"ABSTRAK
Praktek Kerja Profesi di Puskesmas Kecamatan Tanah Abang yang bertempat di Jl. KH. Mas
Mansyur No. 30 Kecamtan Tanah Abang. Kegiatan praktek kerja profesi selama 2 minggu dari
tanggal 1 Juli ? 14 Juli 2015. Kegiatan ini bertujuan agar mahasiswa mengetahui peranan, tugas
dan tanggung-jawab Apoteker dalam praktek pelayanan kefarmasian di puskesmas, memiliki
pengetahuan, keterampilan, sikap professionalism, wawasan dan pengalaman nyata untuk
melakukan pekerjaan kefarmasian di puskesmas, melihat dan mempelajari strategi dan
pengembangan praktik profesi Apoteker di puskesmas, memiliki gambaran nyata tentang
permasalahan pekerjaan kefarmasian di puskesmas, mampu berkomunikasi dan berinteraksi
dengan tenaga kesehatan lain di puskesmas. Mahasiswa pada kegiatan ini memahami peran, tugas
dan tanggung jawab Apoteker dalam praktek pelayanan kefarmasian di puskesmas kecamatan
Tanah Abang meliputi pengelolaan obat dan pelayanan farmasi klinik. Memiliki pengetahuan,
keterampilan, sikap professionalism serta wawasan dan pengalaman nyata pekerjaan kefarmasian
di puskesmas meliputi pelayanan resep dokter, pemberian informasi obat, pengelolaan obat dan
bahan medis habis pakai. Mahasiswa dapat melihat dan mempelajari strategi dan pengembangan
praktik profesi Apoteker di puskesmas diantaranya konseling pada pasien penyakit kronis.
Kendala pekerjaan kefarmasian di puskesmas kecamatan Tanah Abang adalah pada proses
pengadaan melalui e-catalogue dimana obat yang sering kali kosong dan disiasati dengan
pembelian langsung di apotek rekanan. Mampu berkomunikasi dan berinteraksi dengan tenaga
kesehatan lain diantaranya dokter perawat. ABSTRACT Profession Internship Program at Puskesmas Kecamatan Tanah Abang located at Jl. KH. Mas

from the date of July 1 to July 14, 2015. The purpose of this program allow students to understand
the role, duties and responsibilities of pharmacists in pharmacy services at community health
centers, have the knowledge, skills, attitude, professionalism, insight and a real experience for
pharmacy services, learn strategies and development of professional pharmacist practice in health
centers, have a real picture about the problems of pharmacy practice, communicate and interact
with other health workers in health centers. Students on these activities can understand the role,
duties and responsibilities of pharmacists in the practice of pharmacy services include drug
management and clinical pharmacy services. Have the knowledge, skills, attitude, professionalism,
the real experience includes prescription services, giving drug information, drug management and
medical consumable materials. Students can learn strategies and developing of pharmacists
practice in health centers include counseling for patients with chronic disease. The problems in
health center is supplying by e-catalog in which drugs are empty and circumvented by direct
purchase at the pharmacy partners. Ability to communicate and interact with other health
professionals including doctors and nurses.

"Praktek Kerja Profesi (PKP) di industri farmasi dilaksanakan di Departemen ProduksiPT. Medifarma Laboratories. Kegiatan ini berlangsung selama sembilan minggu dari tanggal 01 Juli sampai dengan tanggal 31Agustus 2015. PKP di industri farmasi bertujuan agar mahasiswa apoteker mengerti peranan apoteker, memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian, memahami penerapan Cara Pembuatan Obat yang Baik (CPOB), dan memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di industri farmasi. Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa peran apoteker penting untuk menghasilkan obat-obatan yang aman, bermutu, dan berkhasiat. Apoteker di Departemen Produksi berperan dalam proses produksi dari awal hingga akhir. Mahasiswa apoteker telah berhasil memperoleh gambaran umum dari pelaksanaan pekerjaan kefarmasian di PT.Medifarma Laboratories, khususnya di bidang produksi. Berdasarkan pengamatan, PT Medifarma Laboratories telah menerapkan seluruh aspek CPOB dengan baik.
Profession Internship at pharmaceutical industry was held at Production Department of PT Medifarma Laboratories. This activity was held for nine weeks from July 1st until August 31th 2015. Profession internship at pharmaceutical industry was intended to make apothecary student understand the role of pharmacist, have insight into the implementation of pharmaceutical practice, understand the implementation of Cara Pembuatan Obat yang Baik (CPOB), and know the issues in pharmaceutical practice in pharmaceutical industry. Based on the activities, pharmacists in Production Department play a role in production process at every stage of the process. Apothecary student have obtained insight into general routine activities in Production Department. Based on the observation, PT. Medifarma Laboratories have applied all aspects of CPOB."

