"ABSTRAK
Apotek adalah suatu sarana pelayanan kesehatan, tempat pengabdian
apoteker yang telah mengucapkan sumpah jabatan, tempat dilakukannya praktek
kefarmasian, dan tempat penyaluran perbekalan farmasi kepada masyarakat.
Apoteker sebagai penanggungjawab apotek memiliki peranan penting dalam
pelayanan kesehatan dalam bidang kefarmasian. Tujuan dilaksanakan PKP supaya
apoteker memahami tugas dan tanggung jawab apoteker di apotek, memperoleh
wawasan, pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan
praktek kefarmasian di apotek serta memperoleh gambaran nyata tentang
permasalahan praktek kefarmasian. Peserta PKP melakukan praktik kefarmasian
dan mendapatkan tugas khusus untuk melakukan pelayanan telefarma, yaitu
pelayanan kefarmasian melalui komunikasi jarak jauh, dalam hal ini melalui
telepon. Telefarma bertujuan untuk memantau kepatuhan pasien dan memantau
efek samping obat. Peserta PKP paham mengenai peran dan fungsi apoteker
dalam aspek manajerial, yaitu: melakukan pengawasan seluruh aspek pelayanan
kefarmasian, pengelolaan perbekalan farmasi dan perbekalan kesehatan dimulai
dari pengadaan, penerimaan, penyimpanan, pengelolaan, dan penyaluran sediaan
farmasi di apotek. Peserta PKP mendapatkan wawasan, pengetahuan,
keterampilan, dan pengalaman dalam pengkajian resep, penyerahan obat,
pelayanan informasi obat, konseling, pelayanan kefarmasian di rumah,
pemantauan terapi obat, dan pengawasan efek samping obat. Gambaran nyata
permasalahan yang sering terjadi di apotek adalah penolakan resep karena obat
tidak tersedia.
ABSTRACT The pharmacy is a health-care facilities, places pharmacist do practice
pharmaceutical care and the distribution medicine to public. Pharmacist as a
responsible pharmacist pharmacy has an important role in health care especially in
pharmacy. The goals of this internship are to make pharmacist student understand
jobs and duties as a pharmacist in pharmacy, to get knowledge, skills, and
experience abaout pharmaceutical care as well as to know about problems at
pharmacy. Student did pharmaceutical practice and got duties to do telepharmacy
service over the phone. The purpose of telepharmacy are to monitor patient
compliance and drug side effects. Student understand the role and function of the
pharmacist in the managerial aspects, namely: to supervise all aspects of
pharmacy services, management of pharmaceutical supplies and medical supplies
starting from procurement, receipt, storage, management and distribution
pharmaceutical. Student get knowledge, skills, and experience in the assessment
of prescriptions, compounding and dispensing, counseling, home pharmacy care,
monitoring drug therapy,and drug side effects. A real picture of the problems that
often occur in the pharmacy is rejection prescription because drug out.

"ABSTRAK
Praktek Kerja Profesi (PKP) di apotek bertempat di Apotek Kimia Farma No. 366
Depok. Kegiatan ini berlangsung selama lima minggu dari tanggal 1 sampai
dengan tanggal 31 Oktober 2015. PKP di apotek bertujuan agar mahasiswa
apoteker mengerti peranan Apoteker Pengelola Apotek (APA), memiliki wawasan
tentang pelaksanaan pekerjaan kefarmasian, dan memiliki gambaran nyata akan
permasalahan pekerjaan kefarmasian yang terjadi di apotek. Berdasarkan kegiatan
PKP yang dilakukan, APA di Apotek Kimia Farma No. 366 Depok secara umum
telah melaksanakan tugas dan tanggung jawab dalam pengelolaan apotek terkait
kegiatan teknis kefarmasian dan kegiatan nonteknis kefarmasian sesuai dengan
peraturan perundang-undangan yang berlaku. Mahasiswa apoteker telah berhasil
memperoleh wawasan mengenai kegiatan rutin yang dilakukan di apotek.
Kegiatan teknis kefarmasian yang dilakukan di apotek meliputi pengadaan,
penerimaan, penyimpanan, penjualan, pengelolaan narkotika, pengelolaan
psikotropika, dan dokumentasi resep. Adapun kegiatan nonteknis kefarmasian
yang dilakukan di apotek adalah penyusunan resep dan struk pembayaran,
penandaan harga pada resep kredit untuk penagihan, dan pembuatan Laporan
Ikhtisar Penjualan Harian (LIPH). Masalah yang terjadi di Apotek Kimia Farma
No. 366 Depok adalah ketiadaan Apoteker Pendamping yang dapat menggantikan
APA ketika sedang tidak ada di tempat.

