"Latar Belakang: Gagal napas akut merupakan kondisi mengancam nyawa dan penyebab kondisi ini seperti pneumonia dan edema paru harus segera diketahui sehingga terapi yang sesuai dapat dilakukan. Berbagai panduan klinik untuk kasus gagal napas akut tidak menempatkan USG paru sebagai salah satu modalitas diagnostik. Meta-analisis ini dibuat dengan tujuan mengetahui akurasi USG paru dengan protokol BLUE dalam mendiagnosis penyebab gagal napas akut.
Metodologi: Pencarian secara sistematik dilakukan pada studi potong lintang yang membandingkan akurasi diagnosis protokol BLUE dengan pemeriksaan baku emas untuk setiap diagnosis penyebab gagal napas. Pencarian studi dilakukan dari enam database online yaitu Pubmed/MEDLINE, Embase, Cochrane Central, Scopus, Ebscohost/CINAHL dan Proquest dan sumber gray literature lain pada tanggal 6- 7 September 2020. Penulis melakukan ekstraksi data secara manual dari studi yang memenuhi eligibilitas dan melakukan analisis untuk mendapatkan data gabungan dari sensitivitas, spesifisitas, likelihood ratio dan diagnostic odds ratio.
Hasil: 509 artikel didapatkan dari pencarian studi dengan total akhir 4 artikel yang memenuhi kriteria eligibilitas dan dapat dilakukan meta-analisis. Tidak ada risiko bias yang serius pada 4 studi ini berdasarkan penilaian risiko bias QUADAS-2. Keempat artikel ini mempunyai jumlah total sampel sebesar 770 individu. Dari 4 studi tersebut, 3 studi melakukan penelitiannya di ICU dan 1 studi di IGD. Sebagian besar studi menggunakan operator yang bersertifikasi dan mempunyai pengalaman cukup, sedangkan 1 studi menggunakan operator yang baru diberikan pelatihan. Sensitivitas, spesifisitas, LR+, LR-, DOR protokol BLUE dalam mendiagnosis pneumonia adalah 84% (95% CI, 76-89%), 98% (95% CI, 93-99%), 42 (95% CI, 12-147), 0.12 (95% CI, 0.07-0.2) dan 252 (95% CI, 81-788) secara berurutan. Sensitivitas, spesifisitas, LR+, LR-, DOR protokol BLUE dalam mendiagnosis edema paru adalah 89% (95% CI, 81-93%), 94% (95% CI, 89-96%), 14 (95% CI, 8-25), 0.165 (95% CI, 0.11-0.24) dan 116 (95% CI, 42-320) secara berurutan.
Kesimpulan: Hasil meta-analisis ini menunjukan bahwa protokol BLUE mempunyai sensitivitias dan spesifisitas yang tinggi dalam mendiagnosis pneumonia dan edema paru pada pasien gagal napas akut. Penulis merekomendasikan penggunaan protokol BLUE pada setiap pasien dengan gagal napas akut.

Background: Acute respiratory failure (ARF) is a life-threatening condition. ARF can be caused by variety of pathological conditions such as pneumonia or pulmonary oedema and knowing the etiology of ARF is a vital component in managing ARF. Bedside lung ultrasound in Emergency (BLUE) protocol rarely used for assessing lung pathologies despite multiple studies have shown its reliable performance.
Methods: We conduct meta-analysis to compare diagnostic accuracy of BLUE protocol with gold standard for each diagnosis. Systematic search was done in 6 databases (Pubmed/MEDLINE, Embase, Cochrane Central, Scopus, Ebscohost/CINAHL dan Proquest) and multiple gray-literature sources for crosssectional studies that fulfill eligibility criteria. We manually extracted the data from eligibile studies and calculated pooled sensitivity, pooled specificity, likelihood ratio and diagnostic odds ratio. We follow PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) guideline throughout these processes.
Results: Four studies, from total 509 articles collected, containing 770 subjects were included in this meta-analysis. Pooled sensitivity, pooled specificity, LR+, LR-, DOR of BLUE protocol in diagnosing pneumonia were 84% (95% CI, 76- 89%), 98% (95% CI, 93-99%), 42 (95% CI, 12-147), 0.12 (95% CI, 0.07-0.2) and 252 (95% CI, 81-788), respectively. Pooled sensitivity, pooled specificity, LR+, LR-, DOR of BLUE protocol in diagnosing pulmonary oedema were 89% (95% CI, 81-93%), 94% (95% CI, 89-96%), 14 (95% CI, 8-25), 0.165 (95% CI, 0.11-0.24) and 116 (95% CI, 42-320), respectively.
Conclusions: BLUE protocol has good diagnostic accuracy to diagnose pneumonia and pulmonary oedema. We recommend implementing BLUE protocol as a tool in evaluating cause of ARF.
