"[ABSTRAK
Diabetes mellitus (DM) merupakan salah satu dari penyakit kronis. Penanganan DM tidak hanya membutuhkan pendekatan medis, tetapi juga membutuhkan pendekatan multidisiplin termasuk pendekatan keperawatan. Salah satu pendekatan keperawatan yang digunakan adalah teori self care Orem. Tujuan dari teori self care Orem ini adalah memberikan asuhan keperawatan untuk meningkatkan kemandirian pasien dalam perawatan diri. Ners spesialis adalah perawat yang memiliki keahlian dalam bidang klinik, mampu memberikan asuhan keperawatan kepada pasien secara langsung atau tidak langsung dengan mempengaruhi tenaga keperawatan dibawahnya. Ners spesialis dalam memberikan asuhan keperawatan harus berdasarkan pada evidence based nursing. Salah satu evidence based nursing pada pasien DM adalah diabetes self management education (DSME). Sebagai inovator, Ners spesialis melalukan inovasi promosi kesehatan pada pasien DM berupa pemeriksaan dan pemantauan kesehatan mandiri. Hasil dari penerapan teori self care Orem menunjukkan bahwa teori self care Orem dapat dijadikan sebagai panduan dalam pemberian asuhan keperawatan pada pasien DM.ABSTRACT Diabetes mellitus (DM) is one of the chronic diseases. "[ABSTRAK
Diabetes melitus merupakan sekelompok gangguan metabolisme tubuh, ditandai
dengan hiperglikemik kronis yang dapat mengakibatkan komplikasi akut dan
kronis. Salah satu peran perawat sebagai pemberi asuhan adalah memberikan
asuhan keperawatan yang berkualitas melalui pendekatan Teori Self Care Orem.
Penggunaan pendekatan teori ini diharapkan dapat membantu perawat dalam
menanggulangi keterbatasan yang dimiliki pasien dan melibatkan pasien secara
aktif dalam proses perawatannya melalui peilaku perawatan mandiri. Tujuan
penulisan karya ilmiah akhir ini adalah menganalisis hasil proses belajar residensi
keperawatan medikal bedah melalui penerapan asuhan keperawatan, penerapan
evidence based nursing practice, dan inovasi keperawatan. Data yang diperoleh
selama praktik klinik menunjukkan sebagian besar pasien diabetes melitus
mengalami komplikasi ulkus kaki diabetik yang disebabkan oleh perilaku
perawatan kaki mandiri belum optimal. Hasil penerapan evidence based nursing
practice berupa kegiatan edukasi perawatan kaki mandiri dapat meningkatkan
pengetahuan dan membentuk perilaku perawatan kaki sebagai tindakan
pencegahan dini. Hasil kegiatan inovasi keperawatan melalui pengkajian
kesehatan mandiri adalah pasien dapat melakukan pengkajian mandiri terhadap
kebutuhan edukasi dan mendapatkan edukasi sesuai kebutuhan mereka.
Kesimpulan dari karya ilmiah akhir ini adalah perlunya peningkatan kualitas
asuhan keperawatan pada pasien diabetes melitus melalui pendekatan teori keperawatan untuk meningkatkan kemandirian pasien dalam pengelolaan penyakitnya.ABSTRACT Diabetes mellitus is a metabolic disorder that characterized by chronic

"Diabetes mellitus merupakan penyakit kronik pada system endokrin yang paling banyak dijumpai dipelayanan kesehatan. Perawat dalam memberikan asuhan keperawatan dapat menggunakan berbagai metode pendekatan, salah satunya dengan model pendekatan Self Care Orem yang menggunakan pendekatan untuk memandirikan pasien diabetes melalui manajemen diabetes secara mandiri.Residensi keperawatan medikal bedah peminatan keperawatan endokrin bertujuan melakukan analisis terhadap penerapan model pendekatan Self Care Orem dalam melakukan kegiatan pemberian asuhan keperawatan. Selain itu juga dilakukan penerapan praktek berbasis bukti dengan pengkajian 3 menit kaki diabetesdi poliklinik penyakit dalam diharapkan dapat menjadi skrining terhadap pencegahan terjadinya ulkus diabetik. Selain penerapan Evidence Based Nursing Practice juga dilakukan penerapan inovasi promosi kesehatan dengan tujuan untuk memandirikan pasien dalam melakukan perawatan diri sehari-hari dalam mencegah terjadinya komplikasi diabeti yang dilakukan dipoliklinik penyakit dalam RS. Ciptomangunkusumo, Jakarta. Diharapkan dengan penerapan asuhan keperawatan dengan menggunakan pendekatan Self Care Orem, serta penerapan pengkajian 3 menit kaki diabetik dan promosi kesehatan dapat menjadi acuan dalam penerapan asuhan keperawatan yang komprehensif dan terintegrasi, sehingga pada akhirnya diharapkan dapat meningkatkan kulaitas asuhan keperawatan.
Diabetes mellitus is a chronic disease in the endocrine system of the most prevalent in health care system. Nurses in providing nursing care can use various methods for approach , one of them with models of Orem Self Care approach.
That uses approach for diabetic patients through independent diabetes management. Nursing specialist of Medical-Surgical practice for endocrine conduct an analysis of the application models of Orem Self Care approach in the provision of nursing care activities. It also made the application of evidencebased practice with an assessment of the diabetic foot in 3 minutes in outpatient
with diabetic, with the aims to be screened foot problems for prevention of diabetic ulcers.
In addition to the application of evidence -based nursing practice is also carried
out the implementation of inovation prgramme health promotion. The purpose of
this programme is promote patient ability to be independent for perform daily
self-care and prevention from diabetic complication in Cipto Mangunkusumo
Hospital at Jakarta. The cxpectation of implementation of nursing care by using Orem Self Care Approach, 3 minutes diabetic foot assessment and health promotion can be a reference in the implementation of nursing care is comprehensive and integrated , which in turn is expected to improve the quality of nursing care ."

