"ABSTRAK
Praktik Kerja Profesi (PKP) di puskesmas bertempat di Puskesmas Kecamatan
Kembangan, Jakarta Barat. Kegiatan ini berlangsung selama tiga minggu dari
tanggal 30 Juni hingga 24 Juli 2015. PKP di puskesmas bertujuan agar mahasiswa
apoteker mengerti peranan apoteker, memiliki wawasan tentang pelaksanaan
pekerjaan kefarmasian, mempelajari strategi dan pengembangan praktik profesi
apoteker, memiliki gambaran nyata akan permasalahan pekerjaan kefarmasian di
puskesmas, serta berkomunikasi dan berinteraksi dengan tenaga kesehatan lain.
Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di
Puskesmas Kecamatan Kembangan telah melaksanakan kewajibannya dengan baik.
Mahasiswa apoteker telah berhasil memperoleh wawasan mengenai kegiatan rutin
yang dilakukan di puskesmas. Kegiatan manajerial meliputi perencanaan
kebutuhan, pengadaan, penerimaan, penyimpanan, pendistribusian, pengendalian,
pencatatan, pelaporan, dan pengarsipan obat, serta pemantauan dan evaluasi
pengelolaan obat dan bahan medis habis pakai, dan kegiatan farmasi klinis meliputi
pelayanan resep (dispensing), Pelayan Informasi Obat (PIO), konseling, homecare,
dan penyuluhan kesehatan. peningkatan mutu pelayanan kefarmasian dilakukan
dengan memanfaatkan perkembangan teknologi seperti penggunaan perangkat
lunak dan website. Masalah yang terjadi di puskesmas adalah keterbatasan sarana
dan prasarana pendukung kegiatan pelayanan kefarmasian.

"Telah dilakukan Praktek Kerja Profesi Apoteker (PKPA) di Puskesmas Kecamatan Kembangan Jakarta Barat. Kegiatan PKPA ini berlangsung selama tiga minggu dari tanggal 4 sampai dengan tanggal 24 Agustus 2015. Tujuan dari kegiatan PKPA ini adalah agar mahasiswa Apoteker mampu memahami peranan, tugas dan tanggung jawab Apoteker dalam praktek pelayanan kefarmasian di Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku dan agar mahasiswa Apoteker memiliki pengetahuan, keterampilan, sikap perilaku, serta wawasan dan pengalaman nyata untuk melakukan praktik profesi dan pekerjaan kefarmasian di Puskesmas. Berdasarkan kegiatan PKPA yang dilakukan, diketahui bahwa Apoteker di Puskesmas Kecamatan Kembangan bertanggung jawab atas kegiatan pengelolaan obat dan bahan medis habis pakai dan pelayanan farmasi klinik. Pelayanan kefarmasian di Puskesmas Kecamatan Kembangan mangacu pada Permenkes No. 30 Tahun 2014 tentang standar pelayanan kefarmasian di Puskesmas.
Profession Internship at Puskesmas Kecamatan Kembangan Jakarta Barat has been done for three weeks from August 4 th until 24 th 2015. Puskesmas is community health centers. Profession Internship at this area was intended to make apothecary student understand the role and responsibility of pharmacist accordances to the statutory provisions and ethics, have insight into the implementation of pharmaceutical practice in Puskesmas, and know the issues in pharmaceutical practice in Puskesmas. Based on the activities, pharmacist in Puskesmas have been carrying out the duties and responsibilities in management of pharmaceutical preparation and consumable medical supply and in practice of clinical pharmacy. Pharmacy services in Puskesmas is carried out refers to Peraturan Menteri Kesehatan No. 30 Tahun 2015 about the standard of pharmacy services at Puskesmas."

