"ABSTRAK
Splin intrakorona sebagai terapi tambahan pada perawatan periodontal: laporan dua kasus. Berkurangnya panjang akar yang tertanam dalam jaringan periodontal dapat menyebabkan kegoyangan gigi. Melakukan splin gigi yang goyang ke gigi yang lebih stabil menggunakan prinsip stabilisasi beberapa akar gigi dapat dilakukan. Splin sementara dalam perawatan periodontal bertujuan untuk mencegah migrasi patologis, mengembalikan fungsi kunyah, menstabilkan gigi sebelum/sesudah operasi, dan mengevaluasi prognosis. Penggunaan splin intrakorona
masih kontroversial dan hanya ada sedikit literatur yang mendukung bahwa terapi ini berguna dalam mencapai jaringan periodontal yang sehat. Kami melaporkan dua kasus untuk mengevaluasi efek spin intrakorona dalam perawatan kasus periodontal. Splin intrakorona digunakan sebelum, selama dan sesudah terapi periodontal regeneratif yang menggunakan graf tulang. Penyebab kegoyangan gigi dihilangkan dan prinsip, syarat serta tatacara splin diikuti untuk mendapatkan hasil yang maksimal untuk perawatan periodontalnya. Kedua kasus dievaluasi secara radiograf 10 bulan setelah operasi dan memperlihatkan hasil yang baik. Splin merupakan terapi suportif sebelum selama dan sesudah operasi, namun bukan satu-satunya cara untuk mendapatkan stabilitas oklusi.

Reduction of the amount of tooth roots which are embedded in their periodontium could cause tooth mobility. Splinting a weaker tooth with a more stable one, and using the principle of the multiple-root stabilization is one way to overcome tooth mobility. Temporary splinting aims to prevent pathological migration, restore masticatory function, stabilize teeth before/after surgery, and evaluate the prognosis of periodontal treatment. The use of intracoronal splint is still controversial because there are only a few studies that have evaluated the effect of splinting on periodontal health. We report two cases to evaluate the effect of intracoronal splint on periodontal treatment. Two periodontal cases that use intracoronal splint before, during, and after periodontal regenerative therapy using bone graft. Causes of tooth mobility were removed and the splinting principles, terms and guidelines were mastered to get the maximum results of periodontal treatment. Both cases were evaluated radiographically 10 months after treatment. In these cases, intracoronal splint has supported the therapy before, during, or after surgery. Splinting is only for adjunctive therapy, and does not serve as the sole method in getting occlusal stability."

"Latar belakang: Terapi regenerasi jaringan periodontal pada pola kerusakan tulang alveolar horizontal selama ini belum membuahkan hasil yang memuaskan. Terapi regenerasi memerlukan scaffold, sel punca, dan signaling molecules. Scaffold dalam terapi regenerasi salah satunya yaitu kitosan. Penambahan arginylglycylaspartic acid RGD pada kitosan membantu adhesi sel. Periodontal ligament PDL cell sheet membantu regenerasi periodontal.
Tujuan: Mengevaluasi efek kitosan, RGD, dan PDL cell sheet terhadap perlekatan jaringan periodontal klinis pada kerusakan tulang alveolar horizontal.
Metode dan Bahan: Model kerusakan tulang horizontal pada M. nemestrina dibuat dengan bur dan elastik ortodontik. Sampel dibagi empat kelompok n=8 : kitosan, kitosan RGD, kitosan PDL cell sheet, dan kitosan RGD PDL cell sheet. Peningkatan perlekatan jaringan periodontal klinis dievaluasi setelah empat minggu.
Hasil: Peningkatan perlekatan jaringan periodontal klinis kelompok kitosan RGD PDL cell sheet 3,00 0,756 mm lebih baik dibandingkan kitosan 1,75 0,707 mm dan kitosan RGD 2,13 0,835 mm.
Kesimpulan: Kelompok kitosan RGD PDL cell sheet berpotensi dapat meningkatan perlekatan jaringan periodontal klinis terbaik.

