"PT. Cibaliung Sumberdaya merupakan sebuah perusahaan pertambangan bawah tanah dan pabrik pengolahan bijih emas dan perak yang tergolong sebagai industri berisiko dan bermodal tinggi. Berdasarkan riwayat kecelakaan kerja PT. Cibaliung Sumberdaya sejak tahun 2017-2019, penerapan manajemen risiko yang baik dibutuhkan oleh PT. Cibaliung Sumberdaya. Sebagai salah satu proses dari manajemen risiko, penilaian risiko dapat dilaksanakan untuk menganalisis risiko keselamatan kerja sebagai bentuk penilaian risiko perusahaan terhadap keselamatan dan kesehatan pekerjanya. Tujuan dari penelitian ini adalah untuk mengetahui tingkat risiko keselamatan pada sampel proses kerja yang terpilih di PT. Cibaliung Sumberdaya, yakni proses produksi tambang bawah tanah dan sianidasi pabrik pengolahan. Metode yang digunakan dalam penelitian dilakukan dalam bentuk penilaian risiko melalui observasi dan pengisian formulir penilaian risiko yang menggabungkan formulir IBPR-PK PT. Cibaliung Sumberdaya dengan penggolongan jenis bahaya berdasarkan Makin dan Winder (2008) yang kemudian dianalisis secara kualitatif melalui penggunaan model matriks risiko yang dimiliki oleh PT. Cibaliung Sumberdaya. Hasil dari penelitian menunjukkan bahwa terdapat 143 risiko teridentifikasi dari kedua rangkaian proses, dengan 55 risiko pada proses produksi tambang bawah tanah dan 88 risiko pada proses sianidasi pabrik pengolahan. Dari 143 risiko yang teridentifikasi, 75 risiko (52,45%) tidak dapat diterima dan membutuhkan pengendalian lebih lanjut menurut Tabel Penerimaan Risiko Formulir IBPR-PK PT. Cibaliung Sumberdaya. Selain itu, berdasarkan penggolongan jenis bahaya Makin dan Winder (2008), prioritas risiko dari PT. Cibaliung Sumberdaya adalah risiko yang berhubungan dengan physical workplace. Terdapat 11 komponen bahaya yang berhubungan dengan physical workplace dengan total jumlah risiko sebanyak 120 risiko. Dari 120 risiko yang teridentifikasi, 3 risiko tergolong High Risk dan 64 risiko tergolong Medium Risk. PT. Cibaliung

Sumberdaya is an underground mining and processing plant of gold and silver ores company that is classified as a high risk and high budget industry. According to PT Cibaliung Sumberdayas work accident record since 2017-2019, good implementation of risk management is needed by PT. Cibaliung Sumberdaya. As one of the risk management process, risk assessment could be conducted to analyse occupational safety risks as a form of companies risk assessment towards its workers safety and health. The aim of this research is to discover the safety risk levels on selected PT. Cibaliung Sumberdaya work process samples, which are underground mining production process and processing plant cyanidation process. Method that is used in this research is done in a risk assessment form through observation and filling out risk assessment forms that combines PT. Cibaliung Sumberdaya HIRA-DC form with hazard classification based on Makin and Winder (2008) that is later on analysed qualitatively using a risk matrix model owned by PT. Cibaliung Sumberdaya. Results showed that there were 143 risks identified form both series of process, with 55 risks on underground mining production process and 88 risks on processing plant cyanidation process. From 143 identified risks, 75 risks (52.45%) of them could not be accepted and needs further control based on Risk Acceptance Table of PT. Cibaliung Sumberdaya HIRA-DC Form. Moreover, based on Makin and Winder hazard classification (2008), the risk priority of PT. Cibaliung Sumberdaya are risks related with physical workplace. There are 11 components that is related with physical workplace with the total number of 120 risks. From 120 identified risks, 3 risks are classified as High Risk and 64 risks are classified as Medium Risk."