"Praktek kerja profesi (PKP) di Puskesmas Kecamatan Kembangan Jakarta Barat bertujuan untuk memahami peranan, tugas dan tanggung jawab Apoteker di Puskesmas sesuai dengan ketentuan dan etika pelayanan kefarmasian serta memiliki wawasan, pengetahuan, keterampilan dan pengalaman serta gambaran nyata dalam melakukan praktek kefarmasian di Puskesmas. Berdasarkan kegiatan PKP di Puskesmas Kecamatan Kembangan, Apoteker memiliki peran penting dalam menjamin pelayanan kefarmasian di Puskesmas sesuai dengan standar. Secara umum praktek dan pelayanan kefarmasian di Puskesmas Kecamatan Kembangan telah diupayakan sesuai dengan Permenkes No. 30 tahun 2014. Pelayanan farmasi klinis di Puskesmas Kecamatan Kembangan masih belum berjalan optimal akibat kurangnya tenaga Apoteker yang berfokus pada pelayanan farmasi klinik. Kegiatan PKP di Puskesmas kecamatan Kembangan berlangsung selama 3 minggu dari tanggal 4 Agustus sampai dengan tanggal 24 Agustus2015 dengan tugas khusus yaitu patient medication record penyakit tidak menular di Puskesmas Kecamatan kembangan Jakarta Barat periode 2 Juli-20 Agustus tahun 2015.
Profession internship at Puskesmas kecamatan Kembangan West Jakarta was intended to make apothecary students understand the roles and responsibilities of pharmacists in accordance with the rules and ethics of pharmaceutical care; also have the insight, knowledge, skills and experience as well as know the issues of pharmaceutical care in Puskesmas. Based on the activities, Pharmacists have an important role in ensuring pharmaceutical care at Puskesmas accordance with the standard. In general, pharmaceutical care in Puskesmas kecamatan kembangan has been pursued with Permenkes No. 30 2014. The clinical pharmacy services in Puskesmas Kecamatan Kembangan are still not optimal due to the lack of the number of Pharmacists that focus on clinical pharmacy services. Profession internship at Puseksmas Kecamatan Kembangan was held for 3 weeks from August 4th until August 24th 2015 in a special assignment that is medication record of noncommunicable diseases patient in Puskesmas Kecamatan Kembangan on period 2nd July until 20th August 2015."

Community Health Centres (PHC) is a health care facility that organizes public health attempt and the attempt of individual health first rate, with more emphasis promotive and preventive attempt, to achieve the degree of public health is the highest in the working area. Profession Internship at Puskesmas Kecamatan Kembangan in West Jakarta on February 2016 aims to understand the role, job, and responsibilities of pharmacist in Puskesmas, gain knowledge, skill, attitude, perception, and real experience in pharmaceutical practices at the health center, find strategic ways to develop the role of pharmacist, have real illustration of any problem that occurred in pharmaceutical practices, and have capability to communicate with other health professionals. The profession internship program runs for 3 weeks in Puskesmas Kecamatan Kembangan with special assignment is study design for a book Drug Information Service."

pharmaceutical care, such as receipt assessment, dispensing, drug information,
and evaluation of drug use. Specific assignment that been given is about the

"Pusat Kesehatan Masyarakat (Puskesmas) adalah unit pelaksana teknis dinas kesehatan kabupaten/kota yang bertanggung jawab menyelenggarakan pembangunan kesehatan di suatu wilayah kerja. Berdasarkan Peraturan Menteri Kesehatan Nomor 30 Tahun 2014 tentang Standar Pelayanan Kefarmasian di Puskesmas bahwa pelayanan kefarmasian di Puskesmas merupakan satu kesatuan yang tidak terpisahkan dari pelakasanaan upaya kesehatan. Standar Pelayanan Kefarmasian di Puskesmas, yaitu standar pengelolaan obat dan bahan medis habis pakai serta pelayanan farmasi klinik. Pengelolaan obat dan bahan medis habis pakai meliputi kegiatan perencanaan kebutuhan, permintaan, penerimaan, penyimpanan, pendistribusian, pengendalian, pencatatan, pelaporan, pengarsipan, dan pemantauan serta evaluasi pengelolaan. Sedangkan pelayanan farmasi klinik meliputi pengkajian resep, penyerahan obat, dan pemberian informasi obat; Pelayanan Informasi Obat (PIO); konseling; ronde/visite pasien (khusus Puskesmas rawat inap); pemantauan dan pelaporan efek samping obat; pemantauan terapi obat; dan evaluasi penggunaan obat. Pelayanan kefarmasian di Puskesmas harus mendukung tiga fungsi pokok Puskesmas, yaitu sebagai pusat penggerak pembangunan berwawasan kesehatan, pusat pemberdayaan masyarakat, dan pusat pelayanan kesehatan strata pertama yang meliputi pelayanan kesehatan perorangan dan pelayanan kesehatan masyarakat.