Background: Periodontal regeneration therapy in horizontal bone defect has not been satisfactory yet. Tissue regeneration require scaffold, stem cells, and signaling molecule. One of scaffold that use in regenerative therapy is chitosan. Combination of chitosan with arginylglycylaspartic acid RGD has the ability to improve cell adhesion. Periodontal ligament PDL cell sheet has the ability to promote periodontal regeneration.
Objectives: Evaluate attachment gaining on clinical periodontal attachment using chitosan RGD, and PDL cell sheet in horizontal bone defect.
Material and Methods: The horizontal bone defect model of M. nemestrina was made using bur and orthodontic elastic. Regenerative therapy divided into four groups n 8 chitosan, chitosan RGD, chitosan PDL cell sheet, and chitosan RGD PDL cell sheet. Clinical periodontal attachment was evaluated after four weeks.
Results: Clinical periodontal attachment of chitosan RGD PDL cell sheet 3,00 0,756 mm was better than chitosan 1,75 0,707 mm and chitosan RGD 2,13 0,835 mm.
Conclusion: Chitosan RGD PDL cell sheet groups has the potential to increase clinical periodontal attachment.

"Ruang Lingkup dan Cara Penelitian: Prevalensi penyakit periodontal di Indonesia sangat tinggi. Cara penanggulangan penyakit ini yang umum dilakukan adalah ?flap operation? (FO). Penyembuhan FO membutuhkan waktu cukup lama. Beberapa peneliti menemukan berat penyakit periodontal erat kaitannya dengan produksi ?gingival crevicular fluid? (GCF), konsentrasi ?alkaline phosphatase? (ALP) dan protein dalam GCF. Mineral zinc (Zn) berperan dalam berbagai fungsi faali tubuh di antaranya mempercepat proses penyembuhan luka bakar dan bedah mayor. Tujuan penelitian ini untuk mengetahui manfaat suplementasi Zn per oral terhadap penyembuhan luka FO, dan apakah aktivitas ALP dapat digunakan sebagai parameternya. Sejumlah 23 subyek dibagi dalam 2 kelompok. Kelompok A (12 orang) memperoleh kapsul ZnSO4 220 mg dan kelompok B (11 orang) mendapat plasebo. Masing-masing 3x1 sehari selama 14 hari. FO dilakukan pada hari ke 5. Pada hari ke 5, 12, 19, 26 (F0, K1, K2, K3) dilakkan pemeriksaan klinik dan laboratorik. Data klinik yang diteliti adalah ?papillary bleeding index? (PBI), kedalaman poket, dan kegoyangan gigi. Pemeriksaan laboratorik meliputi konsentrasi Zn plasma; besar produksi, konsentrasi protein dan aktivitas ALP GCF. Status gizi para subyek juga diperiksa.
Hasil dan kesimpulan : Status gizi subyek baik. Data PBI dan kegoyangan gigi kelompok A saat K2 menunjukkan kemaknaan penyembuhan klinik. Konsentrasi Zn menunjukkan kemaknaan penyembuhan klinik. Konsentrasi Zn plasma A selama penelitian (F0 149, K1 127, K2 117 ug/dl) walau tidak bermakna. Saat K1 produksi GCF B meningkat bermakna (p < 0,01) dan konsentrasi protein A menurun bermakna (p < 0,01). Didapatkan perbedaan bermakna (p < 0,01) dari konsentrasi protein A saat K1 dan K3 dibandingkan dengan B. Pemberian ZnSO4 per oral dapat mempercepat penyembuhan FO. Aktivitas ALP GCF tidak mempercepat penyembuhan FO. Aktivitas ALP GCF tidak dapat dinilai, sehingga penggunaan ALP GCF sebagai parameter penyembuhan tersebut belum dapat disimpulkan.

