"Praktik cuci tangan merupakan faktor kunci dalam menurunkan dan mencegah infeksi terkait pelayanan kesehatan. Masih rendahnya praktik cuci tangan di kalangan petugas kesehatan menyebabkan angka infeksi di rumah sakit masih cukup tinggi. Penelitian ini bertujuan mengidentifikasi elemen yang menjadi penyebab masih rendahnya praktik cuci tangan enam langkah melalui pendekatan elemen strategi implementasi multimodal WHO di Unit Rawat Inap Rumah Sakit Mitra Medika Batanghari. Dengan pendekatan kualitatif, Hasil penelitian menunjukkan praktik cuci tangan enam langkah baik (89,6%) dan ketersediaan handrub berbahan dasar alkohol merupakan faktor yang paling dominan berkontribusi terhadap pelaksanaan praktik cuci tangan enam langkah.

Hand washing practice is key factor to decrease and prevent of health-care assosiated infections. Poor of hand washing practice among healthcare workers causing hospital infection rates are still high. This study aimed to identify the elements that cause the low six-step of hand washing practice through the element of WHO multimodal strategy implementation approach in Mitra Medika Batanghari Hospital. The design was cross-sectional, used quantitative approach, continued by qualitative approach. The results showed that the six-step of hand-washing practice was good (89,6%) and availability of alcohol based handrub to be the most dominant factor contributed to the implementation of six steps of hand washing practice.

"Penelitian ini bertujuan untuk mengetahui faktor pendukung dan penghambat yang dapat digunakan sebagai masukan optimalisasi pelayan program rujuk balik di instalasi rawat jalan RS Mitra Medika Batanghari. Metode penelitian. Penelitian menggunakan desain kualitatif dengan metode case study. Informan dari 22 orang pasien hipertensi yang telah dirujuk balik dan 6 orang petugas RS Mitra Medika Batanghari, sumber data dari wawancara mendalam, observasi telah dokumen. Hasil. Sebagian besar pasien hipertensi yang telah dirujuk balik tidak patuh mengunjungi FKTP. Pengetahuan pasien terhadap PRB kurang. Akses menuju fasyankes mudah. Penghambat tidak optimalnya pelayanan PRB adalah kurangnya sosialisasi monitoring dan evaluasi kebijakan PRB di lingkungan rumah sakit, tidak ada SOP terkait PRB, PIC PRB bertugas melayani PRB dan non PRB, tidak ada pelatihan terkait PRB, Pojok PRB tidak tersedia, tidak ada insentif petugas pelaksana PRB, pasien tidak patuh terhadap instruksi DPJP, tidak ada SRB rekomendasi dokter dan lembar resep khusus PRB, SRB tidak diisi lengkap, edukasi pasien singkat. Faktor pendukung pelayanan PRB yaitu petugas pelaksana berkomitmen aktif terhadap PRB, DPJP patuh merujuk balik pasien PRB, komunikasi dan koordinasi antar petugas pelaksana PRB baik, petugas pelaksanan mengetahui formularium nasional obat PRB. Kesimpulan. Program rujuk balik di instalasi rawat jalan RS Mitra Medika Batanghari belum terimplementasi dengan baik karena tidak ada panduan yang jelas terkait PRB dan masih ada pasien hipertensi yang telah direkomendasikan untuk dirujuk balik tidak melanjutkan hingga terdaftar sebagai pasien PRB. Saran. Pelayanan PRB akan terimplementasi dengan baik apabila rumah sakit memiliki panduan pelayanan PRB yang jelas yang mengatur seluruh kegiatan yang berhubungan dengan PRB serta dilakukannya monitoring dan evaluasi yang berkelanjutan. Penelitian lebih lanjut terkait PRB diharapkan dapat meneliti secara holistik dengan melibatkan seluruh stakeholder.

The aim of this study is to determine the supporting and inhibiting factors that can be used as input for optimizing the staff of the Referral Program in the outpatient installation of the Mitra Medika Batanghari hospital. Method. The study used a qualitative design with a case study method. Informants from 22 hypertensive patients who have been referred back and 6 from Mitra Medika Batanghari hospital staff, data sources from in-depth interviews, observations have been documented. Results. Most hypertensive patients who have been referred back do not comply with primary health care. The patients knowledge of the referral program is lacking. Access to health care facilities is easy. Inhibition of suboptimal service of the referral program is the lack of socialization of monitoring and evaluation of referral program policies in the hospital environment, there are no SOP related to the referral program, the PIC referral program is responsible for operating the referral program and non-referral program, there is no training related to the Referrals program, Referral program corner is not available, there is no incentive to implement referral program, patients are not in adherence with the instructions of the specialist, no recommendations from referral doctors and special referral program sheets, referral returns are not fully completed, short patient training. Supporting factors for the referral program services are that the executive officer is actively engaged in the referral program, obedient specialist doctors refer patients back to the referral program, communication and coordination between the referral program performers well, the implementation officer knows the national formulary of the referral program medication from the referral program. Conclusion. The referral program in outpatient facilities at Mitra Medika Batanghari Hospital has not been correctly implemented because there are no clear guidelines and hypertensive patients are still being advised to be referred back to continue until they are registered as referral program patients. Suggestion. The referral program service is well implemented if the hospital has a clear referral program service guide that controls all activities related to the referral program and performs continuous monitoring and evaluation. It is expected that further research on the referral program can be holistically examined by involving all stakeho."

