"Nikotin adalah senyawa utama di dalam tembakau yang menimbulkan ketergantungan pada rokok. Nikotin diinaktivasi oleh tubuh menjadi kotinin melalui kerja enzim CYP2A6. Polimorfisme genetik CYP2A6 memegang peranan penting pada kebiasaan merokok dan ketergantungan pada nikotin. Telah diketahui terdapat alel gen CYP2A6*1A (wild type) yang berhubungan dengan metabolisme nikotin normal/cepat, dan beberapa variasi genetik seperti alel CYP2A6*4, CYP2A6*7, CYP2A6*9, CYP2A6*10, yang berkaitan dengan penurunan aktivitas metabolisme nikotin. Variasi aktivitas metabolisme nikotin ini berpengaruh pada kadar nikotin plasma. Perokok memerlukan kadar nikotin tertentu pada otaknya, sehingga pada individu dengan metabolisme nikotin yang cepat diperlukan jumlah rokok yang lebih banyak. Sebaliknya, pada individu dengan metabolisme nikotin yang lambat, nikotin plasma lambat diinaktivasi dan akan mengakibatkan gejala toksik, sehingga ketergantungan pada rokok menjadi lebih rendah.

Abstract
Nicotine is a major addictive compound in tobacco cigarette smoke. After being absorbed by the lung nicotine is rapidly metabolized and mainly inactivated to cotinine by hepatic cytochrome P450 2A6 (CYP2A6) enzyme. Genetic polymorphisms in CYP2A6 may play a role in smoking behavior and nicotine dependence. CYP2A6*1A is the wild type of the CYP2A6 gene which is associated with normal or extensive nicotine metabolism. In the CYP2A6 gene, several polymorphic alleles have been reported such as CYP2A6*4, CYP2A6*7, CYP2A6*9, and CYP2A6*10 which are related to decreasing nicotine metabolism activity. The variation of nicotine metabolism activity could alter nicotine plasma levels. Smokers need a certain level of nicotine in their brain and must smoke regularly because of nicotine?s short half-life; this increases the number of smoked cigarettes in extensive metabolizers. Meanwhile, in slow metabolizers, nicotine plasma level may increase and results in nicotine toxicity. This will eventually lower the risk of dependence."

"Tests of addiction treatments seldom reveal where treatment exercises its effect (i.e., promoting initial abstinence, preventing lapses, and/or impeding progression from lapse to relapse). The authors illustrate analyses distinguishing effects on these milestones in a randomized trial of high-dose nicotine patch (35 mg; n = 188) versus placebo (n = 136) in adult smokers, who used electronic diaries to monitor smoking in real time during 5 weeks of treatment. High-dose patch promoted initial abstinence (hazard ratio [HR] = 1.3) and decreased the risk of lapsing among those who achieved abstinence (HR = 1.6). The biggest effect of treatment was to prevent progression to relapse among those who had lapsed (HR = 7.1). Analysis of effects by milestones may enhance understanding of cessation treatments and their mechanisms of action."

"ABSTRAK
Pendahuluan: Penelitian ini bertujuan untuk mengetahui perbedaan proporsi ketergantungan nikotin di siswa/i SMA menggunakan Fagerstrom test for nicotine dependence dengan latar belakang lingkungan perkotaan dan pedesaan.
Metode: Jumlah sampel adalah 757 siswa SMA dari enam SMA yang berbeda terdiri dari kelas 1, 2 dan 3 dipilih berdasarkan stratified cluster random sampling, siswa diminta untuk mengisi pertanyaan tentang status merokok dan mengisi Fagerstrom test for nicotine dependence jika responden adalah perokok.
Hasil: Jumlah 167 siswa dengan status merokok diperoleh ketergantungan nikotin sebanyak 28 orang (16,8%) dengan 8 orang (11,1%) di perkotaan dan 20 orang (21,1%) di daerah pedesaan. Faktor yang bermakna secara statistik terhadap ketergantungan nikotin adalah jenis kelamin, pencetus, jenis hisapan, usia pertama kali merokok lama merokok, jumlah rokok yang dihisap per hari dan indeks Brinkman. Kadar CO ekshalasi menunjukkan hubungan yang bermakna terhadap ketergantungan nikotin.
Kesimpulan: Terdapat hubungan yang bermakna antara jenis kelamin, pencetus, jenis hisapan, usia pertama kali merokok, jumlah rokok yang dihisap per hari , lama merokok, indeks Brinkman terhadap ketergantungan nikotin dan kadar CO ekshalasi terhadap ketergantungan nikotin.