Scope and Method of Study: the prevalence of periodontal disease in Indonesia is still high. Flap operation (FO) is the common therapy for this disease is closely related to the production of the gingival crevicular fluid (GCF), alkaline phosphatase (ALP) and protein levvel in GCF. Zinc (Zn) is a mineral with various physiological functions eg to accelerate the healing process of burns and wounds after surgery. The purpose of this study is to investigate the benefit of Zn given orally to wound healing after FO and whther the GCF ALP could be used as the parameter of the healing. The 23 subjects were devided into 2 groups. Group A (12 persons) received 220 mg ZnSO capsuls and group B (11 persons) received placebo 3 ti d for 14 days. FO was done on day 5 of the study. On day 5, 12, 19, 26 (FO, K1, K2, K#) the following were examiner: papillary bleeding index (PBI), pocket depth, looseness of the tooth, plasma Zn level, GCF production, protein level, ALP activity in GCF and the nutritional status was assessed.
Findings and conclusions: all the subjects were in good nutritional status. PBI and the looseness of the tooth of group A on K2 showed significant clinical healing. Although not significantly different the plasma Zn level of group A (FO 208, K1 227, K2 209 ug/dl) was higher than group B (FO 149, K1 127, K2 117 ug/dl). The GCF production of group B on K1 was significantly increased (p <0,01) and GCF protein level of group A was significantly decreased (p < 0,01). The difference in protein level between group A and group B on K1 and K3 was significant (p<0,01). Thus ZnSO4 given orally accelerated the healing of the FO wound. The use of GCF ALP as a parameter for the healing of an FO wound could not yet be proven.

Bila dilihat semenjak didirikannya Sekolah Kedokteran Gigi (Stovia) di Surabaya tahun 1928 maka adanya 2 Guru Besar bidang Periodontologi akan terasa amat langka apalagi bila dibandingkan dengan jumlah dokter gigi yang ada < 10.000 dokter gigi) serta penduduk Indonesia yang 200 juta jiwa. Selama hampir 69 tahun baru ada 2 Guru Besar, namun bila dilihat dart berkembangnya Ilmu ini, maka cabang ilmu Kedokteran Gigi ini merupakan cabang ilmu yang retatif masih baru dikembangkan yaitu sejak tahun 1960. Kelangkaan itu ditambah dengan banyaknya dokter gigi yang kurang berminat masuk di bagian ini, karena secara finanslil dianggap kurang menguntungkan. Kalau Prof. Aryatmo mengatakan bahwa ahli Blologi Kedokteran sama dengan ahli "perkodokan" maka di kalangan dokter gigi menganggap bahwa ahli di bidang Periodontologi sama dengan ahli "perjigongan" (istilah Surabaya ahli "pergudalan"). Namun dengan bertambah majunya ilmu pengetahuan dan teknologi, para dokter gigi sudah menyadari akan pentingnya ilmu ini. Hal ini terbukti dengan banyaknya dokter gigi yang mengambil spesialis bidang periodontologi baik dari kalangan ABRI, Depkes maupun kalangan pendidikan.

Hadirin yang saya hormati

Selama kebanyakan masyarakat hanya mengenal cabut gigi, tambal gigi, gigi palsu dan akhir-akhir ini mulai populer meratakan gigi (ortodonsi) yang oleh kebanyakan remaja sering digunakan untuk menunjukkan status sosial dari orang tuanya karena harganya yang cukup aduhai mahalnya.

Lalu apakah sebenarnya Periodontologi itu ?

Periodontologi yang berasal dad kata Per yang artinya pinggir/sekeliling, odont yang berarti gigi, logi = logos yang berarti ilmu. Jadi Periodontologi adalah ilmu (cabang ilmu kedokteran gigi) yang mempelajari pengetahuan dari jaringan sekitar gigi yang.terdiri dari jaringan gusi, tulang penyangga gigi, jaringan ikat di sekitar gigi dalam keadaan sehat dan sakit, sekaligus melakukan cara pencegahan dan perawatan penyakitnya. Untuk selanjutnya penyakit ini disebut "penyakit periodontal".

Berbagai penelitian menjelaskan bahwa penyakit periodontal ditandai dengan terjadinya kerusakan tulang dan dalam keadaan lanjut gigi menjadi goyang. Terjadinya kegoyangan gigi sering kurang diperhatikan oleh masyarakat karena tidak disertai rasa sakit. Kegoyangan gigi yang tidak/kurang diperhatikan maka lama-kelamaan akan lepas dengan sendirinya.