"ABSTRAK
Pelayanan kefarmasian di Rumah Sakit merupakan bagian yang tidak terpisahkan dari sistem pelayanan kesehatan di Rumah Sakit yang berorientasi pada pelayanan pasien, penyediaan sediaan farmasi, alat
kesehatan dan bahan medis habis pakai yang bermutu dan terjangkau termasuk pelayanan farmasi klinik. Praktek Kerja Profesi (PKP) di rumah sakit dilaksanakan di Rumah Sakit Anak dan Bunda Harapan Kita. PKP di rumah sakit bertujuan agar mahasiswa apoteker mengerti peranan apoteker di rumah sakit, memiliki wawasan tentang pelaksanaan pekerjaan kefarmasian di rumah sakit, dan memiliki gambaran nyata tentang permasalahan pekerjaan kefarmasian di rumah sakit. Kegiatan ini berlangsung selama enam minggu dari tanggal 6 Juli sampai dengan tanggal 31 Agustus 2015 dengan tugas khusus yaitu evaluasi interaksi obat dan kesesuaian dosis pada resep depo farmasi unit gawat darurat bulan Juli 2015. Masalah yang terjadi di Rumah Sakit Anak dan Bunda Harapan Kita
adalah belum optimalnya pelaksanaan pelayanan farmasi klinik mulai dari jumlah tenaga apoteker dan sarana pra sarana yang mendukung.ABSTRACT Hospital pharmacy services are an integral part of the health care system in the Hospital which focused on patient care, the provision of pharmaceutical, medical devices and medical consumable material quality and affordable including clinical pharmacy services. Profession Internship Program at Harapan Kita Mother and Child Hospital aims to make students understand the role of pharmacists in the hospital, having an knowledge
about the execution of work at the hospital pharmacy, and have a real picture of the problems of employment at the hospital pharmacy. This activity lasted for six weeks from July 6 until August 31, 2015 with a special task is the evaluation of drug interactions and dose conformity to the prescription in emergency unit on July 2015. Problems that occur at Harapan Kita Mother and Child Hospital are implementation of clinical pharmacy services didn?t fully work startsg from the number of pharmacists
and facility which has been provided."

Praktek Kerja Profesi di Apotek Hidup Baru yang bertempat di Jl. Hidup Baru
No. 15 Kav. I Kelurahan Gandaria Utara, Jakarta Selatan. Kegiatan Praktek Kerja
Profesi berlangsung selama empat minggu dari tanggal 3 Agustus hingga 29
Agustus 2015. Tujuan dari kegiatan ini agar mahasiswa dapat memahami tugas
dan tanggung jawab Apoteker dalam pengelolaan apotek, memiliki wawasan dan
pengalaman praktis untuk melakukan praktek kefarmasian di apotek serta
memiliki gambaran nyata mengenai permasalahan praktek kefarmasian.
Mahasiswa pada kegiatan ini memahami tugas dan tanggung jawab Apoteker
yaitu kegiatan pengelolaan sediaan farmasi dan kegiatan pelayanan farmasi klinik.
Wawasan dan pengalaman praktis yang didapat oleh mahasiswa meliputi
perencanaan, penerimaan, pencatatan dan pelaporan serta pengkajian resep,
dispensing disertai dengan pemberian informasi obat, swamedikasi, pengelolaan
sediaan narkotika dan psikotropika. Permasalahan dalam praktek kefarmasian di
Apotek Hidup Baru antara lain terdapat beberapa obat yang tidak tersedia, namun
dapat disiasati dengan pembelian secara langsung di apotek rekanan.