"Tesis ini bertujuan untuk menguji validitas dan reliabilitas instrumen NAURAH untuk mengukur kepuasan pasien serta menganalisis kepuasan pasien terhadap mutu pelayanan kesehatan rawat inap Rumah Sakit Mitra Medika Bondowoso menggunakan metode servqual dan importance performance analysis (IPA). Penelitian ini bersifat kuantitatif dan deskriptif-analisis. Penelitian ini diikuti oleh 100 responden yang mengisi kuesioner kepuasan berdasarkan teori Servqual dan instrumen NAURAH. Hasil penelitian menunjukkan reliabilitas dan validitas intrumen NAURAH dan pasien Rumah Sakit Mitra Medika Bondowoso merasa puas dengan pelayanan di rumah sakit tersebut. Berdasarkan analisis IPA, ada aspek empati dari servqual yang mesti diperhatikan, dan aspek understanding dan respect dari instrumen NAURAH yang perlu dikembangkan. Kemudian Rumah Sakit Mitra Medika Bondowoso telah mempraktikkan Total Quality Management. Untuk mengembangkan empati, digunakan matriks PDCA yang mengikutsertakan aspek TQM.

This thesis aims to test the validity and reliability of the NAURAH instrument for measuring patient satisfaction and analyzing patient satisfaction with the quality of inpatient health services at Mitra Medika Bondowoso Hospital using the servqual and importance performance analysis (IPA) methods. This research is quantitative and descriptive-analytic. This research was attended by 100 respondents who filled out a satisfaction questionnaire based on Servqual theory and the NAURAH instrument. The research results showed the reliability and validity of the NAURAH instrument and that patients at Mitra Medika Bondowoso Hospital were satisfied with the services at the hospital. Based on the IPA analysis, there is an empathy aspect of servqual that must be considered, and an understanding and respect aspect of the NAURAH instrument that needs to be developed. Then Mitra Medika Bondowoso Hospital has practiced Total Quality Management. To develop empathy, the PDCA matrix is used which includes TQM aspects."

"Skripsi ini membahas mengenai perilaku cuci tangan perawat pelaksana di Unit perawatan intensif RSUD Budhi asih tahun 2011. Tujuan dari penelitian ini adalah untuk memperoleh analisis gambaran perilaku cuci tangan perawat pelaksana. Apabila perilaku cuci tangan pada perawat sudah dilakukan secara optimal, hal ini dapat meminimalisir terjadinya infeksi nosokomial. Penelitian ini menggunakan pendekatan kualitatif untuk memperoleh analisis perilaku cuci tangan perawat pelaksana di Unit Perawatan Intensif. Data yang diperoleh berdasarkan hasil wawancara mendalam, observasi, telaah dokumen, serta referensi literature-literatur yang terkait dengan topik penelitian ini. Masalah yang di temukan di unit perawatan Intensif dari waktu ke waktu adalah perilaku cuci tangan perawat yang mencuci tangan belum sepenuhnya mengikuti prosedur yang ada.
Berdasarkan Hasil penelitian diperoleh bahwa perilaku cuci tangan perawat pelaksana di Unit perawatan Intensif masih belum optimal yang disebabkan oleh beberapa faktor yang saling terkait diantaranya adalah dari factor predisposisi (pengetahuan, sikap), faktor pemungkin (ketersediaan fasilitas), faktor penguat (Pendidikan dan pelatihan, SOP, Komunikasi). Dari hasil penelitian ini di peroleh bahwa perilaku cuci tangan perawat tergolong tidak baik maka di perlukan adanya peningkatan pengetahuan perawat melalui pendidikan dan pelatihan terkait cuci tangan, adanya SOP yang di sempurnakan dan sosialisasi terkait pentingnya cuci tangan serta dengan adanya pengawasan dari pihak internal rumah sakit sehingga diharapkan timbul kesadaran dan motivasi untuk melakukan cuci tangan yang benar sesuai prosedur.

This paper discusses the behavior of nursing hand -washing executor in the intensive care unit hospitals Budhi compassion in 2011. The purpose of this study was to obtain a picture analysis of hand-washing behavior of nurse executives. When hand-washing behavior in the nurse had done optimally, this can minimize the occurrence of nosocomial infections. This study used a qualitative approach to obtain the analysis of hand-washing behavior of nurse executive at the Intensive Care Unit. Data obtained on the basis of in-depth interviews, observation, document review, as well as literatureliterature references related to this research topic. The problems found in intensive care units from time to time hand washing is the behavior of nurses washed their hands have not fully follow the procedures. Based on the results obtained that the hand washing behavior of nurse executive in the Intensive care unit is still not optimal due to several interrelated factors which include the predisposing factors (knowledge, attitudes), enabling factors (availability of facilities), reinforcing factors (education and training, SOP, Communications).
From the results of this study was obtained that the behavior of nurses wash their hands properly then classified as not in need of increased knowledge and training of nurses through education related to hand washing, the SOP in the perfected and socialization-related importance of washing hands and with the supervision of the hospital so that the internal party expected to arise awareness and motivation to perform the correct hand washing procedure.