"The correlation between endodontic-periodontal lesion has been documented well by researches. Endodontic lesion originates from pulp, while periodontal lesion originates from periodontal tissues. Anatomically they are connected by apical foramen, lateral canal and accesories, as well as dentin tubules. The correlation appeared as the endodontic defect can be from periodontal lesion, or a periodontal defect is from a pulp tissue. Together they can emerge and form a combination lesion. Endodontic infections have been highly correlated with deeper periodontal pockets and furcation involvement in mandibular, the causal relationship between the two pathoses has not yet been established. This consensus supports the influence of degenerated or inflamed pulp that can happen on the periodintium; but not all researchers agree about the effect of periodontal disease on the pulp. Conclusion: The mechanism of endo-perio lesion need to taken care in order to have appropriate diagnostic so that the right therapy would be able to keep the teeth in the oral cavity."

"Latar belakang: Populasi lanjut usia (lansia) Indonesia diperkirakan akan terus meningkat. Kerapuhan dan penyakit periodontal merupakan kondisi kronis yang umum terjadi pada populasi lansia. Keduanya juga diketahui memiliki kesamaan dalam beberapa faktor risiko yang ada. Keterbatasan individu lansia dalam merawat diri sendiri merupakan dasar dari hubungan kerapuhan lansia dengan kondisi kesehatan periodontal. Tujuan: Menganalisis hubungan antara kerapuhan dengan status periodontal pada lansia. Metode Penelitian: Penelitian ini merupakan studi cross-sectional. Pengambilan data dilakukan pada subjek lansia berusia ≥60 tahun. Pemeriksaan tingkat kerapuhan menggunakan kuesioner kerapuhan berdasarkan resistensi, aktivitas, penyakit, usaha berjalan, dan kehilangan berat badan. Status periodontal yang diperiksa berupa skor plak, indeks kalkulus, bleeding on probing (BoP), jumlah gigi, dan stage periodontitis. Hasil Penelitian: Total 60 subjek penelitian dengan 46,6% subjek mengalami kerapuhan. Terdapat korelasi bermakna antara kerapuhan dengan skor plak, indeks kalkulus, BoP, jumlah gigi, dan stage periodontitis pada lansia (p<0,05). Terdapat perbedaan bermakna pada skor plak antara kelompok subjek rapuh dengan normal (p=0,000), pada BoP antara kelompok subjek rapuh dengan normal (p=0,003) dan kelompok subjek prarapuh dengan rapuh (p=0,003), serta pada jumlah gigi antara kelompok subjek rapuh dengan normal (p=0,011) dan kelompok subjek prarapuh dengan rapuh (p=0,023). Kesimpulan: Tingkat kerapuhan berhubungan dengan status periodontal pada lansia.

Background: Population of elderly in Indonesia is expected to continue to increase. Frailty and periodontal disease are chronic conditions that are common in the elderly population. Both are also known to have similarities in several existing risk factors. The limitations of elderly individuals in taking care of themselves are the basis of the relationship between frailty of elderly and periodontal health conditions. Objective: To analyze the relationship between frailty and periodontal status in the elderly. Method: This research is a cross-sectional study. Data collection was carried out on elderly subjects aged ≥60 years. Examination of frailty using a frailty questionnaire based on resistance, activity, disease, effort to walk, and weight loss. Periodontal clinical parameters examined were plaque score, calculus index, bleeding on probing (BoP), number of teeth, and stage of periodontitis. Results: A total of 60 research subjects with 46.6% of subjects experiencing frailty. There was a significant correlation between frailty and plaque score, calculus index, BoP, numbers of teeth, and stage of periodontitis in the elderly (p<0.05). There were significant differences in plaque scores between frail and normal subject groups (p=0.000), in the BoP between the frail and normal subject groups (p=0.003) and the pre-frail and frail subject groups (p=0.003), and in the number of teeth between the subject groups. frail to normal (p=0.011) and pre-frail subjects to frail (p=0.023). Conclusion: Frailty is associated with periodontal status in the elderly."