"ABSTRAK
Praktik kerja profesi di Apotek Apotek Kimia Farma No.352 Depok pada bulan Bulan Oktober Tahun 2017 bertujuan untuk memahami tugas dan tanggung jawab apoteker dalam pengelolaan apotek, serta melakukan praktik pelayanan kefarmasian sesuai dengan ketentuan perundang-undangan dan etika yang berlaku, memiliki wawasan, pengetahuan keterampilan dan pengalaman praktis untuk melakukan praktek kefarmasian di apotek, memiliki gambaran nyata tentang permasalahan praktek kefarmasian serta mempelajari strategi dan kegiatan kegiatan yang dapat dilakukan dalam rangka pengembangan praktek kefarmasian. Praktik kerja profesi di Apotek Kimia Farma No.352 Depok dilakukan selama empat minggu dengan tugas khusus yaitu Pemberian Informasi Obat Pada Pasien Gagal Jantung Melalui Program Home Pharmacy Care. Tujuan dari tugas khusus adalah untuk memahami peran Apoteker dalam melakukan praktek pelayanan kefarmasian terutama home pharmacy care. Selain itu tugas ini juga bertujuan untuk memberikan pemahaman kepada pasien mengenai penyakit yang diderita, terapi yang diterima, dan terapi non farmakologi yang harus pasien lakukan.
ABSTRACT
Internship at Kimia Farma Pharmacy No.352 Depok at October 2017 aims to understand the duties and responsibilities of pharmacists in pharmacy management, as well as to practice pharmaceutical services in accordance with applicable laws and ethics, have the insight, knowledge, skills and practical experience to undertake pharmaceutical practices in pharmacies, can also have the insight of pharmaceutical practice issues and learn strategies and activities that can be undertaken in the course of pharmaceutical practice development. Internship at Kimia Farma Pharmacy No.352 Depok was conducted for four weeks with special assignment Providing Drug Information In Patients with Heart Failure Through Home Pharmacy Care Program. The purpose of the special assignment was to understand the role of the Pharmacist in performing pharmaceutical service practices, especially home pharmacy care. In addition, this assignment also aims to provide understanding to the patient about the illness, therapy received and non pharmacological therapy that patient should do"

"Praktik kerja profesi apoteker dilakukan di PT Dankos Farma, PT Anugerah Pharmindo Lestari, dan Apotek Kimia Farma 352 Margonda pada bulan Juli-Desember tahun 2021. Laporan Praktik Kerja Profesi Apoteker ini memberikan beberapa informasi terkait dengan peran apoteker dalam mengaplikasikan Annex 1 Good Manufacturing Practices (GMP) Guide tentang Manufacture of sterile drugs (GUI-0119) dan Cara Pembuatan Obat yang Baik (CPOB) 2018 Aneks 1 tentang Pembuatan Produk Steril, mengaplikasikan Corrective and Preventive Action (CAPA) dalam upaya pencegahan dan penyelesaian masalah, merancang aplikasi kualifikasi pelanggan untuk PBF Anugerah Pharmindo Lestari, dan melakukan proses praktik kefarmasian jarak jauh atau Telefarma. Laporan disusun menggunakan metode pengkajian atau penelusuran studi literatur dan wawancara dengan pasien atau narasumber yang dilakukan pada periode Juli – Desember tahun 2021. Wawancara dengan pasien atau narasumber dibimbing secara langsung oleh Pembimbing Lapangan (Pembimbing I) dan secara tidak langsung oleh Pembimbing II. Hasil dari beberapa kegiatan ini menunjukkan bahwa apoteker baik di industri farmasi, PBF, maupun apotek memiliki peran yang sangat penting dalam meningkatkan kualitas produk farmasi, pendistribusian produk farmasi, dan pelayanan kefarmasian. Selain itu, kegiatan ini penting dilaksanakan sebagai upaya untuk meningkatkan pengalaman dan pengetahuan seorang calon apoteker.
The professional practice of pharmacists was carried out at PT Dankos Farma, PT Anugerah Pharmindo Lestari, and Apotek Kimia Farma 352 Margonda from July-December 2021. This Pharmacist Professional Practice Report provides some information related to the role of pharmacists in applying Annex 1 Good Manufacturing Practices (GMP) Guide on Manufacturing of sterile drugs (GUI-0119) and Good Manufacturing Practices (GMP) 2018 Annex 1 on Manufacturing of Sterile Products, applying Corrective and Preventive Action (CAPA) in efforts to prevent and solving problems, designing customer qualification applications for PBF Anugerah Pharmindo Lestari, and carry out the process of remote pharmacy practice or Telefarma. The report was prepared using the study method or literature study search and interviews with patients or interviewees from July to December 2021. Interviews with patients or interviewees were guided directly by the Field Supervisor (Supervisor I) and indirectly by Supervisor II. The results of these activities indicate that pharmacists in the pharmaceutical industry, PBF, and pharmacies have a vital role in improving the quality of pharmaceutical products, distribution of pharmaceutical products, and pharmaceutical services. In addition, this activity is essential to increase the experience and knowledge of a prospective pharmacist."