"ABSTRAK
Prevalensi penyakit Gagal Ginjal Kronik cenderung makin tinggi sejak 3 tahun
terakhir ini (sejak tahun 2015). Sehingga mereka harus menjalani hemodialisis
(cuci darah). Adapun komplikasi saat hemodialisis yaitu demam, menggigil,
hipotensi, dan keram. Faktor-faktor yang mempengaruhi komplikasi saat
hemodialisis pada pelayanan mulai dari faktor pasien terdiri dari durasi HD, akses
HD, lama menjalani HD, komorbiditas pasien). Selain itu juga ada peran dari sisi
manajemen yaitu Standar Operasional Prosedur (SOP) tentang pencucian dialiser
ulang dan kebijakan direktur. Faktor pasien dan manajemen penting dalam kualitas
pelayanan hemodialisis rawat jalan. Metode penelitian yang digunakan adalah mix
method. Tahapan yang dilakukan pertama adalah penelitian kuantitatif dengan
pendekatan cross-sectional dan melanjutkan dengan wawancara mendalam dari
hasil penelitian. Hasil analisis bivariat, didapatkan bahwa durasi hemodialisis
berhubungan signifikan secara statistik dengan komplikasi hemodialisis (p
0.005). Hasil analisis multivariat, diperoleh durasi hemodialisis 4 jam 30 menit
berisiko menyebabkan komplikasi sebesar 2.76 (IK 95% 0.9-8.47) kali dan akses
AVF-Shunt berpengaruh paling tingi menyebabkan komplikasi sebesar 3.32 (IK
95% 0.76-14.57) kali. Dari hasil wawancara mendalam dengan informan, bahwa
kurangnya implementasi Standar Operasional Prosedur (SOP) pencucian dialiser
ulang meningkatkan risiko komplikasi. Triangulasi sumber data didapatkan bahwa
RS. Anna belum menjalankan Standar Operasional Prosedur (SOP) dengan baik
dibandingkan RS. Anna Medika dalam pengukuran volume primming atau Total
Cell Volume (TCV) dan tidak dilakukan penandaan dialiser setelah dicuci ulang.
Adanya data pencantuman volume priming pada dialiser ulang merupakan bentuk
penjaminan mutu layanan unit hemodialisis RS. Dan hal ini bisa menjadi kebijakan
direktur terkait penggunaan dialiser ulang. Kejadian komplikasi saat hemodialisis
di Rumah Sakit dapat diatasi dengan memperhatikan aspek keselamatan pasien
yaitu melakukan Standar Operasional Prosedur dengan baik dan menjamin mutu
layanan hemodialisis dengan cara membuat kebijakan penggunaan dialiser ulang.

ABSTRACT
Prevalence of Chronic Kidney Failure tends to be higher since the last 3 years
(since 2015). So they have to undergo hemodialysis (dialysis). The complications
of hemodialysis are fever, chills, hypotension, and cramps. Factors that influence
complications during hemodialysis in services are from patient factors consist of
duration, access, length of time hemodialysis, patient comorbidity. In addition,
there is also a role from the management side, namely the Standard Operating
Procedure (SOP) reuse dialyzer and director policies. Patient and management
factors are important in the quality of outpatient hemodialysis services. The
research method used is the mix method. The first step is quantitative research
with a cross-sectional approach and continues with in-depth interviews from the
results of the study. The results of the bivariate analysis showed that the duration
of hemodialysis was statistically significant with complications of hemodialysis (p
0.005). The results of multivariate analysis, obtained the duration of 4 hours 30
minutes hemodialysis at risk of causing complications of 2.76 (95% CI 0.9-8.47)
times and the highest effect of AVF-Shunt access caused complications of 3.32
(95% CI 0.76-14.57) times. From the results of in-depth interviews with
informants, that the lack of implementation of the Standard Operational Procedure
(SOP) for repeated dialysis washing increases the risk of complications.
Triangulation of data sources was found that Anna hospital has not run the
Standard Operating Procedure (SOP) well compared to Anna Medika hospital in
measuring primming volume or Total Cell Volume (TCV) and dialysis marking is
not done after repeated washing. The data of inclusion priming volume on repeated
dialiser is a form of hemodialysis unit service quality assurance. And this could be
the director's policy regarding the use of repeated dialiser. The occurrence of
complications during hemodialysis at the hospital can be overcome by paying
attention to the aspects of patient safety, namely performing the Standard
Operating Procedure properly and ensuring the quality of hemodialysis services by
making a policy of using dialiser again.