"Praktik residensi di ruang ortopedi untuk memberikan asuhan keperawatan pada gangguan muskuloskeletal menggunakan teori keperawatan Self Care Orems, menerapkan praktik keperawatan berdasarkan pembuktian, dan melakukan inovasi keperawatan. Kasus kelolaan utama pada gangguan sistem muskuloskeletal adalah osteomielitis kronis. Masalah keperawatan paling banyak adalah nyeri dan mobilitas fisik untuk universal self care requisites serta kurang pengetahuan untuk developmental self care requisites. Intervensi keperawatan berdasarkan pembuktian yang telah diterapkan adalah cold compression therapy yang dapat meningkatkan proses penyembuhan dengan indikator penurunan nyeri dan edema, serta peningkatan rentang gerak sendi. Inovasi keperawatan yaitu penggunaan Clinical Practice Guidline untuk melakukan asuhan keperawatan berdasarkan clinical pathway pada pasca ORIF ekstremitas bawah. Perawat dapat menerapkan teori Self Care Orems, cold compression therapy, serta melakukan kegiatan inovasi untuk meningkatkan kompetensi perawat dan kualitas pelayanan keperawatan.
The advanced clinical practice in orthopaedic ward involved the planning of nursing care for musculoskeletal disorders patients with Orem?s Self Care theory, the implementation of evidenced based nursing practice, and the implentation of nursing innovations. The primary case management of the musculoskeletal system disorders is chronic osteomyelitis. Most nursing problems for the universal selfcare requisites are pain and physical mobility, while for the developmental self care requisites is knowledge deficit. The evidence based nursing intervention implemented was cold compression therapy. It can improve the healing process with the indicators of decreased pain and edema, and increased range of motion. The nursing innovation implemented was the use of Clinical Nursing Practice Guidline in giving nursing care based on the clinical pathways post-ORIF lower extremities. Nurses can apply the Orem?s Self Care Theory, cold compression therapy, and innovation activities to improve their competency and also quality of nursing services."

"Kanker merupakan pertumbuhan dari sel yang tidak terkendali dan dapat menyebar serta dapat menyebabkan kematian, sampai saat ini kanker menjadi masalah kesehatan baik di dunia maupun di Indonesia. Salah satu kanker yang sering terjadi pada wanita yaitu kanker servik. Pendekatan teori dalam asuhan keperawatan diperlukan dalam perawat pasien kanker. Karya Ilmiah Akhir merupakan laporam praktik residensi keperawatan medikal bedah peminatan onkologi di rumah sakit kanker Dharmais Jakarta.
Karya ilmiah ini terdiri dari: (1) penerapan teori peaceful end of life pada pasien kanker servik, (2) penerapan mengunyah permen karet meningkatkan motilitas usus pasca operasi ginekologi sebagai evidence based nursing serta (3) proyek inovasi kelompok yaitu pelatihan pengkajian edmonton symptom assessment system dan treatment.
Kesimpulan teori peaceful end of life tepat digunakan dalam perawatan paliatif pasien kanker. intervensi mengunyah permen karet dapat mempercepat motilitas usus pasca operasi dan pengajian edmonton symptom assessment system dan treatment dapat di aplikasikan dalam pengkajian pasien kanker dengan berbagai keluhan yang muncul.
Cancer is the growth of cells that are not controlled can spread and cause death, until now cancer health problem both in the world and in Indonesia. One of the most common cancers in women are cervical cancer. Theoretical approach in nursing care is needed in patients with cancer nurses. This scientific work consists of: (1) the application of the theory of peaceful end of life in patients with cervical cancer, (2) the application of chewing gum increases intestinal motility after gynecologic surgery as evidence based practice and (3) project groups, training assessment innovation edmonton symptom assessment system and treatment.
The conclusion of scientific work is theory peaceful end of life appropriate for use in the palliative care of cancer patients. Chewing gum intervention may accelerate postoperative intestinal motility and edmonton symptom assessment system and treatment can be applied in the assessment of cancer patients with a variety of complaints."