"Latar belakang: Budaya keselamatan pasien terbukti merupakan faktor penting dalam keselamatan pasien di pelayanan kesehatan. Kerja sama tim perawat menjadi salah satu faktor yang mempengaruhi budaya keselamatan pasien di rumah sakit.
Tujuan: penelitian ini bertujuan untuk mengetahui hubungan kerja sama tim perawat dengan budaya keselamatan pasien di Rumah Sakit Pusat Otak Nasional Jakarta.
Metode: penelitian kuantitatif cross sectional dengan pengumpulan data menggunakan kuesioner Hospital Survey on Patient Safety Culture (HSOPSC) dan The Nursing Teamwork Survey (NTS) pada 160 orang perawat di unit rawat inap, rawat jalan dan kamar bedah.
Hasil: penelitian ini menunjukkan bahwa sebanyak 57,5% perawat memiliki persepsi positif mengenai budaya keselamatan pasien dan sebanyak 51,3% perawat memiliki persepsi baik mengenai kerja sama tim. Terdapat hubungan kerja sama tim perawat dengan budaya keselamatan pasien (p value: 0,001) yang berbeda pada tiap strata pendidikan. Perawat berpendidikan sarjana/ ners dengan kerjasama baik berpeluang 10,28 kali lebih besar dalam budaya positif setelah dikontrol variabel usia, masa kerja dan pelatihan.
Kesimpulan: kerjasama tim yang baik pada perawat terbukti memiliki keterkaitan dengan peningkatan perilaku budaya keselamatan pasien.

Background: patient safety culture is an important aspect for quality in the healthcare setting. Nursing teamwork is one of the affecting factor of patient safety culture.
Purpose: this study aims to determine the relationship between nursing teamwork and patient safety culture at National Brain Centre Hospital.
Methods: a qualitative cross sectional survey used questionairre of Hospital Survey on Patient Safety Culture (HSOPSC) and The Nursing Teamwork Survey (NTS) were conducted. A total of 160 nurses working at inpatient, outpatient and operating room participated in the study.
Results: this research showed 57,5% of nurses have positive perception on patient safety culture while 51,3% of nurses have an adequat perception on teamwork. There was a significant correlation between teamwork and patient safety culture (p value: 0,001) in each education grade. Bachelor of Nursing Science (BSN) graduate nurses have 10,28 times of positive perception on patient safety culture.
Conclution: adequate teamwork associated with patient safety culture improvement.

"Permasalahan kontaminasi logam berat, radionuklida alam, dan limbah plastik memerlukan solusi inovatif dan berkelanjutan. Tujuan penelitian ini adalah untuk mengevaluasi purun (Lepironia articulata) sebagai alternatif sedotan berkelanjutan dan potensi fitoremediasi. Metodologi penelitian ini mencakup Analisis Spektroskopi Serapan Atom (AAS) untuk logam berat, spektrometri gamma untuk radionuklida, dan evaluasi mikrobiologis menggunakan Angka Lempeng Total (ALT) serta Kapang Khamir, dengan penilaian penerimaan sosial melalui Structural Equation Modeling (SEM-PLS). Hasil penelitian menunjukkan purun efektif sebagai bioakumulator untuk Pb dan hyperaccumulator untuk Ra-226, Th-232, dan K-40, namun kurang efektif untuk Cd dan U-238. Pada risiko kesehatan dari kontaminasi logam berat (Pb, Cd) dan radionuklida alam (U-238, Ra-226, Th-232, dan K-40) serta kontaminasi jamur pada sedotan purun ini aman sesuai standar yang ada. Namun, sedotan purun masih memerlukan perbaikan pengemasan dan sterilisasi lebih lanjut karena sedikit melebihi batasan cemaran pada kontaminasi bakteri (4.8 x 10⁴ koloni/g). Penelitian ini juga menunjukkan potensi besar sebagai solusi berkelanjutan dengan manfaat ekonomi yang signifikan, sebagaimana ditunjukkan oleh analisis biaya-manfaat. Penerimaan masyarakat cukup tinggi, terutama di sektor pariwisata Belitung, dengan 87% penerimaan, terutama dipengaruhi oleh kepedulian terhadap lingkungan. Kesimpulan dari penelitian ini adalah bahwa purun tidak hanya aman tetapi juga memiliki potensi besar sebagai sedotan ramah lingkungan dan agen fitoremediasi untuk logam berat dan radionuklida alam.

