of the most important moment for the mother and her child. In pregnancy, the mother nutritional need increase, including iron. Had the nutritional need is not fulfilled, it will increase the risk of preterm birth and low weight born baby. This study analyzed correlation between iron uptake and iron status in first trimester pregnant woman. This study used cross-sectional design to assess secondary data in one time. Iron intake was measured with food frequency questionnaire. Iron indicator used in this study were haemoglobin concentration, ferritin concentration, amount of erythrocyte and MCV value. This data was analyzed with test of normality Kolmogorov-Smirnov and Spearman correlation test with SPSS for Windows version 20.00. The result shows that 35.5% of respondent are obese and 19.4% are overweight. Also 82.3% of the respondents have inadequate iron uptake. For iron indicator 11.3% of respondent are anemic (Hb< 11 mg/dL), 6.5% have low ferritin concentration (<15 μg/L), 27.4% have low amount of erythrocyte (<4.2 million/dL) and 14.5% have microcytic value (<80 fl). No correlation is found between iron uptake and haemoglobin concentration, ferritin concentration, amount of erythrocyte and MCV value."

"Female adolescents are vulnerable to iron deficiency anemia (IDA) and if it persists into their reproductive years has serious implications not only for the health of their offsprings but for their own as well. An iron supplementation is warranted in order to provide sufficient iron stores prior to pregnancy and to effect desirable birth outcomes. A communication component of the supplementation is regarded as essential for improving compliance.
Therefore, an experimental community trial involving adolescent female students in two public high schools in Metro Manila was conducted between October-December 1997. The objective of the study was to asses the effect of communication on compliance to weekly iron supplementation. The students were assigned to three groups: iron plus communication (FeC group, n = 82), iron (Fe group, n = 89), and control group (n=78). The FeC and Fe groups received iron tablets containing 60 mg. Elemental iron and 250 mcg. Folic acid while the control group received placebo tablets from Physical Education Health and Music (PEHM) teachers once a week of eight subsequent weeks. Teachers assigned to the FeC group were trained communication. Comparison were made between the three groups on compliance as communication. Comparisons were made between the three groups on compliace as measured by attendance to tablet distribution and actual ingestion through stool test, record on index card and interview. Additionally, levels and prevalence of anemia were measured before and after intervention and a pre and post test about knowledge on IDA and possible causes and treatments were included. Data about side-effects, reaction from students, parents and teachers about the supplementation-communication activities and suggestions for futher improvements in conductiong such future undertaking were obtained from focus group discussions (FGDs).
Comparisons of actual tablet ingestion between the three groups showed significantly higher compliance in the FeC group (P<0.001) than in the other two group as measured by record on index card and interview desoite more side effects felt. Changes from pre to post-test knowledge scores o iron were significantly greater for the FeC group (p<0.001) than in the Fe and control groups. However, hemoglobin levels improved significantly in the Fe group (p<0.05) but not in the FeC group and remained the same in the control group. Reactions obtained through focus group discussions from parent (as reported by students), teachers and the participants found the program beneficial and feasible for implementation on a larger scale. Compliance enhancing strategies and motivational approaches for adolescent female students must consider the potential influence of family, fiends and teachers as revealed by the Venn diagram in planning iron supplementation programs with communication for this target group."

"The effect of weekly iron supplementation and deworming on the hemoglobin level was studied among 289 children aged 2 to 5 years in a randomized double-masked placebo controlled community trial. Subjects were allocated into 3 groups which respectively received iron supplements and deworming, iron supplements only and placebos.
Iron supplementation for 8 weeks using 30 mg elemental iron as ferrous sulphate syrup once per week, led to a significant reduction in the prevalence of anemia from 37.2%. to 16.2%. Using unsupervised distribution by mothers, hemoglobin concentration increased significantly in both groups which received iron (p<0.001) and also in the placebo group (p<0.05), but the changes in both treatment groups were significantly higher than the placebo group (p<0.001).
No significant difference in hemoglobin changes was found between those who received additional deworming and those who received iron supplement only. Positive iron in stool were confirmed in 68.2% of the children who were reported received iron supplements (n=66). It is concluded that weekly iron supplementation is effective to reduce the prevalence of anemia among preschoolers."