"Growth factor is a general term used to denote a class of naturally occuring proteins that function in the body to promote mitogenesis, directed migration and metabolic activity of cells. Growth factor to mediate repair and regeneration the periodontium. A combination of growth factor may more effectively stimulate these diverse processes of regeneration than single growth factor. Combination PDGF and IGF-I, significant amounts of newly formed bone and new cementum. PDGF and TGF beta are known to be abundant in the alfa granules of platelets. Currently the use of Palatelet Rich Plasma (PRP) into a bone graft combination showed a significant gain in periodontal regeneration. It is probable that it's used to promote periodontal regeneration."

"Kejadian kardiovaskular adalah penyebab kematian utama pada artritis reumatoid AR . Periodontitis diketahui berperan dalam patogenesis disfungsi endotel pada AR. E-selectin merupakan penanda disfungsi endotel yang spesifik dihasilkan oleh endotel. Tujuan penelitian ini adalah mengetahui efek terapi periodontal terhadap kadar E-selectin pada pasien AR. Penelitian ini merupakan uji klinis randomisasi pada penderita AR yang berobat di Poliklinik Reumatologi RSCM periode Maret-Mei 2017. Pengambilan sampel dilakukan secara konsekutif. Randomisasi dilakukan dengan randomisasi blok. Subyek dibagi menjadi kelompok terapi periodontal dan kontrol. Dilakukan Uji t untuk melihat perbedaan selisih E-selectin awal dan akhir studi antara kelompok intervensi dengan kontrol. Periodontitis ditemukan pada 31 subyek 64,5 . Tidak didapatkan perbedaan selisih E-selectin awal dan akhir studi yang signifikan secara statistik antara kelompok intervensi dengan kontrol p=0,303 . Sebagai kesimpulan tidak didapatkan pengaruh terapi periodontal terhadap kadar E-selectin pada pasien AR.

Cardiovascular event is the main cause of mortality in rheumatoid arthritis RA . Periodontitis is known to be involved in the pathogenesis of endothelial dysfunction in RA. E selectin is a marker of endothelial dysfunction and was expressed specifically in endothelial cells. The objective of this study was to determine the effect of periodontal treatment on E selectin level in RA patients. This was a randomized clinical trial in RA patients visiting Rheumatology Clinic RSCM between March May 2017. Samples were collected using consecutive sampling method. Randomization was done using block randomization. Subjects was divided into nonsugical periodontal treatment group and control group. T test was used to measure the difference of delta E selectin before after study between periodontal treatment group and control. Periodontitis was found in 31 subjects 64,5 . There was no statistically significant difference of delta E selectin before after treatment between periodontal treatment group and control. As a conclusion, periodontal treatment has no effect on E selectin level in RA patients."

A periodontal health survey, using the WHO crireria 1997. was conducted among 585 Palestinian refugees living in the United Nations Camps in Jordan, the sample consists of 363 females and 222 males aged 15-54 years. The purposes ofthh study were to determine the prevalence and the severity of periodontal disease and the treatment needs among this population and to correlate the periodontal status with age and gender. By using WHO periodontal probes, the community periodontal index (CPI), the treatment needs (TN) and the loss of attachment (LA) were measured for subject. The number of permanent teeth loss was also recorded. Females were found to have worse periodontal conditions than males. Also, a significant age difference was present. All subjects of the present population had experienced periodontal disease, where 34% and 43% of them had a shallow pocket of 4-5 mm and deep pocket of 6 mm or more respectively.
The results of this study have shown that 5.48 of sextant per subject in this population were with bleeding or higher, 4.80 with calculus or higher, 2.48 with shallow pockets or higher, and 0.88 with deep pocket. This study demonstrated that the loss of attachment increased significantly with increasing age and 42% of this population had normal level of periodontal attachment, while the rest of them (58%) had different amounts of attachment loss. Almost all subjects of this population needed oral hygiene instruction, scaling and/or removal of overhangs and 43% of them needed complex periodontal treatment. The mean number of loss permanent teeth was among this population with significant age and gender difference. Our data indicated that high prevalence and severity of periodontal disease were observed in this population require programs for both prevention and treatment."
