ABSTRAKRuang Lingkup dan Cara Penelitian: Merokok adalah salah satu penyebab Penyakit Paru Obstruktif Menahun (PPOM). PPOM bersifat ireversibel sehingga perlu pencegahan dan penanganan lebih dini. Obstruksi saluran nafas pada PPOM dimulai pada saluran nafas kecil, sehingga belum menimbulkan keluhan dan gejala paru. Obstruksi saluran nafas kecil memperlihatkan penurunan nilai V25 pada pemeriksaan kurva 'flow volume'; pada pemeriksaan faal ventilasi paru lain seperti FVC, FEV1, MMF dan PFR belum terlihat.
Penelitian ini bertujuan untuk mengetahui apakah pada perokok tanpa keluhan dan gejala penyakit paru telah ditemukan obstruksi saluran nafas yang dapat dilihat dengan pemeriksaan 'flow volume' dan pemeriksaan faal ventilasi lainnya. Pemeriksaan dengan alat Autospirometer AS500 dilakukan pada 132 orang pria berumur 30 - 39 tahun, yang dibagi dalam 4 kelompok: 33 orang tidak merokok / merokok ≤ 1 batang/hari (kontrol), 33 orang perokok ringan (merokok 5-10 batang/hari), 33 orang perokok sedang (merokok 10-20 batang/hari) dan 33 orang perokok berat (merokok > 20 batang/ hari). Masing-masing perokok telah merokok selama 10-15 tahun. Sebelum pemeriksaan kurva 'flow volume', OP harus memenuhi syarat antara lain pengisian kuesioner, pemeriksaan fisik dan rontgen foto torak.
Hasil dan Kesimpulan: Pada pemeriksaan kurva 'flow volume' didapatkan penurunan bermakna untuk nilai V50 (p
0,05).Dari basil pemeriksaan tersebut dapat diambil kesimpulan bahwa pada perokok ringan, sedang, maupun berat, yang telah merokok selama 10 - 15 tahun, tanpa keluhan dan gejala penyakit paru, telah didapatkan obstruksi saluran nafas kecil.
ABSTRACTScope and Method of Study: Smoking is one of the causes of Chronic Obstructive Pulmonary Disease (COPD). COPD is irreversible so that early precautionary measures and prevention must be taken. The obstruction in COPD begins in the small airways, but at the beginning shows no complaint. Obstruction of the small airways cause a decrease in V25 on the flow volume curve, while other pulmonary function tests such as FVC, FEV1, MMF and PFR are still normal.The purpose of this study is to reveal whether smokers who have no complaints and symptoms of pulmonary disease already have changes in pulmonary function, which can be detected by flow volume curve and other pulmonary function tests. Pulmonary function was assessed with Autospirometer AS 500 on 132 males, age 30 - 39 years. They were divided into 4 groups: 33 nonsmokers / control (smokes ≤ 1 cigarette/day), 33 mild smokers (smokes 5-10 cigarettes/day), 33 moderate smokers (smokes 10-20 cigarettes/ day), and 33 heavy smokers (smokes > 20 cigarettes/day). Each have been smoking cigarettes for 10-15 years. Questionnaires, physical examination, and chest X-ray were done prior to carrying out the pulmonary function tests.Findings and Conclusions: The flow volume curve showed a significant decrease in V50 {pFor V50, there was a significant decrease for the heavy smokers compared to control group and the mild smokers. No decrease was observed for V75, FVC, FEVI, FEV1%, MMF and PFR (p>0.05).
The conclusion from this study is that mild smokers, moderate smokers, and heavy smokers, who have been smoking for 10 - 15 years, without complaints and symptoms of pulmonary disease, al-ready have obstruction of the small airways."