"Berat plasenta umum digunakan untuk mengukur pertumbuhan dan fungsi yang baik dari plasenta. Anemia merupakan salah satu faktor risiko dari berat plasenta yang tinggi. Namun sampai saat ini belum dapat dipastikan apakah anemia karena kekurangan zat besi mempengaruhi berat plasenta. Penelitian ini meneliti hubungan antara anemia defisiensi besi dan berat plasenta pada wanita hamil anemia di Jakarta Timur. Penelitian ini menggunakan desain penelitian potong lintang yang melibatkan 90 ibu hamil anemia di 10 Puskesmas Kecamatan, Jakarta Timur. Prevalensi defisiensi besi pada ibu hamil anemia adalah 36,9%. Berat plasenta rata-rata adalah 549,3 ± 115 gr. Ada hubungan positif antara anemia defisiensi besi dan berat plasenta setelah dikontrol oleh variabel paritas, perokok pasif dan frekuensi kunjungan pemeriksaan kehamilan (B = 0,22; p = 0,038). Berat plasenta yang tinggi pada ibu hamil dengan anemia defisiensi besi menunjukkan adanya mekanisme adaptasi dari plasenta dikarenakan kurangnya oksigen dalam darah.

Placental weight is a commonly used to measure placental growth and function including nutrient transfer to the fetus. Anemia may link to a risk factor for higher placental weight, however, it is uncertain whether iron deficiency anemia influence a placental weight, and could be used a public health measure for fetal growth and healthy pregnancy. This study investigated the relationship between iron deficiency anemia and placental weight among anemic pregnant women in East Jakarta. We conducted a cross sectional study of 90 anemic pregnant women and their singleton pregnancies in 10 Primary Health Center in East Jakarta. The prevalence of iron deficiency among anemic pregnant women was 36.9%. The mean placental weight was 549.3 ± 115 gr. There was a positive relationship between iron deficiency anemia and placental weight after adjusting for parity, passive smoker and ANC visit frequency (B=0.22; p=0.038). A higher placenta weight was observed among iron deficiency anemic pregnant women, suggesting the adaptive mechanism of placenta to chronic poor oxygenation."

"An intervention program on the impact of iron preparation with intensive nutrition education among pregnant women was undertaken in Kecamatan Curug, Kabupaten Tanggerang.
Twwo kinds of iron tablets, both containing 200 mg ferrous sulphate and 0,25 mg folic acid were distributed in four villages, namely village A: Kadu Jaya, village B: Kadu, village C: Curug Wetan and village D: Cukang Galih. The new iron tablet was prepared in a coated membrane form, with red coloring and was distributed in sachets containing 14 or 28 tablets. The existing (currently used) tablet was prepared in coated film form, with grey coloring and was loosely packaged. Intensive nutrition educaation was delivered to twwo villages, which were given the different iron tablet forms as described above.
A number of 162 pregnant women as subjects in four villages were divided into four different intervention groups, 41 of them in village A received new iron with intensive nutition education, 41 in village B received new iron, 40 in village C received existing iron with intensive nutrition education and 40 in village D received exixting iron alone was as the control group, for 12 weeks of intervention period, however only 127 of them could be monitored and evaluated.
Analysisi of nutrition knowledge, attitude and behavior of the subjects, recording of iron tablet consumption, dietary habits, nutrient intake and nutritional status was conducted in the four villages, for each of the four groups before and after intervention. In addition serum ferritin and hemoglobin concentration were determined to evaluate the change in the prevalence of anemia.
Distribution of new iron or existing iron tablets without intensive nutrition knowledge, attitude, behavior of the subjects towards anemia problem. he results show that subjects which received new iron could improve their dietary habits, calories and protein intakes, but not in iron and vitamin C intakes. Although, a significantly higher (p<0.0500) consumption of iron was observed in the group which received new iron, neither in the subsequent improvement in hemoglobin and serum ferritin values.
Intensive nutrition education was positively associated with the improvement of nutritional knowledge, attitude and behavior of the pregnant women as well as significant increase in iron tablet consumption (p<0.0500). Dietary habits were improved, as seen in the increase in calorie and protein intake and the resulting improvement in the nutritional status of the mothers.
The results indicated that the new iron supplementation with intensive nutrition education had a greater effect on invreasing the iron tablet consumption of pregnant women compared to those who only received existing iron supplementation without intensive nutrition education. Since the iron intake was not different in the two villages, their effect on hrmoglobin and serum ferritin concetration was not different.
Witihin the time period of observation, there was no different reduction in the prevalence of anemia between each village of the population studied."

