"ABSTRAK
Telah dilakukan Praktek Kerja Profesi Apoteker (PKPA) di Rumah Sakit Pusat
Angkatan Darat Gatot Soebrot (RSPAD Gatsoe) Ditkesad. Kegiatan PKPA ini
berlangsung selama dua bulan dari tanggal 1 September sampai dengan tanggal 30
Oktober 2015. Tujuan dari kegiatan PKPA ini adalah agar mahasiswa Apoteker
mampu memahami peranan, tugas, dan tanggung jawab Apoteker di Rumah Sakit
sesuai dengan ketentuan dan etika pelayanan farmasi khususnya dan pelayanan
kesehataan umumnya dan agar mahasiswa Apoteker memiliki wawasan,
pengetahuan, keterampilan, dan pengalaman praktis untuk melakukan praktek
kefarmasian di Rumah Sakit. Berdasarkan kegiatan PKPA yang dilakukan,
diketahui bahwa Apoteker di RSPAD Gatsoe Ditkesad bertanggung jawab atas
kegiatan pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai
dan pelayanan farmasi klinik. Pelayanan kefarmasian di Puskesmas Kecamatan
Kembangan mangacu pada Permenkes No. 58 Tahun 2014 tentang standar

"ABSTRAK
Praktek Kerja Profesi di Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad dilaksanakan dengan tujuan untuk mengetahui dan memahami peran dan tanggung jawab apoteker sebagai pengelola instalasi farmasi, baik dalam bidang manajemen instalasi farmasi maupun dalam hal pelayanan farmasi klinik. Peran apoteker dalam manajemen instalasi farmasi yaitu pengelolaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai meliputi pemilihan, perencanaan, pengadaan, penerimaan, penyimpanan, pendistribusian, pemusnahan, pengendalian, pencatatan dan pelaporan. Sedangkan pelayanan farmasi klinik yang telah dilakukan yaitu pengkajian dan pelayanan resep, penelusuran riwayat penggunaan obat, rekonsiliasi obat, pelayanan informasi obat, konseling, visite, pemantauan terapi obat, monitoring efek samping obat, dan dispensing sediaan steril. Tugas khusus yang diberikan adalah analisis drug related problem pada penderita rawat inap dengan diagnosa fraktur mandibula sinistra dengan riwayat HIV/AIDS dan gangguan mental organik akibat toksoplasmosis di lantai 4 perawatan umum Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad untuk dapat memberikan rekomendasi terkait terapi pasien kepada tenaga kesehatan terkait.ABSTRACT Profession Internship at Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad was held to learn and understand the role and responsibility of pharmacist as a pharmacy manager, especially to manage a pharmacy or to deliver pharmaceutical care. "ABSTRAK
Praktek kerja profesi apoteker di rumah sakit pusat angkatan darat (rspad) Gatot soebroto
dilakukan selama periode September hingga oktober 2015. Praktek kerja ini bertujuan untuk
memahami peran dan tanggung jawab apoteker di rumah sakit sebagai bagian dari upaya dalam
meningkatkan pengetahuan, keterampilan, dan kemampuan calon apoteker sehingga nantinya
dapat menerapkan pelayanan kefarmasian di rumah sakit sesuai ketentuan yang berlaku serta
mampu berinteraksi langsung dengan pasien maupun bekerja sama dengan profesi kesehatan
lainnya. Tugas khusus yang diberikan pada praktek kerja profesi adalah analisa masalah terkait
obat pada pasien selulitis di ruang perawatan umum lantai V RSPAD Gatot Soebroto. Pemberian
tugas khusus bertujuan untuk melakukan pemantauan terapi obat pada pasien sehingga mampu
menganalisa masalah terkait obat yang diberikan pada pasien selulitis dan mampu memberikan
intervensi dan saran pengobatan terhadap terapi obat yang diberikan. Analisa dilakukan dengan
cara mengkaji data yang diperoleh dari hasil observasi langsung terhadap pasien, catatan
perkembanagan pasien terintegrasi (CPPT), hasil wawancara dengan pihak-pihak terkait serta
penelusuran literatur terkait. Analisa masalah dilakukan dengan menggunakan metode klasifikasi
menurut PCNE (Pharmaceutical Care Network Europe Foundation) V 6.2 2003-2010 dengan
temuan masalah pada domain penyebab : p1.1, p1.4, p5.1 dan p8.1. Intervensi yang dilakukan
selanjutnya sesuai klasifikasi PCNE adalah i1.2, i1.3, i2.1, i2.2, i2.4, i3.4, i3.5 dan i3.6. ABSTRACT Profession Internship at Indonesia Army Central Hospital (RSPAD) Gatot Soebroto had been held
on the periode of September-Oktober 2015. The internship aims to learn and understand the role
and responsibilities of pharmacists in hospital as part of efforts to enhance knowledge , skills , and
the prospective pharmacists which in turn can apply the pharmacy service in hospital based on
regulation and able to interact directly with patients and cooperate with other health professional.
The special assignment given to analysis drug related problem (DRP) in patients cellulitis in a
medical room PU floor V, Indonesia Army Central Hospital Gatot Soebroto. The special
assignments aims to monitor drug therapy in patients order to be able to analyze the drug related
problem being provided in patients cellulitis and able to provide intervention and treatment
suggestions.The analysis done by means of studying the data obtained from the results of direct
observation towards patients, integrated patient development note (CPPT) , interviewing the
parties involved and searching for the related literature. The analysis was conducted using the
classification methods according to PCNE (pharmaceutical care network europe foundation) v 6.2
2003-2010 with the findings problems in domain cause: p1.1, p1.4, p5.1 and p8.1.then the
interventions done in accordance PCNE classifications are i1.2, i1.3, i2.1, i2.2, i2.4, i3.4, i3.5 and
i3.6. "

