"Fraktur merupakan masalah sistem muskuloskeletal yang paling sering terjadi. Penanganan fraktur yang tidak tepat dapat mengakibatkan osteomielitis. Osteomielitis tibia merupakan osteomielitis terbanyak kedua setelah femur. Perawat berperan sebagai pemberi asuhan keperawatan, peneliti dan innovator. Peran sebagai pemberi asuhan keperawatan dilakukan dengan pendekatan self care deficit theory pada klien osteomielitis. Masalah yang paling sering terjadi pada klien dengan gangguan sistem muskuloskeletal adalah konstipasi, peran sebagai peneliti dilakukan dengan praktik berbasis bukti (evidence based nursing) yaitu bowel management untuk mencegah konstipasi pada klien pasca Total Hip Replacement (THR) dan Total Knee Replacement (TKR). Inovasi pada tempat praktik dilakukan dengan menyediakan panduan komunikasi dengan metode SBAR pada kasus dengan gangguan sistem muskuloskeletal untuk mendukung komunikasi efektif inter-intra disiplin saat handover maupun komunikasi melalui telepon.

Tibial osteomyelitis is the second largest after the femoral osteomyelitis. Nurses act as care providers, researchers and innovators. Nurse as care provider use Orem's self-care deficit theory approach for client with osteomyelitis. Nurse as researcher found that the most often problem that occurs in clients with musculoskeletal system dysorder attempt Total Hip Replacement (THR) and Total Knee Replacement (TKR) is constipation that need to be performed bowel management as evidence based nursing. Nurse as innovator provides guidance communication using SBAR method in cases musculoskeletal system dysorder to support effective communication inter-intra discipline when handover or communication by telephone."

"Osteoartritis pada lutut (OA genu) merupakan OA yang bersifat idiopatik, yang ditandai dengan degenerasi tulang rawan lutut yang berdampak terhadap terjadinya kecacatan atau disability. Akibat disability yang dialaminya, pasien kesulitan untuk melakukan perawatan diri atau self care. Terkait hal tersebut, peran perawat spesialis keperawatan medical bedah diperlukan dalam pemberian asuhan keperawatan dengan pendekatan teori self care Orem pada pasien dengan osteoartritis genu. Peran perawat juga diperlukan dalam penelitian antara lain menerapkan praktik keperawatan berbasis bukti (Evidence based practice nursing) pada pasien ortopedi yang akan menjalani pembedahan dengan anestesi umum untuk mencegah pneumonia post operasi melalui intervensi oral hygiene dengan chlorhexidine mouthwash 0.12%. Hasil penerapan EBN ini efektif yaitu dapat mencegah infeksi pneumonia post operasi pada seluruh partisipan. Sebagai inovator, kegiatan inovasi yang dilakukan adalah membuat buku panduan komunikasi SBAR (Situation, Background, Assesment, Recommendation) yang fokus kepada masalah-masalah keperawatan pada kasus ortopedi untuk digunakan oleh perawat di ruangan pada saat handover.

Osteoarthritis of the knee (OA genu) is an idiopathic OA, which is characterized by degeneration of knee cartilage that may cause disability. Due to the disability they experienced, patient found it difficult to perform self-care. Related to this, the role of medical-surgical nurse specialist are needed in providing nursing care based on Orem?s theory of self care in patients with osteoarthritis of the knee.

The role of the nurse is also required in research, for example, applying evidence-based nursing practice in patients undergoing orthopedic surgery under general anesthesia to prevent postoperative pneumonia through the intervention of oral hygiene with a 0.12% chlorhexidine mouthwash.

The results of the implementation of this EBN was effective to prevent postoperative pneumonia infection among participants. As an innovator, the innovation activity was a SBAR (Situation, Background, Assessment, Recommendation) communication guide book which focused on the problems of nursing in orthopedic cases to be used by nurses in the ward while they do handover."

"Praktik Spesialis Keperawatan Medikal Bedah dengan Kekhususan pada Sistem Muskuloskeletal ini bertujuan untuk mengaplikasikan peran perawat sebagai pemberi asuhan, pengelola, pendidik dan peneliti. Peran sebagai pemberi asuhan dilakukan dengan mengelola sebanyak 30 pasien yang mengalami masalah pada sistem muskuloskeletal dan satu pasien kelolaan utama dengan kasus Fraktur Terbuka Ekstremitas Bawah dengan pendekatan teori self-care Orem. Peran perawat sebagai peneliti dilakukan dengan penerapan tindakan keperawatan yang berbasis bukti ilmiah (Evidence-Based Nursing Practice) yaitu dengan menerapkan wound drain management untuk mencegah kehilangan darah berlebih post operasi Total Joint Arthroplasty. Sedangkan peran sebagai pengelola dilakukan deengan menyusun suatu panduan komunikasi efektif yang terstruktur dengan metode SBAR untuk mengkomunikasikan kondisi pasien dengan masalah sistem muskuloskeletal. Seluruh rangkaian kegiatan tersebut bertujuan untuk mewujudkan asuhan keperawatan holistik dalam meningkatan kualitas pelayanan keperawatan.

