"Di Indonesia, masalah penyakit kanker merupakan masalah yang terus melonjak luar biasa, dalam jangka waktu 10 tahun terlihat bahwa peringkat kanker sebagai penyebab kematian naik dari peringkat dua belas menjadi peringkat empat ditahun 2005. Tumor di bidang urologi merupakan penyakit keganasan yang penting pada penyakit tumor secara keseluruhan, dan masih banyak masyarakat yang sulit untuk mencapai lokasi rumah sakit yang tersedia spesialisasi dibidang urologi baik karena faktor biaya, sulitnya trasportasi, jarak yang sangat jauh dan lainya.
Sebuah studi klinis prospektif yang dilakukan di bangsal urologi RS Dr. Sardjito Yogyakarta, kami mempelajari pasien urologi yang datang untuk berobat lebih dari 1 kali kunjungan setelah adanya gejala awal atau terlambat datang untuk berobat pada periode Juni 2012 – Januari 2013. Pengambilan Sampel dilakukan dengan Mengumpulkan data pasien sebanyak 40 orang dengan diagnosis keganasan di bidang urologi yang dirawat dan kontrol ke poli urologi, kemudian dilakukan wawancara dengan menggunakan kuesioner penelitian.
Penelitian dilakukan dengan metode cross-sectional, dinilai rasio prevalen yaitu membandingkan antara jumlah subjek dengan penyakit pada suatu saat dengan seluruh subjek yang ada. Sebuah studi observasional untuk menentukan hubungan antara faktor resiko dan penyakit. Seluruh metode mempergunakan program statistik SPSS versi 17.
Dilakukan analisis bahwa pada uji hubungan antara lamanya pasien berobat dengan variabel yang dimiliki responden, didapatkan sebanyak 27.5% responden mengaku pernah berobat ke paramedik, 85% responden berobat ke dokter umum dan 77.5% responden pernah berobat ke dokter spesialis, setelah pasien berobat ke tenaga medis (paramedik, dokter umum).
Didapatkan hubungan antar lama berobat dengan jumlah kunjungan p=0.037, didapatkan hubungan bermakna antara lama berobat dengan kunjungan ke dokter spesialis (p=0.001), Didapatkan hubungan bermakna antara lama berobat dengan berobat ke paramedik dengan nilai p = 0.017.

In Indonesia, malignancy is an increasing problem over the years. In the past ten years, it is observed that cancer-related mortality is increasing, from 12th position to 4th at the year of 2005. Urological cancer is one of important malignancy, yet still a lot of patients suffering these diseases couldn’t have access to hospital with attending urologists, due to cost problems, transportation problem, and others.
In one prospective clinical trial done at urological ward at dr. Sardjito hospital, we studied urologic patients who seek for help more than once after their symptoms became apparent, or did not come until their disease reach late stage at periode June 2012 until January 2013. Total 40 inward or outward patients diagnosed with urological malignancy were collected during the research, and an interview using research questionnaire was conducted for each patient.
This is a cross-sectional research, an observational study to determine the relationship between a disease and its risk factor. In this study we assess prevalence ratio; we compared subject with a disease at one time to the population. The whole analysis is using SPSS version 17 software.
We analyzed the correlation between time needed by the patient before seek medical help doctors between variables acquired by them. We got the result that 27,5% patients respond that they did go to the doctors before, 85% of them went to the general practitioners, and 77,5% of them went to specialist after they went to GP.
There is a correlation between time needed by patient to seek for medical help and visiting frequency (p=0,037). There is also a correlation between time needed by patient to go to the doctor and visiting frequency to urologists (p=0,001) and paramedics (p=0,017.

"Latar Belakang: Penyakit keganasan urogenital merupakan spektrum penyakit yang luas, dengan penatalaksanaan mulai dari observasi dan pemantauan ketat hingga pembedahan ekstirpatif mayor. Risiko malnutrisi praoperatif akibat kanker dan pascaoperatif akibat stres pembedahan akan meningkatkan risiko morbiditas dan mortalitas. Terapi nutrisi perioperatif yang adekuat bertujuan untuk menunjang perbaikan klinis dan status nutrisi, mendukung proses pemulihan, menurunkan risiko komplikasi pascaoperasi, serta menurunkan lama rawat di rumah sakit.
Metode: Laporan serial kasus ini menyajikan empat kasus kanker urogenital, terdiri dari dua kasus kanker buli, satu kasus kanker ginjal, dan satu kasus kanker penis. Tiga kasus termasuk kaheksia kanker, dan satu kasus termasuk pra-kaheksia. Seluruh pasien menjalani pembedahan urologi mayor dengan anestesi umum dan epidural. Terapi nutrisi perioperatif yang diberikan antara lain carbohydrate loading, nutrisi enteral dini pascaoperasi, serta pemberian nutrisi secara bertahap berdasarkan kondisi klinis. Dilakukan pemantauan yang meliputi keluhan klinis, antropometri, pemeriksaan fisik, hasil laboratorium, dan analisis asupan.
Hasil: Dua pasien mengalami ileus paralitik pascaoperasi dengan satu pasien di antaranya membutuhkan nutrisi parenteral total, dan dapat teratasi dalam 7 hari pascaoperasi. Satu pasien mengalami hiperglikemia reaktif dan diberikan terapi insulin, dapat teratasi dalam 7 hari pascaoperasi. Satu pasien mengalami perlambatan penyembuhan luka dan memiliki masa rawat pascaoperasi paling lama. Pasien yang mengalami ileus paralitik membutuhkan waktu lebih lama untuk mencapai target kalori 80 , namun seluruh pasien telah dapat mencapai target tersebut dalam 7 hari pascaoperasi. Lama perawatan pascaoperasi bervariasi, sekitar 10-27 hari.
Kesimpulan: Terapi nutrisi perioperatif yang diberikan pada keempat pasien menunjang perbaikan klinis dan status nutrisi, mendukung proses pemulihan, menurunkan risiko komplikasi pascaoperasi, serta menurunkan lama rawat di rumah sakit.