Laporan praktek kerja ini berisi teori mengenai rumah sakit, instalasi farmasi rumah sakit, panitia farmasi dan terapi, pengelolaan perbekalan farmasi di rumah sakit, dan standar pelayanan kefarmasian di rumah sakit. Laporan ini memaparkan informasi mengenai RSPAD Gatot Soebroto Ditkesad, yaitu sejarah, profil, visi, misi, tugas fungsi, dan struktur organisasi secara umum. Laporan ini juga menguraikan hasil kegiatan praktek kerja di Instalasi Farmasi RSPAD Gatot Soebroto Ditkesad. Sebagai tugas khusus dipaparkan mengenai pemantauan terapi obat selama seminggu pada pasien yang mengalami pneumonia geriatr.
The objectives of pharmacist internship in Hospital are making sure that pharmacists understand and well knowledge of their role in hospital whether in pharmaceuticla management and clinical pharmacy. This report contains the theory about Hospital, Pharmacy installation, Commitee of pharmacy and theraphy, drug inventory management, and Standard Operation Procedure of Pharmaceutical care. This report also explain about RSPAD Gatot Soebroto Ditkesad: history,vision, mission, function, and organization’s structure generally. This report explains about pharmacist’s activity during internship in RSPAD Gatot Soebroto Ditkesad. As the special task, drug theraphy monitoring for 1 week in pneumonia geriatri patient is explained in this report."

"Praktek Kerja Profesi Apoteker di Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto Ditkesad bertujuan agar calon Apoteker memahami manajemen perbekalan farmasi dan pelayanan farmasi klinik di RSPAD Gatot Soebroto Ditkesad. Tugas khusus yang diberikan dengan judul Laporan Kasus Pasien CKD Stage 5 di Unit Perawatan Umum Lantai 4 Rumah Sakit Pusat Angkatan Darat (RSPAD) Gatot Soebroto Ditkesad Tanggal 12 Mei - 16 Mei 2014 bertujuan untuk mengetahui DRP (Drug Related Problem) yang terjadi berdasarkan terapi yang diberikan kepada pasien dan mengetahui peran Apoteker dalam mencegah keparahan CKD lebih lanjut.
Apothecary Profession Internship at Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad have a purpose to make student Apothecary understand pharmaceutical supply management and service of clinical pharmacy at Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad. A special assignment that is given by title case report Patients CKD Stage 5 in General Care Unit 4th floor Rumah Sakit Pusat Angkatan Darat Gatot Soebroto Ditkesad date May 12th - May 16th 2014 have a purpose to know the DRP (Drug Related Problem) that occur based on the therapy given to patients and to know the role of the pharmacist in preventing further CKD severity."

"ABSTRAK
Praktek Kerja Profesi Apoteker PKPA di Rumah Sakit Pusat Angkatan Darat RSPAD Gatot Soebroto Ditkesad bertujuan agar calon apoteker mampu memahami peranan, tugas dan tanggung jawab apoteker di rumah sakit dalam manajemen pengelolaan sediaan farmasi, alat kesehatan dan bahan medis habis pakai dan pelayanan farmasi klinik serta memberikan wawasan, pengetahuan, keterampilan dan pengalaman bagi calon apoteker untuk melakukan praktek pelayanan kefarmasian di rumah sakit. Tugas khusus yang diberikan mengenai identifikasi masalah terkait obat pada pasien ICU di RSPAD Gatot Soebroto Ditkesad dengan indikasi neglected fracture right femur, chronic kidney disease on hemodialisa, hipertensi dan diabetes mellitus. Tujuan dari tugas khusus ini adalah untuk memonitoring, mengidentifikasi, menganalisa dan memberikan rekomendasi solusi terkait masalah terkait obat, mempelajari cara berkomunikasi dengan tenaga kesehatan lain dan pasien/keluarga pasien serta memahami tugas dan peranan apoteker dalam bidang farmasi klinis di ruang perawatan. ABSTRACT
Internship at Central Army Hospital RSPAD Gatot Soebroto Ditkesad intended to make apothecary student be able to understand the roles, duties and responsibilities of pharmacist in hospital in the management of pharmaceutical preparations, medical devices and disposable medical products, and clinical pharmacy sevices along with giving insight, knowledge, skills and experience to apothecary student to practice pharmaceutical services in hospital. The given special task is identifying drug related problem on ICU patients at RSPAD Gatot Soebroto Ditkesad with signs of neglected right femur fracture, chronic kidney disease on hemodialysis, hypertension and diabetes mellitus. The purpose of this special task is to monitor, identify, analyze and provide recommendations for drug related problem, learn how to communicate with other health professionals and patients families as well as understanding the duties and roles of pharmacist in clinical pharmacy area in the treatment room.