Clinical Practice of Medical-Surgical Nursing Specialist in the specialty of Orthopaedic Nursing aims to apply the role of nurses as a direct patient care providers, managers, educators and researchers in the clinical setting. Role as a care provider has done by managed 30 patients with musculoskeletal problems and one patient (major managed cases) with Open Fracture of Lower Extremities using Orem’s Self-Care Nursing Theory Approach.

The role of nurses as a researcher has conducted by applying the nursing action based on scientific evidence (Evidence-Based Nursing Practice), the evidence is wound drain management to prevent excessive blood loss post Total Joint Arthroplasty. The role as a nursing manager has been done by applying SBAR method as a strategy to achieve effective communication at orthopaedic ward. All activities aim to realizing the holistic nursing care in order to improve the quality of nursing services."

"Spinal Cord Injury (SCI) merupakan suatu gangguan muskusloskeletal yang berdampak besar baik dari segi fisik, sosial dan ekonomi. SCI berdampak kelemahan pada ekstremitas, tingkat keparahan kelemahan bergantung pada area spinal cord yang terganggu. Untuk itu perlu perawatan yang lama dalam pemulihan dan adaptasi terhadap kelemahan terkait aktivitas sehari hari pasien. Laporan ini merupakan hasil analisa kegiatan praktek residensi penerapan Evidence Based Nursing (EBN) dengan pendekatan Model Adaptasi Roy dalam peran care provider, peneliti, innovator dan role model. Hasil dari penerapan model Adaptasi Roy dapat diaplikasikan pada pasien dengan gangguan muskuloskeletal dimana pasien membutuhkan kemampuan adaptasi dalam menjalani perawatan. Hasil penerapan EBN menunjukkan bahwa relaksasi mendengarkan musik dan masase punggung dapat mengurangi nyeri dan kecemasan. Spinal Cord Injury (SCI) is one of musculosceletal disorder that distrub physic, social and economic faktors. Effect of SCI are extremities weakness, para parese depand on injury areas of spinal cord. Furtermore patients needs adaptation in his/hers paraparese to doing actuvuty daily living (ADL). This article shows tat clinical practice of recidency using Adaptation Roy Model as provider care, researcher, innovator and role model. Result of Adaptation Roy Modelapproach shows that effective in musculosceletal disorder patient care, who nees ability to asaptation in ADL. Result of Evidence Based Nursing shows that relaxation music therapy and back massage effective to reduce pain and anxiety. Result of innovation project format of SBAR (Situation, Background, Assesment and Recommendation)."

"Professionalime ilmu keperawatan adalah bagian disiplin ilmu keperawatan sampai dengan keperawatan sub spesialisasi dimana perawat spesialis sebagai seorang perawat yang dipersiapkan melampaui level seorang perawat generalis yang berwenang untuk praktek sebagai spesialis dengan keahlian dalam cabang bidang keperawatan. Ners spesialis keperawatan medikal bedah dengan peminatan muskuloskletal berperan dalam perawatan kekhususan masalah dan kasus kelolaan yang berkaitan dengan orthopedi atau muskuloskletal. Tujuan utama memberikan asuhan keperawatan secara komprehensif pada pasien kasus Spinal Cord Injury dengan trauma melalui pendekatan keperawatan model teori self-care Orem. Proyek inovasi berupa pengkajian resiko DVT dengan menggunakan Autar Scale adalah bentuk pengembangan dari intervensi keperawatan untuk pencegahan resiko terjadinya DVT pada pasien dengan kasus muskuloskletal. Tujuan seluruh kegiatan yang dilakukan selama kegiatan residensi adalah mewujudkan asuhan keperawatan yang komprehensif berdasarkan bukti ilmiah terkini.

Innovation projects are also carried out as an embodiment of the role as an innovator, namely through the implementation of an innovation project in the form of a DVT (Deep Vein Thrombosis) risk assessment format."

Musculoskeletal system disorders can occur at any age and affect the quality of life of sufferers. If not treated promptly and appropriately, this disorder can lead to increased morbidity and mortality. Nursing interventions based on the results of evidence review in medical surgical nursing practice are necessary in order to obtain professional and quality services. The Specialist Nurse Residency Practice is carried out to implement the role of Specialist Nurse as a care giver, educator, manager of nursing services, researcher, reformer, and executor of duties based on delegation of authority. The role of the Specialist Nurse as a care giver is carried out directly to 1 client as the main case managed with bilateral genu Osteoarthritis and 30 clients as resume cases with a variety of diseases. Orem's self-care theory is utilized as an approach of the research. The role of Specialist Nurses as educators is carried out not only for patients but also with colleagues in the hospital service setting through journal reading activities based on the field phenomena. The role of the Specialist Nurse as a researcher with the application of EBN (Evidence Based Nursing) is using chewing gum as a prevention of postoperative ileus in hip fractures. Meanwhile, the role of the Specialist Nurse as a reformer is carrying out an innovative project in the form of initial risk assessment of deep vein thrombus (DVT) using the "Autar Scale" as the core idea in carrying out nursing care for patients with lower limb disorders."