Background: Genitourinary malignancy represents a broad spectrum of disease, with treatments ranging from watchful waiting to major extirpative surgery. The risk of preoperative malnutrition due to cancer and postoperatively due to surgical stress will increase the risk of morbidity and mortality. An adequate perioperative nutrition therapy aims to support clinical and nutritional status improvement, hasten the recovery process, reduce the risk of postoperative complications, and decrease the length of hospital stay.
Method This case series report presents four cases of genitourinary cancers, consist of two cases of bladder cancer, one case of kidney cancer, and one case of penile cancer. Three cases are classified into cancer cachexia, and one case of pre cachexia. All patients had undergone major urological surgery under general and epidural anesthesia. Perioperative nutrition therapy provides carbohydrate loading, postoperative early enteral nutrition, as well as gradual nutrition based on clinical conditions. The monitoring given included clinical complaints, anthropometric measurement, physical examination, laboratory test results, and intake analysis.
Result Two patients had postoperative paralytic ileus with one patient requires total parenteral nutrition, and resolved within 7 days after surgery. One patient had reactive hyperglycemia and treated with insulin therapy, resolved in 7 days postoperative. One patient experienced impaired wound healing and had the longest postoperative care period. Patients with paralytic ileus may take more days to achieve 80 calorie target, yet all patients have been able to achieve the target in 7 days postoperative. The length of hospital stay after surgery was varied between 10 to 27 days.
Conclusion Perioperative nutrition therapy given to four patients in this case series leads to the clinical and nutritional status improvement, supports the recovery process, decreases the risk of postoperative complications, and shortens the hospital stays.

"Praktik residensi keperawatan medikal bedah kekhususan sistim perkemihan merupakan proses yang harus ditempuh untuk menjadi ners spesialis. Praktikan berperan sebagai care provider dalam memberikan asuhan keperawatan profesional dengan pendekatan model adaptasi Roy pada kasus utama gagal ginjal kronik stage V on hemodialisis dengan komplikasi subarachnoid hemoragic dan tiga puluh kasus perkemihan. Praktikan berperan sebagai peneliti dalam menerapkan tindakan keperawatan berbasis bukti ilmiah dengan topik holi Qur rsquo;an untuk menurunkan skore depresi dengan tahap awal skrining depresi. Peran sebagai inovator adalah dengan menerapkan model edukasi self instructional module SIM untuk meningkatkan pengetahuan dan kepatuhan pasien hemodialisis. Hasil praktik residensi adalah teori model adaptasi Roy efektif digunakan dalam memberikan asuhan keperawatan pada sistim perkemihan, holi Qur rsquo;an efektif menurunkan skore depresi dengan skrining awal menggunakan Beck Depression inventory II dan edukasi model SIM efektif meningkatkan pengetahuan dan kepatuhan pasien hemodialisis.

Surgical-medical nursing residency practice on the specificity of urinary system is a process which must be taken to become a nursing specialist. During the residency practice, the student played several roles. As a care provider, the student delivered professional nursing actions with Roy adaptation model theory on the main case of chronic renal failure stage V on haemodialysis with subarachnoid haemorrhagic complication and thirty urinary cases. As a researcher, the student applied scientific evidence-based nursing actions with holy Qur'an to decrease depression score. Depression screening was conducted as the initial stage. And as an innovator, the student applied self-instructional module SIM to improve haemodialysis patients knowledge and obedience. The results of the residency practice were Roy Adaption model was effective to deliver nursing care on urinary system, holy Qur rsquo;an was successful to decrease depression score with Beck Depression inventory II used in the initial screening , and SIM was also effective to improve haemodialysis patients rsquo; knowledge and obedience.