ABSTRACT
Introduction:The study aims to determine the difference in the proportion of nicotine dependence among high school students using Fagerstrom Test for Nicotine Dependece set in urban and rural environment.
Method: Sample size is 757 high school students from six different high school consists of class 1, 2 and 3 were selected based on stratified cluster random sampling was asked to fill out the question of smoking status and filling Fagerstorm Test For Nicotine Dependence if the respondent is smokers.
Result :Amount of 167 students with smoking status and nicotine dependence measured results obtained by 28 (16.8%) persons with nicotine dependence with 8 (11.1%) people in urban areas and 20 (21.1%) people in the rural area. Factors were statistically significant to nicotine dependence is gender, the originator, type of inhale, age first smoked, number of cigarettes smoked per day, time of smoking and index Brinkman. CO levels and relationship with the level of nicotine dependence shows a strong and positive patterned.
Conclusion:There is a significant relationship between gender, the originator, type of inhale, age first smoked, number of cigarettes smoked per day to nicotine dependence and and level of CO exhalation to nicotine dependence."

Electronic cigarettes e-cigarettes , of which the health effects are still unknown, are significantly gaining their popularity in Indonesia. A massive news release on the internet talking about e-cigarettes might contribute to robust public rsquo;s concern about the use of e-cigarettes that could influence policy makers in considering the regulation of these devices. Therefore, this study aims to analyze how online mass media frame the message of e-cigarettes based upon topics of the story, news source, and coverage of benefits and harms of these products. This study performed a content-analysis method using a variable coding sheet in four popular online news media in Indonesia. This study found 418 articles mentioning about e-cigarettes of which 320 articles 76.6 were included in the content analysis. Majority of the article 70.9 have a negative frame and mainly talked about the policy issue of these devices. The news source that was frequently used in the articles is scientist/researcher 34.7 . The studied articles mostly presented harms of e-cigarettes rather than their benefits. Thus, most of the online news media tend to present unfavourable stories about e-cigarettes and might try to influence the readers to oppose these products.

"Latar Belakang. Candida albicans (C. albicans) merupakan flora normal rongga mulut sebagai agen utama infeksi kandidiasis oral. Asap rokok dilaporkan sebagai salah satu faktor peningkatan biofilm dan transisi perubahan morfologi C. albicans. Tujuan penelitian ini adalah menganalisis peran cigarette smoke condensate (CSC) terhadap pembentukan biofilm dan transisi perubahan morfologi C. albicans isolat saliva. Metode Candida albicans isolat saliva perokok dikultur pada CHROM Agar dan disetarakan dengan Mc. Farland 0,5 (1 × 108 CFU/ml). Selanjutnya diuji potensi pembentukan biofilm berdasarkan optikal densitas spektrofotometri pada panjang gelombang 620 nm yang hasilnya dianalisis dengan one way anova. Sedangkan transisi perubahan morfologi sel C. albicans setelah disensitisasi dengan CSC kretek dan non-kretek diamati dengan mikroskop pada pembesaran 1000x. Hasil. Akititas CSC non kretek lebih kuat menginduksi pembentukan biofilm dibandingkan dengan CSC kretek, khususnya pada waktu 24, 48, dan 72 jam (p<0,05) dibandingkan masa inkubasi 12 jam, dengan korelasi yang sangat kuat (p<0,01), hal ini sejalan dengan profil massa biofilm yang diamati secara visual dengan mikroskop. Hasil tersebut sejalan dengan transisi perubahan morfologi C. albicans dari blastospora ke bentuk psudohypha dan hypha yang diinduksi dengan CSC non-kretek lebih baik dibandingkan dengan CSC kretek dan C. albicans isolat saliva (tanpa sensitisasi dengan CSC). Kesimpulan. CSC kretek dan non-kretek dapat meningkatkan pembentukan biofilm Candida albicans isolat saliva, sekaligus mempercepat perubahan transisi morfologi dari blastospora menjadi pseudohypha dan hypha.

Candida albicans (C. albicans) is a commensal of oral cavity and the main agent of oral candidiasis. Cigarette smoke is reported as predispose factors of biofilm formation and transition of morphological changes of C. albicans. This study to analyze the role of cigarette smoke condensate (CSC) on biofilm formation and transition of morphological changes of C. albicans saliva isolates. Candida albicans smoker saliva isolate is cultured on CHROM-Agar and synchronized with Mc. Farland 0.5 (1×108 CFU/ml). Biofilm assay based on spectrophotometric density at 620 nm wavelength and data analyzed by one way ANOVA. The biofilm mass and transition of morphological changes of C. albicans cells was observed by light microscope at 1000x magnification. Result study shown The CSC no-kretek strongly induced the formation of biofilms compared with CSC kretek, particularly at 24, 48, and 72 hours (P<0.05) compared to the 12-hour, correlation (P<0.01) in accordance with the biofilm mass observed by light microscope also consistent the transition of C. albicans morphological changes from blastospora to pseudohypha and hypha (P<0,05). CSC kretek and non-kretek could increase the biofilm formation of Candida albicans saliva isolates, simultaneously accelerating the morphological transition changes from blastospora to pseudohypha and hypha."