Management of drugs and medical consumables material includes planning needs, request, receipt, storage, distribution, control, record keeping, reporting, archiving, and monitoring and evaluation of management. While clinical pharmacy services include the assessment of prescriptions, drug delivery, and the provision of drug information; Drug Information Service (PIO); counseling; round / visite patients (inpatient specialized health centers); monitoring and reporting drug side effects; monitoring drug therapy; and evaluation of drug use. Pharmacy services at the health center must support the three main functions of the health center, which is a central driver of health oriented development, community development centers, and health care centers that includes the first strata of personal health services and community health services."

"ABSTRAK
Praktek Kerja Profesi (PKP) di puskesmas bertempat di Puskesmas Kecamatan Cengkareng
Jakarta Barat. Kegiatan ini berlangsung selama tiga minggu dari tanggal 1 sampai dengan tanggal
25 September 2015. PKP di puskesmas bertujuan agar mahasiswa program studi apoteker
memahami peranan, tugas dan tanggung jawab apoteker dalam praktek pelayanan kefarmasian di
Puskesmas sesuai dengan ketentuan perundangan dan etika farmasi yang berlaku, dan dalam
bidang kesehatan masyarakat, memahami dan memiliki pengetahuan, keterampilan, sikap perilaku
serta wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian
di Puskesmas, serta melihat dan mempelajari strategi dan pengembangan praktek profesi Apoteker
di Puskesmas.Berdasarkan kegiatan PKP yang dilakukan, dapat disimpulkan bahwa Apoteker di
Puskesmas Kecamatan Cengkareng memiliki tanggung jawab terhadap pengelolaan sediaan
farmasi dan pelayanan farmasi klinik, meliputi pengkajian resep, penyerahan obat, pemberian
informasi obat, dan pelayanan informasi obat. Pengetahuan, keterampilan, sikap perilaku serta
wawasan dan pengalaman nyata untuk melakukan praktek profesi dan pekerjaan kefarmasian di
Puskesmas telah diperoleh penulis. Berdasarkan pengamatan selama pelaksanaan PKPA,
Puskesmas Kecamatan Cengkareng secara umum telah menerapkan seluruh aspek pelayanan
farmasi klinik dengan baik. Masalah yang terjadi di Puskesmas Kecamatan Cengkareng berupa
kurangnya tenaga Apoteker yang tidak seimbang dengan jumlahnya pasien. Selain itu, perlu

behaviors as well as insight into the real experience to practice apothecary profession and work in
health centers, as well as see and learn strategies and the development of professional practice of
pharmacists in community health center. Based on profession internship activities undertaken, it
can be concluded that pharmacists in Cengkareng sub-district health centers have responsibility for
the management of pharmaceutical and clinical pharmacy services, including assessment
prescription, drug delivery, give the information of drugs, and drug information services.
Knowledge, skills, attitudes and behaviors as well as insight into the real experience to practice
apothecary profession and work in health centers has been obtained by the authors. Based on
observations during the implementation, Cengkareng sub-district health centers in general have
implemented all aspects of clinical pharmacy services properly. Problems that occur in
Cengkareng sub-district health center in the form of shortage of pharmacists is not balanced with
the amount of patients. In addition, the holding space needs counseling so that counseling
activities run well.