Pemenuhan perawatan diri yang dilakukan oleh perawat pada pasien yang imobilisasi
"Keperawatan sebagai suatu profesi dituntut untuk memberiken pelayanan yang terbaik kepada masyarakat. Untuk menjamin kepercayaan ini praktek keperawatan harus dilandasi ilmu pengetahuan, menggunakan metodologi keperawatan dan berlandaskan etika keperawatan. Inti praktek keperawatan adalah pemberian asuhan keperawatan. Asuhan keperawatan ditujukan kepada pasien yang tidak mampu, tidak tahu dan tidak mau, termasuk pasien dalam kondisi keterbatasan kemampuann untuk melakukan pergerakan. Asuhan keperawatan yang perlu diberikan pada pasien dengan keterbatasan pergerakan diantaranya adalah perawatan diri, sehingga kesejahteraan dan kenyamanan pasien dapat dipertahanka.n Namun pada kenyataannya hal ini belum diterapkan secara optimal, sehingga menambah permasalahan kesehatan baru bagi pasien
Memperhatikan hal tersebut peneliti tertarik untuk mengidentifikasi tingkat pemenuhan kebutuhan perawatan diri yang dilakukan oleh perawat pada pasien yang imobilisasi. Pcnelitian ini menggunakan metode deskriptif eksploratif pada 30 orang responden pasien yang dirawat di IRNA A Bedah Ortopedi RSUP Cipto Mangunkusumo Jakarta.
Setelah dilakukan pengumpulan dan pengolahan data terhadap 30 kuesioner didapatkan hasil sebagai berikut : Dari 18 pertanyaan tentang pemenuhan perawatan diri, rata - rata pemenuhan perawatan diri yang diberikan berada pada nilai 0,995 - 1,885 yang artinya pemenuhan perawatan diri pada pasien berada pada rentang kurang baik sampai baik. Untuk pemenuhan mandi dan perawatan kulit, toileting, serta berpakaian dan tampil rapi sudah baik, sedangkan untuk pemenuhan perawatan kaki dan kuku, perawatan rambut dan perawatan mata masih kurang baik.
Kesimpulan dari penelitian ini adalah pemenuhan perawatan diri yang dilakukan perawat belum optimal, sehingga perlu optirnalisasi pada masa mendatang. Apabila pemenuhan pcrawatan diri ini tidak dioptimalkan, maka pennasalahan yang timbul akibat imobilisasi temtama gangguan integritas mit akan tetap terjadi. Akibatnya akan menambah permasalahan kesehatan baru bagi pasien."

Gangguan pada sistem endokrin merupakan masalah kesehatan yang cukup tinggi di Indonesia, khususnya Diabetes Melitus (DM) tipe 2 dengan prevalensinya mencapai 2,1% dari total penduduk, dengan 60% dari total prevalensi adalah kasus neuropati. Kadar glukosa darah yang tidak terkontrol menyebabkan tingginya kejadian komplikasi DM baik akut maupun kronis. Pencegahan komplikasi tersebut memerlukan penanganan yang tepat dari berbagai multidisiplin ilmu, salahsatunya adalah peran perawat ners spesialis medikal bedah kekhususan endokrin sebagai praktisi keperawaan untuk ikut memberikan asuhan melalui berbagai pendekatan teori keperawatan seperti penerapan model adaptasi Roy secara holistik menggunakan pengkajian perilaku dan stimulus sehingga tercapai perilaku adaptif. Selain itu penerapan praktek keperawatan berbasis pembuktian ilmiah (evidence based nursing) seperti senam kaki diabetes dapat menjadi acuan intervensi dalam mencegah terjadinya polineuropati dan komplikasi lanjut seperti ulkus diabetik. Sebagai agen pembaharu/inovator, perawat spesialis juga membuat inovasi tentang self health assesment dan booklet sahabat diabetes bagi para diabetesi untuk meningkatkan promosi kesehatan terhadap pengelolaan dan penatalaksanaan DM.