"Angka kejadian anemia pada ibu hamil di puskesmas kecamatan Kemayoran masih tinggi walaupun cakupan ibu hamil yang mendapatkan tablet tambah darah minimal 90 butir selama hamil sudah mencapai 100%. Besarnya suplementasi zat besi juga harus disesuaikan dengan kebutuhan dan kondisi masing-masing ibu. Tidak efektifnya program pemberian TTD untuk menurunkan kejadian anemia kehamilan karena belum adanya media yang efektif untuk memberikan informasi dan edukasi tentang anemia dan TTD.
Penelitian ini bertujuan mengetahui efektifitas intervensi pendidikan kesehatan melalui diagram bantu konseling anemia dan pemberian dosis terapi TTD terhadap peningkatan kadar hemoglobin ibu hamil di wilayah puskesmas kecamatan Kemayoran tahun 2019. Penelitian ini merupakan jenis penelitian quasi eksperiment dengan menggunakan desain nonequivalent control group design. Penelitian dilakukan di wilayah puskesmas kecamatan Kemayoran dari rentang waktu Maret-November 2019. Populasi dalam penelitian ini adalah semua ibu hamil di wilayah puskesmas kecamatan kemayoran dan teknik pemilihan sampel dengan cara purposive sampling.
Hasil penelitian pengaruh intervensi pendidikan kesehatan melalui diagram bantu konseling anemia dan pemberian dosis terapi TTD terhadap peningkatan kadar hemoglobin ibu hamil di wilayah puskesmas kecamatan Kemayoran tahun 2019 pada analisis bivariat menggunakan uji Anova didapatkan nilai p = 0.0005, hubungan pengetahuan dan kepatuhan mengkosumsi TTD dengan Kadar Hemoglobin didapatkan r=0.288, p value=0.035; r=0.422, p value=0.001. Kesimpulan ada pengaruh intervensi pendidikan kesehatan melalui diagram bantu konseling anemia dan pemberian dosis terapi TTD terhadap peningkatan kadar hemoglobin ibu hamil di wilayah puskesmas kecamatan Kemayoran tahun 2019 (nilai p < 0.05). Semakin tinggi pengetahuan dan kepatuhan mengkonsumsi TTD maka semakin besar kadar hemoglobin.

The incidence of anemia in pregnant women in the Kemayoran Health Centre is still high considering that pregnant women who get iron tablets of at least 90 during pregnant are reaching 100%. The amount of iron supplementation must also be adjusted to the needs and conditions of each mother. Ineffective iron supplementation delivery program for pregnant women to reduce the incidence of anaemia because there is no effective media to provide information and education about anaemia and iron supplementation.
This study aims to know the effectivity of health educational intervention through anaemia counseling helping diagram and giving iron suplement therapeutic doses to increasing haemoglobin levels for pregnant women at Kemayoran public health center area in 2019. This study uses a quasi experimental research type using the nonequivalent control group design. The study was conducted in the area of Kemayoran health center from the period March-November 2019. The population in this study were all pregnant women in the area of Kemayoran health center and sample selection techniques by purposive sampling.
The results of research on health educational intervention through anaemia counseling helping diagram and giving iron suplement therapeutic doses to increasing haemoglobin levels for pregnant women at Kemayoran public health center in 2019 on bivariate analysis using Anova test obtained p value = 0.0005, the relationship of knowledge and compliance consuming iron suplement with haemoglobin levels obtained r = 0.288, p value = 0.035; r = 0.422, p value = 0.001. Conclusion: there is influence of health educational through anaemia counseling helping diagram and giving iron suplement therapeutic doses to increasing haemoglobin levels for pregnant women at Kemayoran public health center in 2019 (p value <0.05). The higher of knowledge and compliance of consuming iron suplement, the greater the hemoglobin level.