"Kanker adalah penyakit yang disebabkan oleh pertumbuhan sel-sel tubuh yang tidak terkontrol dan mampu bermetastasis ke organ tubuh lainnya. Teori self care deficit dapat diterapkan pada klien dengan kanker yang mengalami gangguan dalam memenuhi kebutuhan sehari-hari untuk meningkatkan kondisi klinis. Tujuan karya ilmiah akhir ini adalah sebagai laporan praktik praktik keperawatan medical bedah onkologi di RSP Fatmawati dan RSPAD Gatot Subrort Jakarta. Terdapat tiga pokok bahasan pada karya ilmiah ini 1 penerapan Self Care Deficit Theory Orem pada klien kanker tiroid, 2 intervensi relaksasi terbimbing untuk mengurangi kecemasan dan gejala distress pasien kanker payudara yang menjalani kemoterapi 3 proyek inovasi video edukasi untuk mempromosikan efek samping manajemen kemoterapi. Kesimpulan: Self Care Deficit Theory Orem dapat meningkatkan kondisi klinis pasien yang didiagnosis kanker tiroid. Intervensi relaksasi terbimbing dapat digunakan sebagai pilihan manajemen alternatif pelengkap untuk mengurangi kecemasan dan gejala gangguan klien kanker payudara yang menjalani kemoterapi. Video manajemen efek samping kemoterapi dapat memberikan pendidikan kesehatan yang bermanfaat bagi pasien yang menjalani kemoterapi.

Cancer is a disease caused by the growth of body cells that are not controlled and capable of metastasizing to other organ of body. Self care deficit is theory on clinical improvement of cancer client that disturbed of activity daily living. The purpose of scientific work is a practice report of medical and surgical nursing residency of oncology in RSP Fatmawati and RSPAD Gatot Subrort Jakarta. There are tree contains of scientific work 1 the application of Orem rsquo;s Self Care Deficit Theory of Nursing in Thyroid cancer client, 2 guided imagery intervention to reduce anxiety and distress sindrom of breast cancer patient undergoing chemotherapy 3 innovation project video education to promote management side effect of chemotherapy.Conclusion: Orem rsquo;s Self Care Deficit Theory of Nursing could enhance the clinical condition of patients diagnosed thyroid cancer. Intervention guided imagery can be used as complementary alternatife management option to reduce anxiety and symptom distress of breast cancer client undergoing chemotherapy. Management side effect of chemotherapy ducation video is expected to provide a useful healt education for patient undergoing chemotherapy."

"Praktik residensi keperawatan medikal bedah yang dilakukan selama dua semester telah memberikan asuhan keperawatan pada pasien dengan gangguan respirasi, melakukan implementasi evidence based nursing dan dan proyek inovasi serta pengabdian masyarakat yang dilakukan di RSUP Fatmawati dan RSUP Persahabatan. Peran pemberian asuhan keperawatan pada kasus kelolaan utama yaitu PPOK eksaserbasi grup D dan 30 pasien resume menggunakan teori keperawatan self care Orem, teori ini digunakan karena masalah respirasi adalah masalah kronik, teori ini dapat diimplentasikan oleh perawat spesialis untuk meningkatkan perawatan diri pasien (self care) sesuai dengan kemampuannya untuk dapat meningkatkan kualitas hidup pasien. Implementasi EBN berupa balloon blowing exercise terbukti mampu mengurangi keluhan sesak napas pada pasien CLRD, mudah dilakukan, tidak memerlukan waktu lama, murah dan tidak menimbulkan efek samping. Penerapan proyek inovasi pengembangan self management: written action plan dengan media booklet terbukti dapat meningkatkan manajemen diri pasien PPOK. Booklet dapat membantu baik keluarga maupun pasien PPOK dalam mengetahui kondisi PPOK pasien serta melakukan manajemen yang tepat sesuai dengan kondisi pasien. Pengabdian masyarakat berupa edukasi kepada pasien TB Paru tentang TB Paru dan manajemen terapi pengobatannya terbukti dapat meningkatkan pengetahuan pasien, hal ini kemudian diharapkan dapat menurunkan angka putus obat pasien TB Paru.

The medical surgical nursing residency practice which was carried out for two semesters has provided nursing care to patients with respiratory disorders, implemented evidence based nursing and and innovation projects as well as community service carried out at Fatmawati Hospital and Friendship Hospital. The role of providing nursing care in the main managed cases, namely COPD exacerbation group D and 30 patients resumed using Orem's self care nursing theory, this theory is used because respiratory problems are chronic problems, this theory can be implemented by specialist nurses to improve patient self care according to its ability to improve the patient's quality of life. The implementation of EBN in the form of balloon blowing exercise is proven to be able to reduce complaints of shortness of breath in CLRD patients, is easy to do, does not take long, is inexpensive and does not cause side effects. The implementation of self-management development innovation projects: written action plans with booklet media has been proven to improve COPD patient self-management. Booklets can help both families and COPD patients in knowing the patient's COPD condition and carrying out appropriate management according to the patient's condition. Community service in the form of educating pulmonary TB patients about pulmonary TB and management of treatment therapy has been proven to increase patient knowledge, this is then expected to reduce the dropout rate of pulmonary TB patients."