Environmental issues such as heavy metal contamination, natural radionuclides, and plastic waste necessitate innovative and sustainable solutions. The purpose of this research is to evaluate purun (L. articulata) as a sustainable alternative for straws and as a phytoremediation agent. The methodology includes Atomic Absorption Spectroscopy (AAS) analysis for heavy metals, gamma spectrometry for radionuclides, and microbiological evaluation using Total Plate Count (TPC) as well as Mould and Yeast analysis, with social acceptance assessment through Structural Equation Modeling (SEM-PLS). The findings indicate that purun is effective as a bioaccumulator for Pb and as a hyperaccumulator for Ra-226, Th-232, and K-40, but less effective for Cd and U-238. Regarding the health risks from heavy metal contamination (Pb, Cd) and natural radionuclides (U-238, Ra-226, Th-232, and K-40) as well as fungal contamination, purun straws are safe according to FAO Codex Alimentarius and NEA/OECD standards, PerBPOM No 13 of 2019, and ISO 7387:2009. However, purun straws still require improvements in packaging and further sterilization due to slightly exceeding bacterial contamination limits (4.8 x 10⁴ colonies/g). Nevertheless, this demonstrates significant potential as a sustainable solution with notable economic benefits, as indicated by the cost-benefit analysis. Public acceptance is relatively high, particularly in the Belitung tourism sector, with an 87% acceptance rate, primarily influenced by environmental concerns. The conclusion of this study is that purun is not only safe but also holds considerable potential as an environmentally friendly straw and phytoremediation agent for heavy metals and natural radionuclides."

"Pendahuluan, ditengah tumbuhnya rasa keingintahuan para pemimpin dan pembuat kebijakan. Teknologi Informasi di bidang kesehatan, seperti halnya informatika kesehatan, belum dipahami dengan baik dan bahkan terkadang belum dapat menemukan kata sepakat untuk digunakan oleh para akademisi dan profesional di lapangan. Oleh karena itu penggunaan program aplikasi teknologi informasi kesehatan dan sistem informasi manajemen rumah sakit, sangat penting untuk individu-individu yang bekerja di segmen terbesar dari rumah sakit, yaitu informatika klinis. Menghadapi kasus gawat darurat di Unit Gawat Darurat rumah sa'rit, kebanyakan dokter lebih mengutamakan penanganan klinis dari pada non klinis, demi keselamatan pasien. Salah satu yang sering terabaikan adalah pembuatan rekam medik secara tepat. Namun kenyataannya, rekam medik yang tidak tepat justru kerap menjadi sumber masalah di kemudian hari. Metode Penelitian, kualitatif dengan kuasi eksperimental menggunakan pre test dan post lesi, karena terdapat sistem yang sudah digunakan sebagai pembanding. Hasil Penelitian data empirik membuktikan bahwa dalam sistem pelayanan kesehatan masalah-masalah medical error sering terjadi dengan derajat yang beragam, Patient Safety Indicators (PSP) dikembangkan untuk mengidentifikasi masalah- masalah medik yang berpotensi menimbulkan outeome yang tidak diharapkan. Untuk mengatasi masalah-masalah tersebut, diperlukan suatu sistem yang berbasis komputer untuk penunjang suatu keputusan (Decision Support System=DSS), yang diharapkan dapat digunakan oleh seorang atau beberapa orang pemimpin pada setiap tingkatan organisasi di rumah sakit dalam membuat keputusan sebagai dasar untuk pemecahan masalah. Dalam hal ini pengolahan data dibutuhkan untuk, pengumpulan data yang menggambarkan aktivitas , manipulasi data menjadi bentuk yang berguna, menyimpan data sampai digunakan kembali, dan menghasilkan dokumen-dokumen yang dapat digunakan oleh perorangan ataupun kelompok.

Introduction, despite the growing interest by leaders, policy makers, and others, the terminology of health informal ion technology as well as health informatics is poorly understood and not even agreed upon by academics and professionals in the field. To make program application for health informatics technology and health informatics management is most important for individuals who work in the fargesi segment in the hospital, namely clinical informatics. Faced with the emergency cases in the emergency room hospitals, most doctors prefer the clinical treatment then non-clinical, for the safety of patients. One often overlooked is the design of appropriate medical records. But in reality, the medicai records that are not exactly a frequent source of problems later on. Research Methods, qualitative with quasi-experimental using pre test and post test because there is a system designed for comparison. Research Results, empirical data prove that the health care system problems of medicai errors often occur with a degree, Patient Safety Indicators (PSI) has been developed to identify medical problems that potentially lead to unexpected results. To overcome these problems, needs a computer-based system to support the decision (Decision Support System = DSS), which is expected to be used by one or more leaders at every level of organization at the hospital in making decisions as a basis for solving the problem. In this case, the processing of data necessary for, collecting data that describe the activities, manipulating data in a useful form, save the data to be used again, and produce documents that can be used by individuals or groups."