"Praktik Kerja Profesi Apoteker di Apotek Kimia Farma Nomor 352 Margonda bertujuan untuk memberikan pengetahuan kepada mahasiswa sebagai calon apoteker mengenai tugas pokok dan tanggung jawab Apoteker Pengelola Apotek (APA) di apotek dan juga memberikan pengalaman praktis kepada calon apoteker dalam melaksanakan praktik kefarmasian di Apotek. Dengan adanya pelaksanaan PKPA ini, calon apoteker dapat memahami tugas dan tanggung jawab seorang apoteker sebagai manajer dan tenaga kesehatan profesional serta mendapatkan pengalaman praktis dalam melakukan praktik kefarmasian di apotek sesuai dengan perundang-undangan dan etika profesi yang berlaku. Tugas khusus yang diberikan pada praktik kerja ini bertujuan untuk melakukan analisis resep pada pasien hipertensi di Apotek Kimia Farma Nomor 352. Hasil dari tugas khusus ini menunjukkan bahwa resep yang diterima oleh pasien hipertensi tersebut telah tepat indikasi, efektif, dan aman untuk pasien.
Internship at Apotek Kimia Farma No 352 Margonda aims to provide knowledge to pharmacy student as prospective pharmacist about the main duties and responsibilities of responsible pharmacist in pharmacy and also provide practical experience to prospective pharmacist in pharmacy practice. Because of the implementation of this internship, prospective pharmacist can understand pharmacists duties and responsibilities as a manager and health professional as well as gain practical experience in accordance with applicable law and professional ethics. Special assignment given in this internship aims to analyse drug prescription which is reserved for hypertensive patient of Kimia Farma No 352. The result of this special assignment shows that the prescription received by that hypertensive patient is right indication, effective, and safe for patient."

During the implementation of the PKPA, a special task was also carried out, entitled " reating Standard Operating Procedure of Usage of Thickness Tester for Packaging Materials at PT Takeda Indonesia's Quality Control Department", "Implementation of the Customer Qualification Flow and Process in the APL Innovation Management System (IMS) Application Design at PT Anugerah Pharmindo Lestari", and "Implementation of Home Pharmacy Method for Patients with Referral Program (PRB) with Hypertension at Apotek Kimia Farma 352 Margonda". The preparation of this special task aims to identify, observe and analyze the implementation of pharmacist duties in related fields."

"Pelayanan Kefarmasian di Indonesia telah mengalami perubahan dari yang semula hanya berfokus kepada pengelolaan obat (drug oriented) berkembang menjadi pelayanan komprehensif meliputi pelayanan obat dan pelayanan farmasi klinik yang bertujuan untuk meningkatkan kualitas hidup pasien. Pelayanan kefarmasian di Apotek harus memenuhi standar yang ditetapkan oleh pemerintah berdasarkan peraturan menteri kesehatan nomor 35 tahun 2014. Apotek adalah sarana pelayanan kefarmasian tempat dilakukannya praktik kefarmasian oleh apoteker.
Pada laporan praktik kerja profesi ini dijelaskan pelaksanaan operasional dan pelayanan pada Apotek Kimia Farma No. 352 Margonda. Apotek kimia farma No. 352 Margonda sudah melaksanakan kegiatan sesuai dengan peraturan yang berlaku. Pelaksanaan praktik kerja profesi di Apotek Kimia Farma No. 352 Margonda dapat memberikan gambaran nyata kepada calon apoteker tentang pelaksanaan praktik kefarmasian di apotek. Mereka dapat meningkatkan pengetahuan dibidang kefarmasian dan dapat berkomunikasi dengan tenaga kesehatan lainnya.
In Indonesia, pharmaceutical services had been changed from drug oriented to patient oriented with the aim to improved the quality of life of patients. Pharmaceutical services at pharmacy should comply with goverment regulation, such as peraturan menteri kesehatan nomor 35 tahun 2014. Pharmacy is pharmaceutical care facility which where apothecary proffesion needs to do they pharmaceutical services.
This report is explain about implementation of operational and pharmaceutical service at Apotek Kimia Farma No. 352 Margonda. Generally, Apotek Kimia Farma No. 352 Margonda comply with goverment regulation. Internship at Apotek Kimia Farma No. 352 Margonda can provide real view to future apothecary about pharmaceutical care. They can improve their knowledge and able to communication with other health professional."