activity lasted for four weeks from August 3 until August 29, 2015. The purpose

Praktek Kerja Profesi (PKP) di apotek bertempat di Apotek SamMarie Basra Jalan
Basuki Rahmat No. 31 Pondok Bambu, Jakarta Timur. Kegiatan ini berlangsung
selama empat minggu dari tanggal 3 sampai dengan tanggal 31 Agustus 2015.
PKP di apotek bertujuan agar mahasiswa apoteker mengerti peranan Apoteker
Pengelola Apotek (APA), memiliki wawasan tentang pelaksanaan pekerjaan
kefarmasian, dan memiliki gambaran nyata akan permasalahan pekerjaan
kefarmasian yang terjadi di apotek. Berdasarkan kegiatan PKP yang dilakukan,
APA di Apotek SamMarie Basra secara umum telah melaksanakan tugas dan
tanggung jawab dalam pengelolaan apotek terkait kegiatan teknis kefarmasian dan
kegiatan nonteknis kefarmasian sesuai dengan peraturan perundang-undangan
yang berlaku. Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai
kegiatan rutin yang dilakukan di apotek. Kegiatan teknis kefarmasian yang
dilakukan di apotek meliputi pengadaan, penerimaan, penyimpanan, penjualan,
pengelolaan narkotika, pengelolaan psikotropika, dan dokumentasi resep. Adapun
kegiatan nonteknis kefarmasian yang dilakukan di apotek adalah penyusunan
resep dan struk pembayaran, penandaan harga pada resep kredit untuk penagihan,
dan konsultasi pemakaian obat kepada pasien. Masalah yang terjadi di Apotek
SamMarie Basra adalah lemari penyimpanan narkotika dan psikotropika yang
tidak terpisah dan lemari tersebut kurang kuat.
ABSTRACT Profession Internship at pharmacy was held at Apotek SamMarie Basra. This
activity was held for a month on August 2015. Profession Internship at pharmacy
was intended to make apothecary student understand the role of pharmacist,
understand into the implementation of pharmaceutical practices, and know the
issues in pharmaceutical practice in pharmacy. Based on the activities, pharmacist
as Pharmacy Manager have been carrying out the duties and responsibilities in
pharmacy management. Apothecary student have obtained insight into routine
activities in pharmacy. Pharmaceutical technical activities include procurement,
reception, storage, sales, management of narcotics, management of psychotropics,
and prescription documentation. Non-technical activities include archiving
prescription and receipt of payment, price labelling of credit prescription for
billing purpose, and the making of Daily Sales Summary Report. Issue at
SamMarie Basra Pharmacy is the storage boxes of both narcotics and
psychotropics are not separeated each other and the storage boxes are not strong
enough.

Rumah sakit adalah institusi pelayanan kesehatan yang menyelenggarakan pelayanan kesehatan perorangan secara paripurna yang menyediakan pelayanan rawat inap, rawat jalan, dan gawat darurat.Apoteker di Rumah Sakit memiliki peran untuk mengelola perbekalan farmasi dan melakukan pelayanan farmasi klinik.Praktek Kerja Profesi Apoteker di RSPAD Gatot Soebroto Ditkesad bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawabnya di Rumah Sakit, memiliki wawasan, pengetahuan, keterampilan dan pengalaman praktis untuk melakukan praktik kefarmasi di Rumah Sakit, memiliki gambaran nyata tentang permasalahan praktik kefarmasian serta mempelajari strategi dan kegiatan-kegiatan yang dapat dilakukan dalam rangka perkembangan praktik kefarmasian di Rumah Sakit. Kegiatan yang dilakukan diantaranya mempelajari sistem pengelolaan perbekalan kefarmasian di RSPAD Gatot Soebroto Ditkesad , praktek pelayanan farmasi klinik, dan melakukan pengkajian masalah terkait obat pada salah satu pasien dengan penyakit tertentu. Dalam rangka peningkatan kualitas pelayanan kefarmasian di RSPAD Gatot Soebroto Ditkesad baik di rawat inap maupun rawat jalan perlu dilakukan penambahan sumber daya manusia yakni apoteker sehingga pekerjaan kefarmasian dapat dilakukan secara maksimal dan perlu dilakukan peningkatan kerjasama antara tenaga kefarmasian terutama apoteker dengan tenaga medis lainnya ABSTRACT Hospital is a health care institution that organizes personal health services in plenary that provides inpatient, outpatient, and emergency. Pharmacist at Hospital has role to manage pharmaceutical supply and clinical pharmacy service The purposes of Internship Program in RSPAD Gatot Soebroto Ditkesad are for understanding about the roles and responsibilities of pharmacist candidate in pharmaceutical practice and management in hospital, having perceptions, knowledge, skills, real experiences and illustrations about pharmaceutical practice problems which probably happen, and finding strategic ways to develop pharmaceutical practices in hospital. The acitivities were studying about system management of pharmacy in hospital, clinical pharmacy practice, and doing assessment dug related problem in one of patient who has certain disease. Praktek kerja profesi di Apotek dilaksanakan di Apotek Kimia Farma No. 298
yang beralamat di Jalan Bendungan Hilir Nomor 27, Jakarta Pusat. Praktekkerja
dilaksanakan mulai tanggal 2 November ? 30 November 2015. Pelaksanaan
praktek kerja ini bertujuan agar mahasiswa apoteker: mampu memahami tugas
dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktek
pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika
yang berlaku; memiliki wawasan, pengetahuan, keterampilan, dan pengalaman
praktis untuk melakukan praktek kefarmasian di apotek; dan memiliki gambaran
nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan
kegiatan-kegiatan yang dapat dilakukan dalam rangka pengembangan praktek
